Repair of Stanford type A aortic dissection with ascending aorta and hemiarch replacement combined with stent-graft elephant trunk technique by using innominate cannulation.

OBJECTIVE: The objective was to report our experience with a simplified procedure of ascending aorta and hemiarch replacement with a stent-graft elephant trunk in type A dissections. The efficacy of innominate artery cannulation was investigated. METHODS: From January 2007 to January 2010, 46 patients (35 men, 11 women; mean age, 52.7 ± 12.3 years) with acute type A dissection with 3 vessels of the arch free of lesions received ascending aorta and hemiarch replacement with implantation of a stent graft into the descending aorta under hypothermic circulatory arrest and selective antegrade cere

[The endovascular repair of aortic dissection: early clinical results of 178 cases].

OBJECTIVE: To discuss the safety, feasibility, efficacy and problems of endovascular repair for aortic dissection. METHODS: From June, 1998 to Dec, 2004, 178 aortic dissections were treated by stent-grafts, including 76 acute cases and 102 chronic cases, 19 cases with Stanford A and 159 cases with Stanford B. Under local or general anesthesia, every stent-graft was deployed at the proper position of first tear entry through femoral artery under X-ray fluoroscopic. The changes of hemodynamic in true and false lumen, visceral and limbs blood supply were investigated after operation. RESULTS: 1

Management of aortic root in type A dissection: Bentall approach.

BACKGROUND: We analyzed the results of the modified Bentall procedure in a high-risk group of patients presenting with acute type A aortic dissection (ATAAD). METHODS: ATAAD patients undergoing a modified Bentall between 1996 and 2018 (n = 314) were analyzed. Mechanical composite conduits were used in 45%, and biological ones using either a bioprosthesis implanted into an aortic graft (33%) or xeno-/homograft root conduits (22%) in the rest. Preoperative malperfusion was present in 34% of patients and cardiopulmonary resuscitation required in 9%. RESULTS: Concomitant arch procedures consiste

Morphological aspects of myocardial bridges.

Although some myocardial bridges can be asymptomatic, their presence often causes coronary disease either through direct compression of the "tunnel" segment or through stimulation and accelerated development of atherosclerosis in the segment proximally to the myocardial bridge. The studied material contained 30 human hearts received from the Department of Anatomy. The hearts were preserved 3 to 5 days in 10% formalin solution. Thereafter, the fatty tissue was removed and arterial blood vessels prepared by careful dissection with special reference to the presence of the myocardial bridges. Leng

Treatment of unusually located traumatic intracranial aneurysms and severe vasospasm following a gunshot wound to the head: A case report.

BACKGROUND: Traumatic intracranial aneurysms (TICAs) represent up to 1% of all intracranial aneurysms. They can be the result of non-penetrating and penetrating brain injury (PBI). Approximately 20% of TICA are caused by PBI. Endovascular treatments as well as surgical clipping are reported in the literature. Other vascular complications of PBI include vasospasm although the literature is lacking on this topic. CASE DESCRIPTION: The authors present a unique case of multiple TICAs after a PBI in a 15-year-old patient who sustained a gunshot wound to the head. The patient sustained injury throu

Anatomic considerations in extraperitoneal approach to radical nephrectomy.

The interrelationship between the lateral and posterior portions of the peritoneum and Gerota's fascia allows them to be separated from each other, permitting ready visualization of the major renal vessels. The posterior relationship between Gerota's fascia and the transversalis fascia overlying the more posterior psoas muscle can be used to advantage in exposing the renal vessels posteriorly. That the fascial layers defining the retroperitoneal spaces might therefore be used in dissecting the renal hilum and vessels in the performance of radical nephrectomy prompted the present investigation.

Experimental surgical treatment for recurrent epistaxis.

PURPOSE: To evaluate a new technique of submucosal supraperichondrial (SMSP) dissection of the nasal septum and its subsequent effect on the vascularity of the mucosa. A reduction may decrease the rate of recurrent anterior epistaxis. MATERIALS AND METHODS: The procedure was performed on one side of the nasal septum of 16 laboratory rabbits. After healing occurred 3 months later, the animals' septa were excised and stained. Both sides were then compared by using standardized microscopic field analysis. RESULTS: The reduction in both the number of blood vessels on the operated side of the sep

Anatomy and blood supply of the lower four cranial and cervical nerves: relevance to surgical neck dissection.

This study is a continuation of previous work searching for possible anatomic reasons to explain variable and usually unpredictable postoperative pain and dysfunction after the same nerve losses with similar neck dissection operations. The study consisted of dissections of 19 deceased unpreserved elderly subjects arterially injected with dyed latex. Of the 19 subjects, 14 had brain stem and cervical spinal cord dissections, and all had neck dissections. The findings suggested two possible anatomic reasons for the pain and dysfunction: (i) The intracranial anatomy of the lower four cranial nerv

Anomalous Systemic Arterial Supply to the Left Basal Segment With Arterial Dissection of the Feeding Artery: A Case Report and Literature Review.

