The impact of radioimmunoguided surgery (RIGS) on surgical decision-making in colorectal cancer.

Radioimmunoguided surgery (RIGS system) was performed in ten patients with rectal or low sigmoid colon carcinoma with the use of a hand-held gamma detector (Neoprobe 1000) intraoperatively and externally after injection of radiolabeled (125I) monoclonal antibody to detect pelvic and metastatic tumor. Fifteen procedures, including six exploratory laparotomies, four transperineal explorations, two transsacral explorations, one transvaginal biopsy, one brachytherapy, and one transanal polypectomy, were performed. Two patients had previous low anterior resection, seven abdominoperineal resection,

Surgical Education Visual Gaps: Anorectal Pathology Image Diversity in Colorectal Surgery Textbooks.

INTRODUCTION: We sought to evaluate the representation of different skin tones depicted in images of anorectal pathology found in colorectal surgery textbooks. METHODS: Nonillustrated photographic images depicting anorectal pathology were identified and analyzed from colorectal surgery textbooks. Images were classified based on category and pathology. The Fitzpatrick skin phenotype scale and constitutive skin color scale were used to evaluate each image. Fitzpatrick phototypes I-III represented skin of noncolor and phototypes IV-VI represented skin of color. The skin tone distribution was ass…

Defining Volume Thresholds for Colorectal Cancer Surgery Using Stratum Specific Likelihood Ratios.

BACKGROUND: Higher procedure volume correlates with improved surgical outcomes, yet recent literature has questioned whether this relationship persists in modern times. Reports of an attenuated volume-outcome association may reflect the limitations of a single, static, cutoff rather than true weakening of the underlying relationships. Stratum-specific likelihood ratios generate multiple empirically derived volume strata that may detect persistent gradients that are obscured by binary threshold approaches. OBJECTIVES: (1) To define hospital-level volume thresholds for colorectal cancer resecti

Hospital volume matters: a meta-analysis of mortality after colorectal cancer surgery.

BACKGROUND: Postoperative mortality serves as a critical indicator for assessing surgical safety in patients undergoing colorectal cancer surgery; however, the relationship between annually hospital surgical case volume and postoperative mortality remains unclear. Here, we aimed to investigate the volume-outcome relationship for colorectal cancer surgery and to identify potential thresholds for high-volume hospitals. METHODS: Studies reporting the association between hospital volume and postoperative mortality in colorectal cancer patients undergoing colorectal cancer surgery were systematica

Prevalence and Risk Factors of Acute Kidney Injury After Colorectal Cancer Surgery.

PURPOSE: Acute kidney injury is a sentinel event affecting colorectal cancer patients either as a consequence of surgery or systemic chemotherapy. It is highly correlated with both short and long-term adverse outcomes. This work aimed to study the prevalence, risk factors, and impact on survival of postoperative (PO-AKI) and post-chemotherapy (PC-AKI) after colorectal cancer (CRC) surgery in Egyptian patients. METHODS: Data of the patients with CRC who underwent surgery over the previous 5 years was retrieved from an internet-based medical system. The incidence of PO-AKI and PC-AKI was calcul

External Lymphatic Fistula After Radical Surgery for Colorectal Cancer: A Case Series.

BACKGROUND: The incidence of external lymphatic fistula (ELF) represents a relatively rare complication after surgery for colorectal cancer, especially in Western countries. However, the rate of this complication is progressively increasing following the introduction of complete mesocolic excision and central vascular ligation with consequent extensive lymphadenectomy. There are no guidelines for the management of ELF, with therapeutic options varying from conservative procedures to more invasive surgeries. The aim of this study was to retrospectively quantify the rate of ELF after surgery for

Iatrogenic Ureteral Injury and Prophylactic Stent Use in Veterans Undergoing Colorectal Surgery.

