Leiomyosarcoma of the abdominal aorta: a rare cause of renovascular hypertension.

We describe the case of a 44-year-old woman who presented with renovascular hypertension caused by primary leiomyosarcoma of the abdominal aorta that had metastasized into the renal arteries. Despite an extensive radiological evaluation, the diagnosis was mistaken first for Takayasu's arteritis and then for retroperitoneal hematoma or neoplasm. The patient developed renal failure due to bilateral renal infarction, and died 3 months after her initial presentation with ischemic colitis. Postmortem examination confirmed the diagnosis.

Clinicopathologic Features and Diagnostic Challenges of Mycophenolate Mofetil-Associated Enterocolitis in Lung Transplant Recipients.

Mycophenolate mofetil (MMF) is a commonly used immunosuppressant for lung transplant recipients. Although it efficiently suppresses transplant rejection and improves graft function, MMF has been found to occasionally cause gastrointestinal (GI) injury. The symptoms are nonspecific and include diarrhea, nausea, vomiting, and rare cases of bleeding. Endoscopic findings range from normal appearance to erythema to ulcers. The histopathologic features mimic several different disorders like graft-versus-host disease (GVHD), cytomegalovirus (CMV) colitis, ischemic colitis, or inflammatory bowel disea…

Pathologic features of gastrointestinal tract lesions in childhood-onset systemic lupus erythematosus: study of 26 patients, with review of the literature.

A review of the gastrointestinal tracts of 26 autopsied cases of childhood-onset systemic lupus erythematosis (SLE) showed the single most common pathologic finding to be chronic nonspecific mucosal infiltration (96%). The most frequent general category was ischemic bowel lesions, attributable to vascular lesions of SLE (60%) or to nonocclusive causes of circulatory insufficiency such as congestive heart failure, uremia, or shock (40%). Complications of ischemic bowel lesions included secondary invasion by opportunistic organisms such as Candida (9 patients), pneumatosis intestinalis (1), and

Toxic megacolon.

Toxic megacolon represents a dreaded complication of mainly inflammatory or infectious conditions of the colon. It is most commonly associated with inflammatory bowel disease (IBD), i.e., ulcerative colitis or ileocolonic Crohn's disease. Lately, the epidemiology has shifted toward infectious causes, specifically due to an increase of Clostridium difficile-associated colitis possibly due to the extensive (ab)use of broad-spectrum antibiotics. Other important infectious etiologies include Salmonella, Shigella, Campylobacter, Cytomegalovirus (CMV), rotavirus, Aspergillus, and Entameba. Less freq

Ischemic and hemorrhagic abdominal complications in COVID-19 patients: experience from the first Italian wave.

PURPOSE: To report ischemic and haemorrhagic abdominal complications in a series of COVID-19 patients. To correlate these complications with lung involvement, laboratory tests, comorbidities, and anticoagulant treatment. METHODS: We retrospectively included 30 COVID-19 patients who undergone abdomen CECT for abdominal pain, between March 16 and May 19, 2020. Ischemic and haemorrhagic complications were compared with lung involvement (early, progressive, peak or absorption stage), blood coagulation values, anticoagulant therapy, comorbidities, and presence of pulmonary embolism (PE). RESULTS:

Peripheral Artery Disease Diagnosis is an Independent Predictor Factor for Reduced Long-Term Survival after Fenestrated/Branched Endovascular Aortic Repair.

BACKGROUND: This study aimed to investigate the outcomes of fenestrated/branched endovascular aortic repair (F/BEVAR) patients comparing individuals with and without a prior diagnosis of peripheral artery disease (PAD). METHODS: A single-center retrospective analysis was performed, including patients with the diagnosis of complex aortic aneurysms treated with an F/BEVAR procedure. The diagnosis of PAD was determined by an ankle-brachial index <0.9 or a toe-brachial index <0.7 in nondiabetic and diabetic patients, respectively. The ischemia grade of the WIFi classification was applied to categ

Preoperative leukocytosis among female patients predicts poor postoperative outcomes following endovascular aneurysm repair for intact infrarenal abdominal aortic aneurysms.

OBJECTIVES: Elevated white blood cell count (WBC) can be predictive of adverse outcomes following vascular interventions, but the association has not established using multi-institutional data. We evaluated the predictive value of preoperative WBC after endovascular aneurysm repair (EVAR) for nonruptured abdominal aortic aneurysms (AAAs) in a nationally representative surgical database. METHODS: Patients with nonruptured AAA undergoing EVAR were identified in the vascular-targeted National Surgical Quality Improvement Program (NSQIP) database. Baseline characteristics were compared between pa

Pembrolizumab-induced immune-mediated fatal colitis with concurrent giardia infection.

Advancements in medicine have enabled the use of monoclonal antibodies in the field of oncology. However, the new adverse effects of immunotherapeutic agents are still being reported. We present the first case of pembrolizumab-induced fatal colitis with concurrent Giardia infection in a patient with metastatic ovarian cancer. A 47-year-old woman with metastatic ovarian cancer who was being treated with pembrolizumab admitted to our clinic complaining of persisting bloody diarrhoea. Her stool antigen test was positive for Giardia. The patient received metronidazole. A colonoscopy with mucosal b

Stercoral colitis: CT imaging findings and clinical risk factors.