Anomalous systemic arterial supply to the left basal segment is a rare congenital vascular anomaly, particularly when complicated by dissection of the feeding artery. This case report aims to present a unique instance of this condition and to review relevant literature. We present a young patient who was hospitalized with sudden chest pain, leading to the discovery of an anomalous artery originating from the thoracic aorta that supplied blood to the basal segment of the left inferior lobe of the lung, accompanied by dissection of the feeding artery. The patient underwent successful surgical re

Common iliac artery injury from blunt trauma: case reports.

Common iliac artery injury from blunt trauma is uncommon. We describe two cases of common iliac artery injury from blunt abdominal trauma resulting in occlusion in one case and aneurysm in the other. The diagnosis of such injuries is based on clinical suspicion, the presence of bruits, pulse change or discrepancy, or continued unexplained blood loss. Vessel wall damage may not manifest itself initially but may result in thrombosis, subintimal hemorrhage, dissection, or aneurysmal dilatation. These can result in hemorrhage, pain, or ischemia developing remotely from the initial trauma.

The quadriceps Femoris allograft as an extension of the Angiosome concept: A cadaveric-based anatomical feasibility study.

BACKGROUND: Vascularised composite allo-transplantation (VCA) is emerging as a tailored approach for complex tissue reconstruction. This study focuses on the quadriceps VCA as a potential solution for tissue repair, following trauma, necrotising fasciitis/myositis, or tumor ablation. METHODS: Dissections were undertaken in 10 adult cadaveric lower limbs to characterize the blood supply to the quadriceps femoris for en bloc muscle allo-transplantation. A mock cadaveric transplantation was performed to (a) define the best neurovascular VCA design and (b) test the feasibility of the procedure. A

Clinical Outcomes of Cuff Anastomosis Combined with Single-Branched Frozen Elephant Trunk for Acute Type A Aortic Dissection.

BACKGROUND: Total arch replacement combined with the frozen elephant trunk (FET) technique has become the predominant surgical strategy for acute type A aortic dissection (ATAAD); however, reconstruction of arch vessel perfusion remains technically demanding and often results in prolonged ischemia associated with branch vessel reconstruction. METHODS: This retrospective single-center study included all patients who were diagnosed with ATAAD and underwent surgical treatment between January 2024 and June 2025. Patients were allocated to either the cuff anastomosis combined with a single-branche

Ureteral Triplication- Laparoscopic Partial Nephrectomy With Highly Selective Arterial Dissection: Description of Surgical Technique.

BACKGROUND/OBJECTIVE: Ureteral triplication is a rare congenital malformation with a wide spectrum of presentation that requires a high index of suspicion in order to diagnose. To date, only about 100 cases have been published in medical literature. Smith et al1 proposed a classification for ureteral triplication that describes it as three separate ureters and three separate ureteral orifices with no interconnection between ureters. In this video, we present a surgical approach for laparoscopic partial nephrectomy of both upper moieties in the context of ureteral triplication. MATERIALS AND M

[Spontaneous occlusion of PICA-involved dissecting aneurysm with development of a collateral channel from the posterior meningeal artery].

A 53-year-old man suffered severe headache, which continued for three days. No abnormality was shown on CT scan, and a dissecting aneurysm of the right vertebral artery was suspected on MRI. Cerebral angiography revealed a dissection aneurysm of the right vertebral artery involved with the posterior inferior cerebellar artery (PICA) as pearl and string sign. The patient was conservatively managed under careful blood pressure control, and was followed by serial MRI. He presented with Wallenberg syndrome three weeks later. Second angiography revealed the occlusion of the PICA-involved dissecting

[Spinal cord infarction due to aortic dissection during coronary artery bypass grafting under percutaneous cardiopulmonary support].

We report a case of aortic dissection due to improper position of a percutaneous cardiopulmonary support (PCPS) cannula into the femoral artery during coronary artery bypass grafting (CABG). A 77-year-old man with 3-vessel disease underwent off-pump CABG (OPCAB). Blood pressure suddenly lowered during bypass grafting to the right coronary artery. PCPS was performed between the left femoral artery and the right atrium. Bradycardia occurred 37 min after initiation of PCPS, and transesophageal echocardiography revealed Stanford type A aortic dissection. By converting the perfusion site from the f

Notching of the femoral neck during resurfacing arthroplasty of the hip: a vascular study.

During hip resurfacing arthroplasty, excessive valgus positioning or surgical technique can result in notching of the femoral neck. Although mechanical weakening and subsequent fracture of the femoral neck are well described, the potential damage to the retinacular vessels leading to an ischaemic event is relatively unknown. Using laser Doppler flowmetry, we measured the blood flow in 14 osteoarthritic femoral heads during routine total hip replacement surgery, before and after notching of the femoral neck. In ten hips there was a reduction in blood flow of more than 50% from the baseline valu

Thoracic Endovascular Repair after Total Aortic Arch Replacement with Frozen Elephant Trunk for Type a Aortic Dissection.

BACKGROUND: The management of residual aortic dissection after initial type A repair with the Frozen elephant trunk technique remains mostly unexplored. This work aimed to evaluate endovascular second-stage surgery for patients with residual aortic dissection. METHODS: A retrospective analysis of consecutive patients that underwent Type A aortic repair with Frozen elephant trunk, followed by a second-stage endovascular procedure was done from March 2016 to December 2021. The primary outcome was aortic-related adverse events or mortality, and secondary outcomes were aortic remodeling and perio
