Cerebral Dysfunction of a Diving Fisherman After Prophylactic Recompression Treatment: A Case Report.

INTRODUCTION: Prophylactic recompression treatment (PRT) is an empirical treatment conducted in China for divers who conduct repetitive diving or asymptomatic insufficient decompression to eliminate residual nitrogen and microbubbles and prevent decompression sickness (DCS). CASE REPORT: We report a case of a 43-year-old healthy diving fisherman who experienced cerebral dysfunction after PRT. The fisherman suddenly experienced dizziness accompanied by vomiting, vertigo, nausea, confusion, and aphasia six hours after a 500kPa-PRT. Laboratory tests revealed a significantly elevated serum creati…

Vertical energy seascapes and diving behaviour modulate metabolic scope in a pelagic predator.

Pelagic fishes must obtain resources in prey-sparse habitats and may be considered energy speculators with maximization, gambling high energy costs (e.g. metabolism) for a high rate of return (prey capture). As such, they may have to carefully use their energy seascape to obtain the resources necessary for high growth rates. For diving animals, their energy seascape will also have a vertical component in addition to a horizontal one, which is rarely considered. Dolphinfish, Coryphaena hippurus, embody the maximization strategy as they have high metabolic rates and fast growth rates. We coupled…

From surfing to diving into the tumor microenvironment through multiparametric imaging mass cytometry.

The tumor microenvironment (TME) is a complex ecosystem where malignant and non-malignant cells cooperate and interact determining cancer progression. Cell abundance, phenotype and localization within the TME vary over tumor development and in response to therapeutic interventions. Therefore, increasing our knowledge of the spatiotemporal changes in the tumor ecosystem architecture is of importance to better understand the etiologic development of the neoplastic diseases. Imaging Mass Cytometry (IMC) represents the elective multiplexed imaging technology enabling the in-situ analysis of up to

Arterial Gas Embolism after breath-hold diving. An interesting case report in Greece.

BACKGROUND: Pulmonary overinflation syndrome (POIS) is a group of barotrauma-related conditions caused by the expansion of gas trapped in the lung or over-pressurization of the lung with subsequent overexpansion and rupture of the alveolar air sacs. We report a case in which neurological sequelae (arterial gas embolism-AGE) and pneumothorax occurred after breath-hold diving. Treatment with Hyperbaric Oxygen Therapy (HBO₂) led to resolution. CASE REPORT: A 20-year-old male diver was transferred to the Athens Naval Hospital with symptoms after breath-hold diving. He developed neurological defic…

Pygmy sperm whale multi-omics data reveal hypoxia adaptations in deep-diving cetaceans.

BACKGROUND: Deep-diving cetaceans tolerate acute hypoxia better than their terrestrial ancestors and shallow-diving counterparts. However, our poor understanding of how genetic factors, cellular functions, and physiological characteristics combine to drive hypoxia adaptation in deep-diving cetaceans remains a critical gap. RESULTS: Here, we studied the genetic basis for this ability by creating a de novo genome assembly for the pygmy sperm whale (Kogia breviceps) and comparatively analyzing genomes from 12 cetacean species, including 2 other deep-diving cetaceans. We also sequenced and compar…

Klebsiella pneumoniae Lung Abscess Due to Aspiration of Seawater During Recreational Free Diving.

Cough persisting for 5 wk, low-grade fever, and thick, pinkish to reddish, foul-tasting sputum made the doctors seeing this patient think of tuberculosis at first. However, a history of aspirating seawater during recreational free diving gave a clearer picture of how this patient got sick. Investigations later revealed a cavitary lesion in the right lung, and sputum culture grew Klebsiella pneumoniae, resulting in a diagnosis of lung abscess. This case is being reported to show this peculiar way that a bacterial pathogen entered the respiratory tree and that recreational free diving poses a he…

Central command activation during exercise is an essential mechanism to sustain diving bradycardia.

Previous studies have shown that the bradycardic response to diving is maintained or enhanced during exercise. However, the integrative mechanism by which diving-induced bradycardia supersedes exercise-induced tachycardia remains unclear. This study aimed to elucidate the contributions of central and peripheral afferent mechanisms to the diving response in humans under controlled laboratory conditions. Thirty-two healthy participants [17 males, 15 females; mean age: 22 ± 3 yr; body mass index (BMI): 24 ± 4 kg/m2] were exposed to simulated diving via trigeminal nerve stimulation (TGS) under thr…

Molecular adaptations in MMP genes support lung elasticity and diving adaptations in cetaceans.

Cetaceans are a unique group of marine mammals that have evolved from terrestrial to fully aquatic life. During diving, they experience extreme physiological challenges, including lung collapse, limited gas exchange, and the risk of decompression-related injuries. The matrix metalloproteinase (MMP) gene family plays a central role in extracellular matrix (ECM) remodeling, vascular repair, and inflammatory responses, and is also involved in the formation and maintenance of elastic fibers-key components that contribute to lung elasticity. Enhanced lung elasticity is thought to facilitate reversi…

Diving Into the Rarity: A Case Report of Nonsyndromic Bilateral Branchial Cleft Fistula.

Bilateral branchial cleft fistulas are extremely rare congenital anomalies that may be part of a larger syndromic association. Prompt recognition and treatment are crucial in syndromic patients to halt disease progression, improve outcomes, and reduce the risk of long-term complications. Complete surgical excision is the standard treatment for fistulas.

Diving deep into healing: the promising role of fish skin in wound recovery.

