[Genetic factors, Na K ATPase activity and neuropathy in diabetics].

A genetic predisposition to develop a polyneuropathy in case of diabetes seems to exist. Some ethnic groups such as North Africans are prone to develop a diabetic polyneuropathy. To identify this predisposition could help in targeting a preventive treatment. We have observed that red cell Na/K ATPase activity was lower among diabetic patients than controls and even lower when diabetic neuropathy was present. Now an impaired NA/K ATPase activity has been implicated in the pathogenesis of diabetic neuropathy and ethnic differences in this enzyme activity have been demonstrated. For these reasons

Reproducibility and persistence of neural and adrenal autoantibodies in diabetic autonomic neuropathy.

The presence of autoantibodies to autonomic nervous tissue structures is a feature of patients with symptomatic diabetic autonomic neuropathy. It has not been established whether these autoantibodies cause, contribute to or simply reflect nervous tissue damage. Serum samples were tested for the presence of complement-fixing autoantibodies to adrenal medulla, vagus nerve, and sympathetic ganglion cells, to demonstrate: (a) reproducibility of the technique, (b) persistence of the antibodies, and (c) whether or not they occur in patients with non-insulin-dependent (Type 2) diabetes (NIDDM) with n

Diversity of the neuropathies in patients with non-insulin-dependent diabetes mellitus.

The relationships between cardiac autonomic neuropathies, diabetic somatic neuropathy, metabolic parameters, general parameters (such as age and duration of illness) and diabetic microangiopathy and macroangiopathy were investigated in 103 patients with non-insulin-dependent diabetes mellitus (NIDDM). Spearman's correlation coefficients were calculated for the comparisons of all the parameters of the neuropathies with all the other parameters. Variables were selected using a stepwise procedure and multiple regression analysis was carried out using these variables. The results of the regression

Cough reflex threshold in diabetes mellitus with and without autonomic neuropathy.

The cough reflex, a vagus-mediated respiratory protective reflex, was investigated in diabetics and healthy controls by establishing a cough reflex threshold to increasing concentrations of citric acid aerosol. Diabetics with autonomic neuropathy (n = 20), whether overtly symptomatic (n = 5) or evident only on certain non-invasive cardiovascular autonomic function tests, were found to have significantly (p < 0.05 by chi 2 test) raised cough reflex threshold (range 1 to > 100%, median value 50%) in contrast to either the non-neuropathic diabetics (n = 20) or normal controls (n = 20; range 1-10%

Severe sensory-autonomic neuropathy and endocrinopathy in insulin-dependent diabetes.

A patient developed insulin-dependent diabetes mellitus at the age of 9 years, complicated by a sensory/autonomic polyneuropathy which presented with a respiratory arrest at the age of 41 years. The neuropathy increased in severity over the subsequent two decades. At the age of 52 years she had hypopituitarism, hypothyroidism and low normal adrenal function. Autopsy at the age of 59 years revealed loss of pituitary tissue with evidence of hypophysitis, a lymphocytic thyroiditis and severe adrenal atrophy with lymphocytic infiltration of the medulla. The pancreas showed reduced numbers and size

A comparison of perineurial and vascular basal laminal changes in diabetic neuropathy.

Measurements were made of the thickness of the basal lamina of perineurial cells in the sural nerve in a series of patients with diabetic neuropathy and compared with a group of patients with type I hereditary motor and sensory neuropathy (HMSN) and with organ donor control cases. The thickness was significantly greater in the diabetic patients as compared both with the HMSN cases and the organ donor controls. This was most obvious for the intermediate layers of the perineurium. Perineurial basal laminal thickness was only slightly greater in the HMSN cases than in the organ donor controls and

An investigation of antibodies to nerve growth factor in diabetic autonomic neuropathy.

Nerve growth factor (NGF) is required for development and maintenance of autonomic nervous function and is highly expressed in the iris. An association between sensory and autonomic diabetic neuropathy and NGF function has been postulated. In light of the evidence for an autoimmune component to the pathogenesis of autonomic neuropathy in Type 1 diabetes mellitus, an enzyme linked immunosorbent assay (ELISA) was established to investigate the presence of autoantibodies to mNGF in: 20 patients with long-standing Type 1 diabetes with abnormal autonomic function tests, of whom 14 had symptomatic a

Circadian blood pressure variation in diabetic patients with autonomic neuropathy.

OBJECTIVE: To elucidate distinctive patterns of circadian variation of blood pressure in diabetic patients. DESIGN: Circadian variations and indices of blood pressure variability were examined by non-invasive ambulatory blood pressure monitoring. The indices were assessed for a correlation with various possible signs of autonomic neuropathy, including the coefficient of variation of the RR interval and orthostatic hypotension. METHODS: The study was conducted on 38 normotensive diabetics and 15 age- and sex-matched non-diabetic controls, all of whom were outpatients at the Third Department o

[Obesity and autonomic neuropathy in non-insulin-dependent diabetes mellitus].

