The diaphragm muscle pathology in four patients with Fukuyama type congenital progressive muscular dystrophy (FCMD) was reported. In the diaphragm muscle fibers of three patients aged more than 14 years exhibiting chronic respiratory failure, a lot of electron dense lesions, which varied in size but did not extend over the whole length of the muscle fibers, and a marked increase in mitochondria were observed as well as the dystrophic changes. The dense lesions, consisting of electron dense fibrillary material comparable to Z-band materials in electron density, seemed to be due to Z-band disrup…

The forces generated by the cat diaphragm (DIA) during different ventilatory and nonventilatory behaviors were determined by measuring transdiaphragmatic pressures (Pdi). The Pdi generated during eupnea was only approximately 12% of the maximum Pdi (Pdimax) generated by bilateral phrenic nerve stimulation. When the animals breathed a gas mixture of 10% O2 and 5% CO2, the Pdi increased to approximately 28% of Pdimax. During total airway occlusion, the Pdi generated by the diaphragm increased to approximately 49% of Pdimax. Only during the gag reflex and sneezing did Pdi reach maximal levels. A …

We have had experience with diaphragm pacing in 24 patients at the Toronto Western Hospital. Fourteen patients have undergone bilateral implants to treat chronic ventilatory insufficiency (CVI) caused by traumatic tetraplegia at the C1/2 level (eight patients), neurogenic apnea (five) and one case of neonatal apnea. Unilateral stimulators for nocturnal pacing have been implanted in five patients with central alveolar hypoventilation (sleep apnea) and five patients who suffered CVI resulting from various etiologies. Of the patients who were ventilatory dependent, 80% were successfully weaned an…

Despite dissemination of modern contraceptive techniques among college women, unplanned pregnancy and pregnancy termination rates remain a major health issue. An investigation of 495 college women who use the diaphragm method of contraception was conducted in order to estimate pregnancy occurrence, to better understand factors related to successful use, and to profile service utilization patterns. Data were collected by medical record audit and client survey. Fifteen percent of the diaphragm clients reported a diaphragm-related pregnancy; clinical effectiveness was estimated to be 88% with a r…

The prepyloric diaphragm is a rare entity, with well-established anatomicopathological features but of unknown etiology. Approximately half of the 180 cases described occurred in children. The clinical presentation may be a chronic partial obstruction as in pyloric stenosis, or a complete gastric outlet obstruction when the orifice of the diaphragm is very small or nonexistent. Despite the variable clinical picture, radiological and endoscopic examinations may reveal typical pathognomonic features. With cases being reported more frequently, the treatment had changed from a wholly surgical appr…

Diaphragm muscle strength was measured as maximal transdiaphragmatic pressure during airway occlusion in ten near-miss sudden infant death syndrome infants aged 4.1 +/- 0.6 (SE) months post-term, range 2 to 7 months, and ten control infants aged 4.5 +/- 0.8 months post-term, range 0.8 to 8 months. In the near-miss sudden infant death syndrome group, the mean maximal transdiaphragmatic pressure was 106 +/- 6 cm H2O, range 78 to 132 cm H2O, compared with a mean maximal transdiaphragmatic pressure 86 +/- 4 cm H2O, range 69 to 106 cm H2O, in the control group. Diaphragm strength is normal or incre…

PURPOSE OF REVIEW: This review is prompted by substantial new information on the composition and structure of the podocyte slit-diaphragm and the identification of anti-nephrin antibodies in patients with acquired diseases of the glomerular podocyte, including steroid-sensitive nephrotic syndrome of childhood, minimal change disease and some cases of primary focal and segmental glomerulosclerosis (FSGS).
RECENT FINDINGS: New methodologies including multi-epitope affinity purification, high resolution proteomics and cryo-electron tomography reveal that the slit-diaphragm consists of a multi-la…