Fish skin has emerged as a potential candidate for improving wound healing due to its notable results in human trials, in which it has been directly applied as a dressing on wounds. The current review explores the mechanisms by which fish skin can boost the wound healing process. The natural wound healing process involves inflammation at the wound site to initiate tissue repair. The body balances this inflammation through interleukin signaling, and imbalances can cause chronic wounds or scarring. The wound site also secretes epidermal growth factor, which activates the Ras/Raf/MEK/ERK and PI3K

Cardiopulmonary adaptations of a diving marine mammal, the bottlenose dolphin: Physiology during anesthesia.

Diving marine mammals are a diverse group of semi- to completely aquatic species. Some species are targets of conservation and rehabilitation efforts; other populations are permanently housed under human care and may contribute to clinical and biomedical investigations. Veterinary medical care for species under human care, at times, may necessitate the use of general anesthesia for diagnostic and surgical indications. However, the unique physiologic and anatomic adaptations of one representative diving marine mammal, the bottlenose dolphin, present several challenges in providing ventilatory a…

Plunge-diving into dynamic body acceleration and energy expenditure in the Peruvian booby.

Daily energy expenditure (DEE) is the result of decisions on how to allocate time among activities (resting, commuting and foraging) and the energy costs of those activities. Dynamic body acceleration (DBA), which measures acceleration associated with movement, can be used to estimate DEE. Previous studies of DBA-DEE correlations in birds were carried out on species foraging below their thermoneutral zone, potentially decoupling the DBA-DEE relationship. We used doubly labelled water (DLW) to validate the use of DBA on plunge-diving seabirds, Peruvian boobies (Sula variegata), foraging in wate…

South Pacific Underwater Medicine Society (SPUMS) position statement regarding paediatric and adolescent diving.

This paediatric diving position statement was developed from a targeted workshop at the 51st Annual Scientific Meeting of the South Pacific Underwater Medicine Society (SPUMS) on 8 June 2023. It highlights the factors that SPUMS regards as important when undertaking health risk assessments for diving by children and adolescents (defined as aged 10 to 15 years). Health risk assessments for diving should be performed by doctors who are trained in diving medicine and who are familiar with the specific risks which result from breathing compressed gas in the aquatic environment. Undertaking a diver…

Heart rate reduction during voluntary deep diving in free-ranging loggerhead sea turtles.

Air-breathing vertebrates exhibit cardiovascular responses to diving including heart rate reduction (diving bradycardia). Field studies on aquatic mammals and birds have shown that the intensity of bradycardia can vary depending on diving behaviour, such as the depth of dives and dive duration. However, in aquatic reptiles, the variation in heart rate during deep dives under natural conditions has not been fully investigated. In this study, we released five loggerhead sea turtles (Caretta caretta) outfitted with recorders into the sea and recorded their electrocardiogram, depth, water temperat…

Diving into progress: a review on current therapeutic advancements in spinal muscular atrophy.

Spinal muscular atrophy (SMA) is an uncommon disorder associated with genes characterized by the gradual weakening and deterioration of muscles, often leading to substantial disability and premature mortality. Over the past decade, remarkable strides have been made in the field of SMA therapeutics, revolutionizing the landscape of patient care. One pivotal advancement is the development of gene-targeted therapies, such as nusinersen, onasemnogene abeparvovec and risdiplam which have demonstrated unprecedented efficacy in slowing disease progression. These therapies aim to address the root caus

Handedness and individual roll-angle specialism when plunge diving in the northern gannet.

Many vertebrates show lateralized behaviour, or handedness, where an individual preferentially uses one side of the body more than the other. This is generally thought to be caused by brain lateralization and allows functional specializations such as sight, locomotion, and decision-making among other things. We deployed accelerometers on 51 northern gannets, Morus bassanus, to test for behavioural lateralization during plunge dives. When plunge diving, gannets 'roll' to one side, and standard indices indicated that 51% of individuals were left-sided, 43% right-sided, and 6% 'non-lateralized'. …

A review of 149 Divers Alert Network emergency call records involving diving minors.

INTRODUCTION: Minors have been scuba diving for decades, and while the initial concerns about potential long-term complications related to bone development appear to be unfounded, the incidence of scuba diving injuries among them has been poorly studied. METHODS: We reviewed 10,159 cases recorded in the DAN Medical Services call centre database from 2014 through 2016 and identified 149 cases of injured divers younger than 18 years. Records were analysed for case categorisation on the most common dive injuries. Information about demographics, level of training, risk factors, and relevant behav…

Lethal event in diving with self-contained underwater breathing apparatus: A forensic study.

INTRODUCTION: In fatal accidents during scuba diving, it is necessary to differentiate a death from primary drowning from a secondary drowning death that is primarily due to other etiopathogenetic factors. The inhalation of water can only be the end point of a series of events capable of causing the diver's exitus. This study aims to demonstrate that even low-risk heart disease in daily life become potentially fatal during scuba diving. METHODS: In this case series, we describe all cases of deaths in the course of diving that came to the observation of the Forensic Institute of the University…

Bilateral pneumatic blowout fractures in a patient with silent sinus syndrome while diving.

A 37-year-old scuba diver developed sudden severe headache, mid-facial pressure/pain and diplopia while diving. Upon examination, he had signs and symptoms consistent with silent sinus syndrome (SSS), including bilateral enophthalmos, hypoglobus, and diplopia/strabismus in conjunction with CT findings of occluded maxillary sinus ostia, atrophic/collapsed maxillary sinuses, and bilateral orbital floor fractures with fat herniation. As there was no history of trauma or chronic sinusitis, this rare case of bilateral SSS was deemed the cause of the spontaneous fractures (i.e. barotrauma secondary …