PURPOSE: To describe and update stercoral colitis clinical risk factors, relative frequency, location, and CT imaging features correlated with surgical and pathological results. METHODS: CT reports over a 5-year period (05/2017-05/2022) at a single medical center were searched. Main inclusion criteria were luminal distention with formed stool, wall thickening, and surrounding inflammation. Positive cases were graded as mild (early or developing stercoral colitis) versus moderate-to-severe based on CT findings. Medical records were reviewed for risk factors and outcome data in moderate-to-seve

MDCT in ischaemic colitis: how to define the aetiology and acute, subacute and chronic phase of damage in the emergency setting.

Ischemic colitis (IC) is the most common vascular disorder of the gastrointestinal tract with a reported incidence of 6.1-44 cases/100,000 person years with confirmatory histopathology. However, the true incidence of IC poses some difficulty, and even vigilant clinicians with patients at high risk often miss the diagnosis, since clinical presentation is non-specific or could have a mild transient nature. Detection of IC results is crucial to plan the correct therapeutic approach and reduce the reported mortality rate (4-12%). Diagnosis of IC is based on a combination of clinical suspicion, rad

Colitis possibly induced by quetiapine.

A 39-year-old man with bipolar disorder was hospitalised for depression. He was started on quetiapine (titrated up to 300 mg), lactulose (a laxative) and tropatepine (an anticholinergic). Valpromide (a mood stabiliser) and prazepam were later added and rapidly withdrawn. Seven days after quetiapine initiation, the patient reported abdominal pain and constipation; 2 days later, CT revealed an important distention of the colon including the caecum and a pre-perforation. A subtotal colectomy was performed and histology confirmed necrotising ischaemic colitis. The patient survived. This is the fir

New histologic findings in idiopathic mesenteric phlebosclerosis: clues to its pathogenesis and etiology--probably ingested toxic agent-related.

BACKGROUND: Idiopathic mesenteric phlebosclerosis (IMP) is a recently known and rare disease entity, which is a member of non-thrombotic, non-inflammatory stenosis or occlusion of the mesenteric veins. In spite of the unique histopathology and particular location, the cause and pathogenesis of IMP remain unknown. The aim of this brief study was to propose a pathogenesis and possible etiology based on the reviewed clinical data and some newly discovered pathologic findings in several recent cases in our and other hospitals. METHODS: The clinical data of 5 patients were collected, with detailed

Rhabdomyolysis and necrotic bowel after acetaminophen and ibuprofen overdose.

A 28-year-old man with schizophrenia intentionally ingested a lethal dose of acetaminophen and an unknown quantity of ibuprofen. He arrived at the hospital with acute renal and fulminant liver failure complicated by rhabdomyolysis. His creatine kinase level was 20,306 U/L on admission, which increased to 245,595 U/L by hospital day 2, and subsequently decreased to 339 U/L by day 16. The patient underwent liver transplantation on day 3; necrotic bowel was found during surgery. Rhabdomyolysis associated with acetaminophen overdose has been described only in a few case reports, but rarely in asso

Rare causes of acute abdomen and review of literature: Primary/secondary omental torsion, isolated segmental omental necrosis, and epiploic appendagitis.

BACKGROUND: Primary/secondary omental torsion (POT/SOT), isolated segmental omental necrosis (ISON), and primary epiploic appendagitis (PEA) are rare causes of acute abdominal pain that often lead to misdiagnosis. Although the differential diagnosis may vary according to the localization and severity of the pain, and associated symptoms such as nausea-vomiting and fever, depending on the disease and site of involvement, the character of the pain is generally similar and in a continuous form. Depending on pain localization, it can mimic different clinical pictures such as acute appendicitis, ac

[NSAID-colonopathy].

The pain-relief properties of NSAID/ASA preparations have been known for a long time. In particular, the gastrointestinal tract shows side effects such as: erosion, ulceration, and even perforation. In the upper gastrointestinal tract, our group has shown that a correct histological diagnosis of NSAID/ASA induced lesions can be made in a high percentage of cases on the basis of recognition of ischemic necrosis. NSAID/ASA induced lesions are less commonly found in the lower gastrointestinal tract. We could also demonstrate a correct histological diagnosis of NSAID-colonopathy on the basis of fi

Ischemic proctosigmoiditis.

UNLABELLED: Rectal ischemia is rare because of excellent collateral supply. Although rectosigmoid ischemia is usually accompanied by more proximal colonic involvement, it may occur alone. METHODS: A retrospective review of all patients diagnosed as having colonic ischemia at the Mayo Clinic from 1976 to 1991 was performed. Clinical, endoscopic, radiological, and pathological data were obtained from patient charts. Patients with involvement of the rectosigmoid colon extending to no more than 30 cm above the dentate line on endoscopy were included in the study. A single radiologist reviewed CT

Submucosal hematoma producing mechanical obstruction in the sigmoid colon, complicated by systemic amyloidosis.

A case of systemic amyloidosis involving the gastrointestinal tract is presented. The initial manifestation of this case was mechanical obstruction. On laparotomy, a submucosal hematoma of the sigmoid colon which completely obliterated the lumen, was found. With intense medical treatment, the obstructed lumen became patent, but segmental ischemic colitis ensued. The terminal course of this case was complicated by chronic renal failure, upper gastrointestinal bleeding and coagulopathy. Pathological examination of the stomal specimen revealed massive amyloid deposits in the wall of the large int