It has been said that the presence of autonomic neuropathy is related to the onset of obesity. Given the high prevalence of association between autonomic neuropathy and Diabetes Mellitus and the association between metabolic disease and obesity, we considered it important to evaluate the possible relationship in order to ascertain the pathogenic connection and its impact on therapy. We evaluated 2 groups of diabetic patients: one group was composed of obese patients and the other of non-obese. According to our results, there appears to be no relationship between autonomic neuropathy and obesit

Efficacy of desipramine in painful diabetic neuropathy: a placebo-controlled trial.

Although amitriptyline relieves pain in many patients with painful diabetic neuropathy, side effects often preclude effective treatment. Desipramine has the least anticholinergic and sedative effects of the first generation tricyclic antidepressants. We compared a 6 week course of desipramine (mean dose, 201 mg/day) to active placebo in 20 patients with painful diabetic neuropathy in a double-blind crossover trial. Pain relief with desipramine was statistically significant in weeks 5 and 6. Eleven patients reported at least moderate relief with desipramine, compared to 2 with placebo. Pain rel

[Autonomic cardiac neuropathy in patients with insulin-deficient diabetes mellitus].

Altogether 60 patients with insulin-deficient diabetes aged 16-40 with a disease standing of 11.2 +/- 0,93 years were examined. Diabetic autonomous cardiac neuropathy was detected in 73% of the patients by means of a study of the R-R interval variation in normal and deep breathing and Valsalva's index. Direct correlation of the time-length of diabetes with retinopathy and distal polyneuropathy was found. Orthostatic hypotension was diagnosed in 10% of the patients.

[Early diagnosis of autonomic diabetic neuropathy using sensitivity to temperature].

Testing temperature sensitivity thresholds and sensory nerve velocity in 76 type I diabetics and 16 healthy controls gave the following results: In 24 patients with dysfunction of one or more autonomic nervous system functions (histamine test reaction, erectile impotence, gastroparesis, orthostatic hypotension and bladder atonia) temperature sensitivity of the lower limbs, as an early manifestation of distal diabetic neuropathy, was markedly impaired compared with healthy controls as well as patients without autonomic dysfunction (n = 52); sensory nerve velocity (sural nerve) was similar in th

Multipeaked esophageal peristaltic pressure waves in patients with diabetic neuropathy.

We evaluated esophageal function in 14 consecutive insulin-dependent diabetic patients who had evidence of peripheral and autonomic neuropathy, but no esophageal symptoms. One to three contraction waves immediately followed a primary peristaltic contraction wave. The majority of these multipeaked pressure complexes consisted of two peaks. Multipeaked contractions were observed with all peristaltic waves in 12 of the 14 diabetic patients and with most of the peristaltic complexes in the remaining 2 patients. Multipeaked peristaltic waves were present in 1 of 6 diabetic patients without neuropat

Axonal dysfunction in the neuropathy of diabetes mellitus: a quantitative electrophysiological study.

The extent of axonal dysfunction in a group of patients with diabetes mellitus has been investigated using recently introduced quantitative techniques of electromyography. In 40 diabetic patients, estimates of motor unit numbers in the extensor digitorum brevis muscles and measurements of the parameters of electrically evoked motor unit potentials therein are presented, along with significant correlations between these parameters and motor nerve conduction velocities and distal motor latencies in the lateral popliteal nerves. The results indicate that axonal dysfunction and concomitant collate

Determinants of Cardiac Autonomic Neuropathy Among Patients with Diabetic Peripheral Neuropathy: A Facility-based Cross-Sectional Study.

INTRODUCTION: Cardiac Autonomic Neuropathy (CAN) is frequently an underdiagnosed consequence of Diabetes Mellitus (DM), increasing the risk of cardiac arrhythmia, silent myocardial ischemia and sudden cardiac death. Diabetic Peripheral Neuropathy (DPN) is a common consequence of diabetes. We aimed to study the proportion of CAN among patients with DPN and identify the predictors of CAN in these patients. METHODOLOGY: This is a hospital-based cross-sectional study conducted over a six-month period. A total of 60 DM patients with nerve conduction study-proven DPN, who fulfilled the inclusion an

Treatment of Diabetic Cardiovascular Autonomic, Peripheral and Painful Neuropathy. Focus on the Treatment of Cardiovascular Autonomic Neuropathy with ACE Inhibitors.

Neuropathies of the peripheral and autonomic nervous systems affect up to half of all people with diabetes mellitus, and are major risk factors for foot ulceration, amputation and cardiovascular dysfunction. Peripheral neuropathies manifest with either painful or painless symptoms, but many patients experience both. Once diagnosed appropriately, painful diabetic neuropathy management presents a unique challenge for physicians and should be considered as a syndrome, clinically distinct from diabetic peripheral neuropathy. The aetiology is multifactorial: metabolic changes in diabetes may direct
