To our knowledge, this is the first report of a method of stretching the interdigital skin of syndactyly by means of a pincer. The pincer, a U-shaped metal spring plate with a sponge cushion, is applied before the operation for about two months; pressure is placed against both sides of the interdigital fusion to expand the skin and to develop hollows. Use of the pincer facilitates a syndactyly operation by allowing defect coverage with a local flap only. Four patients successfully treated by this method are reported. The method has the great advantage of not requiring skin grafts, which often

A rare case of true median cleft of the upper lip associated with three skin masses is reported. While 114 cases of median cleft of the upper lip have been reported, in our literature review we found only 1 case, reported by Sharma, in which the median cleft was associated with skin masses. The embryology of this case is also discussed.

An incision placed at right angles to the direction of skin cleavage lines is susceptible to the development of a haematoma within the sutured wound. Moreover, the edges of the sutured incision are not firmly fixed together by the material and the sutured incision is subjected to direct tension. For these 3 reasons an incision not conforming with the direction of skin cleavage lines may frequently allow stretching and postoperative hypertrophy of a scar. The biological function of a young hypertrophic scar may be an expansion-relaxation movement of the capsule of scar tissue like a bellows, at

From the examination of 197 patients who presented with one or more head nodules in the Sudan savanna in Cameroon, and from previous examinations of patients with ocular onchoceriasis, it concluded that in this part of Africa the formation of a head nodule is often preceded by the development of ocular lesions. Head nodulectomy is therefore of limited prophylactic value. Head nodules were removed from 107 of these patients, of whom 17 also received diethyl-carbamazine citrate (DEC), and 20 received both DEC and suramin. Fifteen other patients received DEC as the only treatment, and 4 suramin a

A new technique for the correction of trochanteric bulges is described. A buried dermal-fat flap is used as a suspensory mechanism to hang the weight of the thigh skin and subcutaneous tissues on the deep fascia, rather than on the upper skin flap. This technique is particularly applicable to patients with asymmetry of the buttocks and thighs as well as those with ptosis of the buttocks. Another advantage is that it creates a new gluteal fold at a predetermined higher level.

The nail is not only aesthetically significant but also functionally essential, as it protects the distal digits and serves as a counterpart for pressure-bearing areas. Nail surgery presents unique challenges since, even after successful treatment of pathological conditions, scarring may lead to cosmetic deformities, and partial or complete nail loss can result in functional impairment. Fortunately, recent advancements in nail surgery have improved treatment success rates while minimizing complications. This review focuses on the surgical management of 6 key nail disorders: ingrowing nails, pi

BACKGROUND: Approximately 10% of patients report penicillin allergy. Based on historical guidelines, patients with a penicillin allergy should receive an alternative to cephalexin for prophylaxis in dermatologic surgery.
OBJECTIVE: To determine, based on evidence, if cephalexin should be used as a first-line prophylactic antibiotic in dermatologic surgery for patients with penicillin allergy.
METHODS: Systematic review of the literature was performed with defined search terms.
RESULTS: Less than 5% of those with penicillin allergy labels have a true penicillin allergy. For patients with tru

Post-acne scarring is the most common permanent consequence of acne vulgaris. Subcision is an intervention in which a needle or blunt cannula is inserted under the scar and moved back and forth to cut fibrotic strands and form new connective tissue, thereby elevating the atrophic scars. In this study, we reviewed the efficacy and adverse effects of different subcision techniques alone or in combination with other modalities to manage acne scars. The terms (subcision), (subcision AND acne scar), and (subcision AND scar) were searched in PubMed and Google Scholar. We included all available repor

Ticks have a cosmopolitan distribution and, as such, are also found in Japan. Ticks are typically ectoparasites of wild animals, however, humans can also be bitten when visiting environments inhabited by ticks. Herein, we describe two cases with atypical tick bites. Case 1 was an elderly Japanese male patient who presented with a fully engorged tick measuring 20 × 17 × 8 mm; it is rare for ticks to attain a length of 20 mm. Case 2 was an elderly Japanese female with severe dementia who presented with multiple tick bites, which is rare, after going missing for 6 days before being found in a den

In this article we give an overview of which skin conditions that are currently treated with laser and explains the basic principles of treatment. In addition, we summarize recommendations of the Danish Dermatological Society for demarcation of medical treatments which can be provided free of charge from cosmetic self-payment treatments.

PURPOSE: Open abdominal wall reconstruction is often a complex endeavor, usually performed on patients with multiple risk factors and co-morbidities.
METHODS: In this article, we review soft tissue management techniques that can optimize the skin and subcutaneous tissue, with the goal of reducing surgical-site occurrences.
RESULTS: Regardless of the hernia repair technique used, outcomes can be highly dependent on the appropriate management of the skin and subcutaneous tissue. Indeed, dehiscence and surgical-site infection can jeopardize the entire reconstruction, especially in cases where s

Pyoderma gangrenosum (PG) is a neutrophilic dermatosis characterised with ulcerations. Inflammatory bowel diseases (ulcerative colitis and Crohn's disease) and haematologic diseases (leukaemia, preleukaemia and monoclonal gammopathy) have been reported in about 40-50% of PG patients in whom the treatment of the underlying disease is important for the improvement of the lesions. We herein report a colorectal adenocarcinoma patient with PG, who responded partially to topical treatments and systemic immunosuppressants and healed completely with the aid of surgical wound repair and hyperbaric oxyg

The tremendous ability of the skin's epidermis to regenerate is due to the presence of epidermal stem cells that continuously produce keratinocytes, which undergo terminal differentiation to a keratinized layer that provides the skin's barrier properties. The ability to control this process in vitro has made it possible to develop various types of tissue-engineered skin grafts, some of which are among the first tissue-engineered products to ever reach the market. In the past 30 years, these products have been applied with some success to the treatment of chronic skin wounds such as diabetic an

A 14-year-old girl was inadvertently immunised with an excessively large dose of BCG-Connaught vaccine (1.0 ml instead of 0.1 ml). Examination of the patient a few hours later revealed a subcutaneous fluctuant tender lump at the injection site. This was surgically excised within 12h of immunisation and she was treated with isoniazid and rifampicin for six weeks. The patient developed no complications, other than a minor surgical scar. This case highlights the importance of the correct administration and dosing of BCG vaccine and suggests a management option that may prevent the severe local an

Tissue-engineered skin is now a reality. For patients with extensive full-thickness burns, laboratory expansion of skin cells to achieve barrier function can make the difference between life and death, and it was this acute need that drove the initiation of tissue engineering in the 1980s. A much larger group of patients have ulcers resistant to conventional healing, and treatments using cultured skin cells have been devised to restart the wound-healing process. In the laboratory, the use of tissue-engineered skin provides insight into the behaviour of skin cells in healthy skin and in disease
