Successful Conservative Management of Acute Microperforation in a Diverticulum Within a Meckel's Diverticulum: A Case Report.

Meckel's diverticulum is a common congenital gastrointestinal anomaly that can lead to various complications. This case report presents a rare instance of microperforation of a diverticulum within Meckel's diverticulum. A 31-year-old male presented with a two-day history of progressively worsening, intermittent right lower quadrant pain and fever. The patient reported dull pain that migrated from the epigastric region to the right lower quadrant over the course of two days. The pain was exacerbated by movement. Blood tests revealed WBC 13,900/μL and CRP 5.30 mg/dL. Given the patient's characte…

Hypercontractile Esophagus Causing Midesophageal Diverticulum.

Esophageal diverticulum (ED) is a rare condition with a clinical presentation that can be variable. Esophageal diverticulum has been associated with motility disorders; however, the association with mid-ED is less clear. Hypercontractile esophagus, also known as jackhammer esophagus, is a rare motility disorder of peristalsis diagnosed by esophageal high-resolution manometry after exclusion of mechanical obstruction. We describe the second reported case of mid-ED secondary to hypercontractile esophagus successfully treated through robotic diverticulectomy with long myotomy. KEYWORDS: esophagus…

Perforated duodenal diverticulum treated conservatively.

Perforation is the rarest complication of duodenal diverticulum but also one of its most serious complications. Mortality rate was reported to up to 30%. Clinical diagnosis is usually vague and non-specific. High clinical suspicion is important as rapid deterioration is likely. Consensus regarding management is lacking and currently guided by a small series of case reviews. Surgical treatment is historically the standard therapeutic option. However, more current literature suggests improvement in patients with non-surgical management. In this paper, we discuss a case of a patient in her early …

Surgical treatment of Kommerell's diverticulum.

Kommerell's diverticulum (KD) is a rare congenital vascular abnormality due to the formation of an abnormal aortic arch, most commonly associated with dilatation at the proximal part of the aberrant subclavian artery. This article describes an option of surgical treatment in a patient with KD. The proposed new technique of aortic arch surgical reconstruction with a stent-graft placement in the descending aorta reduces CPB time, aortic clamp and circulatory arrest time.

Symptomatic Meckel's Diverticulum in pediatrics.

INTRODUCTION: Meckel's diverticulum (MD) is the remnant of the vitelline duct (VD) also called omphalomesente ric duct and it is considered the most frequent gastrointestinal malformation. Most of the cases are asymptomatic and the diagnosis of this type is always a challenge. OBJECTIVE: To describe 3 sympto matic presentations of MD and to discuss its symptoms, signs, and possible diagnostic-therapeutic tools. CLINICAL CASES: Case 1: A six-month-old patient with obstructive bowel syndrome. In explo ratory laparotomy, an MD was identified with a mesodiverticular band causing an internal hern…

Meckel Diverticulum Causing Intestinal Volvulus.

Intestinal volvulus is rare and responsible for upper bowel obstruction. They occur more frequently in a patient with abdominal surgery history. We report a case of small intestine volvulus on Meckel diverticulum, which occurred in a 21-year-old patient, with no history of laparotomy. The diagnosis confirmation was intraoperative, and the management consisted in a segmental resection of the small intestine with immediate anastomosis. The postoperative follow-up was good. This case underlines the scarcity and the severity of this presentation which therefore requires appropriate care in order t…

[Giant stone complicating calyceal diverticulum].

Calyceal diverticulum is acystic cavity in the renal parenchyma, communicating with the collecting system via an infundibulum. In more than 40% of cases it is associated with a stone and in most cases appears as milk of calcium forming a fluid level or multiple small stones. We here report the case of an 82-year old patient with no previous history, presenting with mild and intermittent right flank pain lasting for 8 years. Clinical examination was normal. The patient first underwent X-ray of urinary tree without preparation showing a density of calcium projecting into the upper pole of the ri…

A Petal-like Calyceal Diverticulum.

Calyceal diverticula are rare outpouchings of the upper collecting system that likely have a congenital origin. They are usually single cystic lesions accompanied by stones inside them and located in the upper pole calyces. We present a case containing a petal-like cystic lesion with multiple septa in the lower pole of the left kidney, which was confirmed as calyceal diverticulum by computed tomography urography and intraoperative findings.