INTRODUCTION: Iatrogenic ureteral injury (IUI) is an uncommon complication in colorectal surgery. Prophylactic ureteral stenting (PUS) gained acceptance to aid in intraoperative identification of the ureter. Despite its use, the benefit of pus to avoid IUI remains debatable. We sought to analyze the rates of IUI after colorectal surgery in veterans and to compare the outcomes after PUS using a large matched cohort. METHODS: The veterans affairs surgical quality improvement program database was queried for patients who underwent colorectal surgery from 2008-2015. To analyze the outcomes of PUS…

Liver Injury Among Japanese Patients Treated Using Prophylactic Enoxaparin After Colorectal Surgery.

BACKGROUND: Enoxaparin, a low molecular weight heparin, has been used to prevent thrombotic events during major surgery without increasing the rate of hemorrhage. On the other hand, it was reported to cause liver injury, but the details of liver injury induced by prophylactic enoxaparin after abdominal surgery remain unclear. AIMS: This study aimed to clarify the relationship between prophylactic enoxaparin and liver injury after colorectal surgery, and characterize the injury profile. METHODS: We retrospectively reviewed 732 Japanese patients who underwent elective resection of the colorect…

Intraoperative core temperature and infectious complications after colorectal surgery: A registry analysis.

STUDY OBJECTIVE: Moderate hypothermia (e.g., 34.5 °C) causes surgical site infections, but it remains unknown whether mild hypothermia (34.6 °C-35.9 °C) causes infection. Therefore, the objective of this study was to evaluate the relationship between intraoperative time-weighted average core temperature and a composite of serious wound and systemic infections in adults having colorectal surgery over a range of near-normal temperatures. DESIGN: Retrospective, single center study. SETTING: The operating rooms of the Cleveland Clinic Foundation from January 2005 to December 2014. PATIENTS: Adu…

Risk factors for incisional surgical site infection after elective laparoscopic colorectal surgery.

BACKGROUND: Surgical site infection (SSI) is a common morbidity in patients undergoing colorectal surgery, and the focus of previous studies has primarily been on incisional SSI. Most reports thus far have focused on open surgery rather than on laparoscopic colorectal surgery (Lap CR). Therefore, the aim of the present study was to identify the risk factors for incisional SSI in patients undergoing elective Lap CR. METHODS: This retrospective study was conducted to evaluate the occurrence and risk factors of incisional SSI for elective Lap CR. From January 2008 to June 2018, 1825 consecutive …

Ureteral stents increase risk of postoperative acute kidney injury following colorectal surgery.

BACKGROUND: Ureteral stents are commonly placed before colorectal resection to assist in identification of ureters and prevent injury. Acute kidney injury (AKI) is a common cause of morbidity and increased cost following colorectal surgery. Although previously associated with reflex anuria, prophylactic stents have not been found to increase AKI. We sought to determine the impact of ureteral stents on the incidence of AKI following colorectal surgery. METHODS: All patients undergoing colon or rectal resection at a single institution between 2005 and 2015 were reviewed using American College o…

Diverting stoma-related complications following colorectal endometriosis surgery: a 163-patient cohort.

OBJECTIVE: Deep endometriosis may simultaneously infiltrate the vagina and the rectosigmoid, which associated resection may increase the risk of postoperative complications. Among these complications, rectovaginal fistula is one of the worst. To reduce the risk of rectovaginal fistula and related complications, surgeons may employ diverting stoma. The literature is rich in data concerning the usefulness of stoma in patients managed for low rectal cancer. However, extrapolation of these data to patients managed for rectal endometriosis is disputable. For this reason, there are no guidelines on

Frailty indicators and functional status in older patients after colorectal cancer surgery.

OBJECTIVES: The number of older survivors from colorectal cancer is increasing, but little is known regarding long-term consequences of cancer treatment in this patient group. Physical function is an important outcome for older patients, affecting both autonomy and quality of life. We aimed to investigate physical function in older patients with colorectal cancer before and after surgery, and to examine the role of individual frailty indicators as predictors of functional decline. MATERIAL AND METHODS: We present 16-28 months follow-up data of older patients after elective surgery for colorec
