Sensitivity of ivacaftor to drug-drug interactions with rifampin, a cytochrome P450 3A4 inducer.

The CFTR potentiator ivacaftor is responsible for significant clinical improvements among a subset of patients with cystic fibrosis. Because it is a substrate of the cytochrome P450 system, specifically CYP3A4/5, ivacaftor is subject to significant drug-drug interactions, including due to commonly used antimicrobials such as rifampin. While the interaction of rifampin and ivacaftor has been examined in vitro, severe adverse events resulting from this interaction have not been reported in the literature. In this report, we describe the termination of steady, long-term improvement in a patient t…

Adverse reactions and drug-drug interactions in the management of women with postmenopausal osteoporosis.

The pharmacological management of disease should involve consideration of the balance between the beneficial effects of treatment on outcome and the probability of adverse effects. The aim of this review is to explore the risk of adverse drug reactions and drug-drug interactions with treatments for postmenopausal osteoporosis. We reviewed evidence for adverse reactions from regulatory documents, randomized controlled trials, pharmacovigilance surveys, and case series. Bisphosphonates are associated with gastrointestinal effects, musculoskeletal pain, and acute-phase reactions, as well as, very…

Drug-Target Interaction Prediction via Dual-Interaction Fusion.

Accurate prediction of drug-target interaction (DTI) is crucial for modern drug discovery. However, experimental assays are costly, and many existing computational models still face challenges in capturing multi-scale features, fusing cross-modal information, and modeling fine-grained drug-protein interactions. To address these challenges, We propose Gated-Attention Dual-Fusion Drug-Target Interaction (GADFDTI), whose core contribution is a fusion module that constructs an explicit atom-residue similarity field, refines it with a lightweight 2D neighborhood operator, and performs gated bidirec

Potentially Clinically Relevant Drug-Drug Interactions with Direct Oral Anticoagulants: Prevalence and Contributing Factors.

BACKGROUND: Direct oral anticoagulants (DOACs) are widely prescribed but remain susceptible to drug-drug interactions (DDIs) that increase bleeding or thrombotic risk. The real-world prevalence and factors associated with potentially clinically relevant DDIs in patients on chronic DOAC therapy require further study. OBJECTIVE: To determine the prevalence of clinically relevant DDIs involving DOACs and identify patient- and treatment-related factors associated with their occurrence. METHODS: A retrospective cross-sectional study was conducted among hospitalized adults receiving a DOAC as a ho…

Polypharmacy and Drug-Drug Interactions in Chronic Obstructive Pulmonary Disease: A Narrative Clinical Review.

Background: Chronic obstructive pulmonary disease (COPD) is commonly managed alongside multimorbidity, polypharmacy, recurrent treatment escalation, and older age, all of which increase vulnerability to drug-drug interactions (DDIs). We aimed to synthesize the main DDI domains relevant to COPD pharmacotherapy and to distinguish harmful DDIs from beneficial combination therapy and formal compatibility findings. Methods: We performed a narrative review using structured literature searches and citation tracking to evaluate COPD-related studies. We prioritized direct COPD-specific DDI evidence, wh…

Immune checkpoint blockade reshapes drug-associated toxicity: a pharmacovigilance atlas of drug-ICI interactions.

BACKGROUND: Immune checkpoint inhibitors (ICIs) are usually treated as direct culprits in immune-related adverse events, but checkpoint blockade may also reset tolerance to other medications. We tested whether ICI exposure reshapes the organization, drug specificity and timing of reported treatment toxicity. METHODS: We analyzed 13,701,106 deduplicated FDA Adverse Event Reporting System reports from 2016 through 2025. Cancer-restricted reporting associations and cross-organ community detection characterized the ICI-associated toxicity landscape. Adjusted logistic models tested primary-suspect

Adverse Drug Reactions and Drug Interactions in Multimorbid Patients: A Review of Current Evidence.

The rising prevalence of multimorbidity, defined as the coexistence of two or more chronic health conditions, is particularly pronounced among older adults and often necessitates polypharmacy. This increases the risk of adverse drug reactions (ADRs) and drug-drug interactions (DDIs), yet multimorbid individuals remain underrepresented in clinical trials. This narrative review synthesizes evidence from 55 recent sources, including randomized controlled trials, observational studies, systematic reviews, and real-world data, to evaluate the burden, risk factors, and management strategies for ADRs…

Pharmacokinetic drug-drug interactions of JBPOS0101 mediated by cytochrome P450 3A4 and UDP-glucuronosyltransferases.

JBPOS0101 is a new antiepileptic drug and is a substrate of UDP-glucuronosyltransferases (UGTs) in in vitro test. In vitro experiments showed different results regarding whether JBPOS0101 induces (EC50 136 μM) or inhibits (IC50 95.4-386.5 μM) cytochrome P450 (CYP) 3A4. As co-medication of JBPOS0101 and carbamazepine (CBZ) is expected in clinical settings, drug-drug interactions (DDIs) between them should be determined. This study aimed to investigate pharmacokinetic (PK) interactions of JBPOS0101 influenced by CYP3A4 and UGTs using midazolam (MDZ) and CBZ. A two-cohort, open-label, fixed-seque…

Switching from cangrelor to oral P2Y12 inhibitors: a focused review on drug-drug interactions.

INTRODUCTION: Cangrelor, the only intravenous platelet P2Y12 receptor inhibitor, is characterized by a prompt and potent platelet inhibition, with a rapid offset of action. Large-scale clinical trials have shown that cangrelor reduce peri-procedural thrombotic events among patients undergoing percutaneous coronary interventions and not pre-treated with an oral P2Y12 receptor inhibitor. However, high P2Y12 receptor occupancy provided by cangrelor raises concerns for drug-drug interactions (DDIs) when transitioning to oral P2Y12 inhibitors. AREAS COVERED: An understanding of the pharmacology of…
