A comparison of phenomenological, clinical and familial psychiatric features of pediatric OCD and trichotillomania.

OBJECTIVES: Although trichotillomania (TTM) is classified in the obsessive-compulsive disorders (OCD) chapter of the DSM-5, several studies showed that it has several differences. The aim of this study was to examine the phenomenology, comorbidity, and family psychiatric characteristisc of childhood TTM and OCD. METHODS: This study compared youth ages 6-17 years with a primary diagnosis of TTM (n = 63) to those with primary OCD (n = 65) on clinical and familial psychiatric characteristics. RESULTS: In our study, the findings showed that family history of schizophrenia (42.3%) was higher amon

Personality disorder type only sometimes matters: An exploration of patient's personality disorder as a source of variance in early therapeutic alliance.

OBJECTIVE: The study aimed to analyse personality disorders among patients in the early phase of psychotherapy as a source of variance in the quality of the emerging alliance. METHODS: The sample consisted of 131 individual psychotherapy patients with borderline (BPD), narcissistic (NPD), dependent (DPD) and obsessive-compulsive (OCPD) personality disorders and 131 psychotherapists. Different sources of alliance estimation were included. Therapeutic alliance was operationalized on several dimensions. The hypothesis was tested using an independent samples one-way analysis of variance. RESULTS

Diagnostic agreement between a doctor and a nurse for psychiatric disorders: a pilot study.

The objective of the study was to examine inter-rater agreement for psychiatric disorders by means of a structured interview performed by a psychiatrist and a nurse. Sixteen psychiatric inpatients with a mean age of 38.9+/-12.9 years were assessed independently by a psychiatrist and a nurse using a structured psychiatric interview, Strukturert Psykiatrisk Intervju for Allmennpraksis (SPIFA). For most diagnoses, agreement was substantial to perfect (kappa = 1 for the diagnoses major depression, generalized anxiety disorder, obsessive-compulsive disorder and substance abuse). Psychiatric Disease as a Potential Risk Factor for Dementia: A Narrative Review.

Neurodegenerative disease is a major global health problem with 150 million people predicted to have dementia by 2050. Genetic factors, environmental factors, demographics, and some diseases have been associated with dementia. In addition to associations between diseases such as hypertension and cerebrovascular disease and dementia, emerging findings associate some psychiatric disorders with incident dementia. Because of the high and increasing global prevalence of dementia and the high worldwide prevalence of psychiatric disorders, the primary objective of this narrative review was to evaluat

Obsessive-compulsive disorder and comorbid anxiety problems in a national anxiety screening sample.

Questionnaire data were obtained from 5867 participants attending a national anxiety screening program. These participants were selected from more than 15,000 respondents on the basis of never having received treatment for a mental health problem. A screening instrument was designed to assess five anxiety disorders (obsessive-compulsive disorder, posttraumatic stress disorder, social phobia, generalized anxiety disorder, and panic disorder). The present study focused on those participants meeting full or partial screening criteria for obsessive-compulsive disorder (n = 3212), with those not me

[Introduction and transformation of the psychiatric term "anancasm". From Gyula (Julius) Donáth via Kurt Schneider to ICD-10].

The conceptual history of "anancasm" in psychiatry remains almost unexplored and this article will help to remove this deficit. It was the B … nd hyper-correct. Based on Schneider's concept anancasm has survived as a name for a subdivision of compulsive personality disorders in ICD-10; however, these rather complex personality defects were not what Donáth had in mind when he first suggested the term anancasm. The paper discusses further discrepancies between Donáth, Schneider and ICD-10.

Neuropsychological functions in anxiety disorders in population-based samples: evidence of episodic memory dysfunction.

Most of the available evidence on neuropsychological functioning in anxiety disorders is based on clinical samples, investigating persons affected by obsessive-compulsive disorder. Knowledge is sparse regarding cognitive functions in other types of anxiety disorders. The aim of this study was to examine whether persons diagnosed with an anxiety disorder show neuropsychological impairments relative to healthy controls in tasks tapping episodic memory, verbal fluency, psychomotor speed, and executive functioning. Population-based samples comprising individuals affected by panic disorder with and

[Clinical characteristics of obsessivecompulsive disorder].

Clinical characteristics of obsessive-compulsive disorder. Obsessive-compulsive disorders (OCD) are composed by repetitive behaviors and intrusive thought, associated with doubt and avoidance. Washers and checkers are the most common dimension of OCD, but it can also be characterized by taboo thoughts or other personalized symptoms. OCD severity could be measured by the Y-BOCS and the balance between obsession and compulsion severity has led to separate 3 kinds of OCD: mostly compulsive, obsessive or mixed. Avoidance should also be included in the measure of OCD severity. How the relatives are

An exploratory analysis of obsessive-compulsive personality traits, pathologic anger and quality of life among trauma-exposed veterans.

