Microcosting implementation facilitation for emergency department-initiated buprenorphine for untreated opioid use disorder.

BACKGROUND: Adoption of ED-initiated buprenorphine for opioid use disorder has been slow despite evidence of its effectiveness. Implementation facilitation (IF) is an evidence-based implementation strategy to promote adoption of new practices within clinical settings. The Clinical Trials Network (CTN)-0069 Project ED Health" study evaluated whether provision of ED-initiated buprenorphine with referral for community-based medication for opioid use disorder (MOUD) increased after IF. We identified the health system resources required to conduct IF and sustain ED-initiated buprenorphine with refe

Pediatric response to a large-scale child protection intervention.

OBJECTIVE: In a rural area of the US state of Texas, in April 2008, the Texas Department of Family and Protective Services (DFPS) responded to evidence of widespread child abuse in an isolated religious compound by removing 463 individuals into state custody. This mass child protection intervention is the largest such action that has ever occurred in the United States. The objective of this paper is to characterize the burdens placed on the area's community resources, healthcare providers, and legal system, the limitations encountered by the forensic and public health professionals, and how th…

Pharmacist-led community network approach addressing social needs and adherence (Connect AF).

BACKGROUND: Social determinants of health (SDOH) are nonmedical factors that impact health outcomes and medication use. As national initiatives and value-based care models prioritize SDOH interventions, pharmacists are uniquely positioned to support these efforts. However, evidence describing pharmacist-led SDOH screening, particularly through collaborations between health-system and community pharmacists, remains limited. OBJECTIVE: To learn how community pharmacists address SDOH-related care gaps among patients with atrial fibrillation (AF) and how vertically integrated partnerships can str

Temporal dynamics of direct reciprocal and indirect effects in a host-parasite network.

1. Temporal variation in the direct and indirect influence that hosts and parasites exert on each other is still poorly understood. However, variation in species' influence due to species and interactions turnover can have important consequences for host community dynamics and/or for parasite transmission dynamics, and eventually for the risk of zoonotic diseases. 2. We used data on a network of small mammals and their ectoparasites surveyed over 6 years to test hypotheses exploring (i) the temporal variability in direct and indirect influences species exert on each other in a community, and (

"People are more afraid of a dementia diagnosis than of death": The challenges of supporting advance care planning for persons with dementia in community settings.

Improving early uptake of advance care planning (ACP) for persons with dementia and their families requires that staff in community-based settings feel armed and equipped to encourage and support this process. Yet few studies have explored whether staff within non-medical environments feel prepared to support early ACP engagement for persons with early-stage dementia and their families. Our qualitative interpretivist study aimed to fill this gap by facilitating, transcribing and thematically analyzing deliberations from three focus groups with 17 community-based staff. Our findings revealed fo

Kids and cocaine--a treatment dilemma.

Despite aggressive efforts to recruit into treatment cocaine users 21 years and younger, we obtained only 12 such clients over a period of 9 … the community to help assure that younger cocaine users will obtain the assistance they need. Four community resources are seen as particularly relevant: family, schools, the medical community, and public agencies in the areas of health, social services, and criminal justice. The capacity of each to take action and possible impediments to that action are discussed.

Geographic proximity to neighborhood resources and depression among older adults in South Korea.

OBJECTIVES: Older adults are closely connected to their neighborhoods and they spend more time there than younger adults. Because their mobility is often impeded by diminished health and functioning, access to neighborhood health and social services is essential for their well-being. This article examines whether geographic proximity to these types of neighborhood resources is associated with depression among older adults in South Korea. METHODS: Data are from A Profile of Older Adults: 2015 which sampled 1,455 community-dwelling individuals ages 60 and older in South Korea. The Patient Healt

Clinical and epidemiological factors associated with severe dehydration from cholera among pediatric patients in the community of Lusaka, Zambia, October 2023-May 2024.

Factors associated with severe dehydration from cholera among children in the community is still not fully understood. We analyzed the characteristics of the pediatric cases who were hospitalized in the community cholera treatment centers in the capital Lusaka between October 2023 and May 2024. Presence of underlying conditions (e.g. human immunodeficiency virus (HIV) infection, severe acute malnutrition), specific catchment areas (e.g. low-income residential areas), and an early phase of the outbreak was associated with increased numbers of severe cases in the community by multivariate analys

Community participation in disease control.

The main determinants of community participation in disease control programmes are identified and a framework with eleven variables is developed. Attention is drawn to the political background, community characteristics, the managerial capacity of the provider and the epidemiology of the disease. The framework is designed to guide health professionals in the systematic assessment and monitoring of participation in disease control programmes. Analysis of the Ghanaian Guinea Worm Eradication Programme and the Nicaraguan Tuberculosis Control Programme are presented as case studies. They show that

Staff perspectives on using long-acting antiretroviral treatment among persons being released from carceral settings in Baltimore, Maryland, USA.

The prevalence of HIV among individuals who experience incarceration is estimated to be three times higher than that of the general population in the United States. Currently, the standard of care, antiretroviral medications taken daily, is effective in reducing mortality and HIV transmission but individuals experiencing community re-entry face barriers while re-integrating with their community which may make adherence to daily medication challenging. Long acting injectable antiretroviral treatment (LAI ART) may offer an opportunity for individuals experiencing community re-entry to remain in