There can be many reasons that damage the function of the diaphragm, either transiently or permanently, involving one hemilate or both muscle portions. The diaphragm is associated only with breathing, but many other functions are related to it. The patient is not always aware of the presence of diaphragmatic dysfunction, and it is not always immediate to identify non-respiratory diaphragmatic symptoms. Pseudoanginal pain, night sweats, difficulty memorizing, or muscular and visceral problems of the pelvic floor are just some of the disorders linked to reduced diaphragmatic contractility. A dec…

KEY POINTS: Piezo channels, known for detecting mechanical pressure, were found to be expressed at the lacuna channel membranes of nephrocytes. Piezo loss of function caused nephrocyte dysfunction, including disrupted slit diaphragm structure and altered lacuna channel morphology. Piezo deficiency led to internalized slit diaphragm proteins, reduced autophagy, increased endoplasmic reticulum stress, and impaired calcium homeostasis.
BACKGROUND: The Piezo gene encodes a highly conserved cell membrane protein responsible for sensing pressure. The glomerular kidney and the slit diaphragm filtrat…

KEY POINTS: Drosophila nephrocytes feature a special basement membrane that may serve to model joint function of the glomerular filtration barrier. Silencing of Drosophila laminin and collagen IV genes reduced the density of slit diaphragms in nephrocytes, showing a direct effect of the matrix. Matrix receptor silencing phenocopied basement membrane disruption, indicating that the matrix guides slit diaphragm position through matrix receptors.
BACKGROUND: The glomerular basement membrane and the slit diaphragm are essential parts of the filtration barrier. How these layers collaborate remains…

PURPOSE OF REVIEW: The purpose of this review is to summarize the role of lung ultrasound and diaphragm ultrasound in guiding ventilator settings with an emphasis on positive end-expiratory pressure (PEEP). Recent advances for using ultrasound to assess the effects of PEEP on the lungs and diaphragm are discussed.
RECENT FINDINGS: Lung ultrasound can accurately diagnose the cause of acute respiratory failure, including acute respiratory distress syndrome and can identify focal and nonfocal lung morphology in these patients. This is essential in determining optimal ventilator strategy and PEEP…

Diaphragm paralysis can lead to significant dyspnoea and, in severe cases, the need for invasive ventilation. Surgical repair via diaphragm plication remains the only surgical option for these patients. Nerve transplantation and pacing have not proven to be effective alternatives. Nevertheless, diaphragm plication, whether through a thoracotomy or via minimally invasive techniques, has proven to yield high recurrence rates during the years following plication surgery. Re-exploration of recurrent cases has identified that plication relapse may be caused secondary to the elasticity of the non-pl…

Diaphragm paralysis may be either idiopathic or associated with several medical conditions including viral and bacterial infection. The association of phrenic nerve palsy with viral infections is rare but well-appreciated in several case reports. Neuropathy, both central and peripheral, is a common neurological consequence of COVID-19. Here, we describe a case of diaphragm paralysis in a woman who was admitted to the hospital because of COVID-19 pneumonia. Post-COVID-19 unilateral paralyzed diaphragm was diagnosed with a chest X-ray for her and the disorder was attributed to COVID-19 because n…

Although the diaphragm represents a critical component of the respiratory pump, the clinical presentations of diaphragm dysfunction are often non-specific and can be mistaken for other more common causes of dyspnoea. While acute bilateral diaphragm dysfunction typically presents dramatically, progressive diaphragm dysfunction associated with neuromuscular disorders and unilateral hemidiaphragm dysfunction may be identified incidentally or by recognising subtle associated symptoms. Diaphragm dysfunction should be considered in individuals with unexplained dyspnoea, restrictive respiratory funct…

Mechanical ventilation (MV) is a life-saving instrument used to provide ventilatory support for critically ill patients and patients undergoing surgery. Unfortunately, an unintended consequence of prolonged MV is the development of inspiratory weakness due to both diaphragmatic atrophy and contractile dysfunction; this syndrome is labeled ventilator-induced diaphragm dysfunction (VIDD). VIDD is clinically important because diaphragmatic weakness is an important contributor to problems in weaning patients from MV. Investigations into the pathogenesis of VIDD reveal that oxidative stress is esse…
