Excessive response to provocation rather than disinhibition mediates irritable behaviour in Huntington's disease.

BACKGROUND: Irritable and impulsive behaviour are common in Huntington's disease (HD: an autosomal dominant disorder causing degeneration in … PPSP), Barratt Impulsivity Scale], in addition to cognitive tasks of provocation, motor inhibition, delay discounting and decision making under uncertainty. We used generalised linear models to determine differences between cases and controls, and associations with irritability in the HD group. RESULTS: We found differences between cases and controls on the negati …

Effects of neuromodulation on cognitive performance in individuals exhibiting addictive behaviors: A systematic review.

BACKGROUND: There is growing interest in non-invasive brain stimulation techniques as treatments for addictive disorders. While multiple rev … ks were organized into five domains: (1) Inhibitory control, (2) Risk-taking, (3) Impulsive choice (delay discounting), (4) Executive function, and (5) Implicit biases. RESULTS: Twenty-four articles met the inclusion criteria. Fifty-seven percent of studies used tDCS and 43% used TMS, with nearly all studies (96%) targeting the dorsolateral prefrontal cortex. Ten …

High Prevalence of Suicidal Behavior in Severe Alcohol Use Disorder Linked to Specific Neurocognitive Deficits: The Cognition, Alcohol, Genes, and Environment Study.

OBJECTIVE: Alcohol Use Disorder (AUD) and Suicidal Thoughts and Behaviors (STB) frequently co-occur. Their association could be explained by … rocognitive variables and found that alcohol problems (AUDIT-P) were associated with inhibition and delay discounting, while the co-occurrence of AUD and suicide attempt was related to inhibition and shifting abilities. CONCLUSIONS: These preliminary findings open research avenues to study the mechanisms related to the co-occurrence of AUD and STB, with the hope o …

Proof-of-concept trial of Goal Management Training+ to improve executive functions and treatment outcomes in methamphetamine use disorder.

BACKGROUND: Deficits in executive function are common in methamphetamine use disorder (MUD), likely contributing to difficulties in sustained treatment success. Cognitive remediation interventions are designed to treat such deficits but have not been adapted to the needs of people with MUD. This study presents a proof-of-concept trial to evaluate a new cognitive remediation program for MUD, Goal Management Training+ (GMT+). METHODS: This was a cluster-randomised crossover trial comparing GMT+ with a psychoeducation-based control (Brain Health Workshop; BHW). GMT+ is a therapist-administered g

Frontal Traumatic Brain Injury Increases Impulsive Decision Making in Rats: A Potential Role for the Inflammatory Cytokine Interleukin-12.

Traumatic brain injury (TBI) is associated with the development of numerous psychiatric diseases. Of particular concern for TBI patients is the impact of chronic impulsivity on daily functioning. Despite the scope of the human problem, little has been done to address impulsivity in animal models of brain injury. In the current study, we examined the effects of either a severe or a milder bilateral frontal controlled cortical impact injury on impulsivity using the Delay Discounting Task (DDT), in which preference for smaller-sooner over larger-later rewards is indicative of greater impulsive ch…

Urethral inflammation in male chimpanzees caused by ureaplasmas and Chlamydia trachomatis.

Specimens from men with acute non-gonococcal urethritis were tested to determine their microbial content and then given intra-urethrally to male chimpanzees. Two animals received ureaplasmas only and one became infected. The second did so when given a different strain. Both developed a polymorphonuclear leukocyte (PMNL) response. Two chimpanzees received a mixture of ureaplasmas and Chlamydia trachomatis and there was a suggestion that the ureaplasmas delayed or suppressed the chlamydial response. The latter, that is urethral infection with a pronounced PMNL response, was most clearly seen in

Accustomed to enduring: experiences of African-American women seeking care for cardiac symptoms.

OBJECTIVE: Understand the meaning of delayed treatment seeking in African-American women with unstable angina and myocardial infarction. METHODS: Phenomenologic analysis of in-depth interview data and field notes on 12 African-American women hospitalized with unstable angina or myocardial infarction. RESULTS: Women's interpretation of and response to symptoms were informed by experiences of marginalization and their self-understanding as people who were strong and who had endured life's hardships. When hospitalized, some women experienced trivialization of their complaints by clinicians and

Epistemic Injustice in Rheumatoid Arthritis Care: A Narrative Review of Invisible Suffering, Ageism, and Treatment Delay.

Despite advances in disease-modifying therapies and treat-to-target strategies, many patients with rheumatoid arthritis (RA) continue to experience persistent pain, fatigue, and functional impairment. These symptoms are particularly common among older adults and are frequently under-recognized in clinical practice. This study examines RA care through the lens of epistemic injustice to explore how patients' experiential knowledge is interpreted, valued, or discounted, and how these processes contribute to treatment delay in aging societies. We conducted a narrative review of peer-reviewed liter

Conditions under which arousal does and does not elevate height estimates.

We present a series of experiments that explore the boundary conditions for how emotional arousal influences height estimates. Four experiments are presented, which investigated the influence of context, situation-relevance, intensity, and attribution of arousal on height estimates. In Experiment 1, we manipulated the environmental context to signal either danger (viewing a height from above) or safety (viewing a height from below). High arousal only increased height estimates made from above. In Experiment 2, two arousal inductions were used that contained either 1) height-relevant arousing i

Isolated T-Wave Inversion in Lead aVL: A Diagnostic Blind Spot in Emergency Department Chest Pain Evaluation.

BACKGROUND: Isolated T-wave inversion (TWI) in lead aVL is commonly regarded as a benign normal variant in asymptomatic individuals during routine electrocardiogram (ECG) interpretation. In patients presenting with chest pain, however, this subtle abnormality may represent early high lateral myocardial ischemia, often involving diagonal branch disease, and may be overlooked when the remainder of the ECG appears normal. The diagnostic challenge in such cases is not a lack of knowledge regarding lead aVL, but the ease with which isolated abnormalities can be discounted when patients appear clini

Single-Session Transcranial Direct Current Stimulation Temporarily Improves Symptoms, Mood, and Self-Regulatory Control in Bulimia Nervosa: A Randomised Controlled Trial.

BACKGROUND: Evidence suggests that pathological eating behaviours in bulimia nervosa (BN) are underpinned by alterations in reward processing and self-regulatory control, and by functional changes in neurocircuitry encompassing the dorsolateral prefrontal cortex (DLPFC). Manipulation of this region with transcranial direct current stimulation (tDCS) may therefore alleviate symptoms of the disorder. OBJECTIVE: This double-blind sham-controlled proof-of-principle trial investigated the effects of bilateral tDCS over the DLPFC in adults with BN. METHODS: Thirty-nine participants (two males) rec

Pathological gambling and alcohol dependence: neural disturbances in reward and loss avoidance processing.

Pathological gambling (PG) shares clinical characteristics such as craving and loss of control with substance use disorders and is thus considered a behavioral addiction. While functional alterations in the mesolimbic reward system have been correlated with craving and relapse in substance use disorders, only a few studies have examined this brain circuit in PG, and no direct comparison has been conducted so far. Thus, we investigated the neuronal correlates of reward processing in PG in contrast to alcohol-dependent (AD) patients and healthy subjects. Eighteen PG patients, 15 AD patients and