INTRODUCTION: Trauma exposure and post-traumatic stress disorder (PTSD) are associated with high rates of co-occurring personality pathology, including Obsessive Compulsive Personality Disorder (OCPD). OCPD is characterized by rigidity and perfectionism, leading to internalized distress, interpersonal difficulties, and lower quality of life (QOL). Individuals with OCPD often report high levels of anger, which may exacerbate distress and interpersonal issues. However, the relationship between OCPD, anger, and QOL in trauma-exposed Veterans remains understudied. MATERIALS AND METHODS: 92 Vetera…

In-session emotional expression predicts symptomatic and panic-specific reflective functioning improvements in panic-focused psychodynamic psychotherapy.

In panic-focused psychodynamic psychotherapy (PFPP), exploration and interpretation of avoided and conflicted emotions and fantasies surrounding anxiety are thought to promote panic-specific reflective functioning (PSRF), which drives panic disorder improvements. Patient emotional expression within a session may be a marker of engaged processing and experiencing of affectively charged material. Degree of in-session expressed emotion, indicating both verbal and nonverbal emotions, was examined across three early therapy sessions for prediction of subsequent outcomes. We further investigated whe

The effectiveness of psychodynamic psychotherapies: An update.

This paper provides a comprehensive review of outcome studies and meta-analyses of effectiveness studies of psychodynamic therapy (PDT) for the major categories of mental disorders. Comparisons with inactive controls (waitlist, treatment as usual and placebo) generally but by no means invariably show PDT to be effective for depression, some anxiety disorders, eating disorders and somatic disorders. There is little evidence to support its implementation for post-traumatic stress disorder, obsessive-compulsive disorder, bulimia nervosa, cocaine dependence or psychosis. The strongest current evid

Impulse control disorders in Parkinson's disease: prevalence and possible risk factors.

There is an increasing awareness that impulse control disorders (ICDs), including compulsive gambling, buying, sexual behavior and eating, can occur as a complication of Parkinson's disease (PD). In addition, other impulsive or compulsive disorders including dopamine dysregulation syndrome (DDS) and punding are reported. Case reporting and prospective studies documented an association between ICDs and the use of dopamine agonists (DAs), and to a lesser extent levodopa use, while DDS occurs with higher dosages of levodopa or short-acting DAs. Data suggest that risk factors for an ICD may includ

Altered grey matter volume and white matter integrity in individuals with high schizo-obsessive traits, high schizotypal traits and obsessive-compulsive symptoms.

Altered brain structures have been found in patients with schizo-obsessive disorder, schizophrenia and obsessive-compulsive disorder in previous studies. However, it is unclear whether similar brain changes are also found in individuals with high schizo-obsessive traits (SOT), high schizotypal traits (SCT) and obsessive-compulsive symptoms (OCS). We examined grey matter volume (GMV) and white matter integrity (WMI, including fractional anisotropy, mean diffusivity, axial diffusivity and radial diffusivity) in 26 individuals with high SOT, 30 individuals with high SCT, 25 individuals with OCS a

Depression predicts persistence of paranoia in clinical high-risk patients to psychosis: results of the EPOS project.

BACKGROUND: The link between depression and paranoia has long been discussed in psychiatric literature. Because the causality of this association is difficult to study in patients with full-blown psychosis, we aimed to investigate how clinical depression relates to the presence and occurrence of paranoid symptoms in clinical high-risk (CHR) patients. METHODS: In all, 245 young help-seeking CHR patients were assessed for suspiciousness and paranoid symptoms with the structured interview for prodromal syndromes at baseline, 9- and 18-month follow-up. At baseline, clinical diagnoses were assesse

[Comorbidity and characteristic of obsessive-compulsive symptoms in anorexia nervosa].

There is constant interest in possible relations between obsessive-compulsive disorder (OCD) and eating disorders, particularly anorexia nervosa (AN). The comorbidity rate for OCD and AN is reported to be between 10% and even 40%. There is also an increased incidence of prior AN in OCD patients and high number of anorectic patients with obsessional premorbid personality. Similarities between AN and OCD lie in the symptoms of the disorders: intrusive, fearful thoughts, a compulsive need to perform rituals aimed at reducing the level of anxiety and obsessions maintaining these rituals. In case o
