Understanding Postmarketing Requirements and Commitments for CYP3A-Mediated Drug-Drug Interactions: An Analysis of FDA-Approved Drugs From 2015 to 2024.

Addressing knowledge gaps at drug approval through drug-drug interaction (DDI)-related postmarketing requirements (PMRs) and commitments (PMCs) is essential for safe and effective use. This analysis reviewed CYP3A-related PMRs and PMCs, the most evaluated enzyme in postmarketing studies, to characterize their scope. CYP3A-related PMRs, PMCs, and labeling recommendations were identified for small-molecule new molecular entities approved by the U.S. Food and Drug Administration (2015-2024) using the Certara Drug Interaction Database (DIDB). The latest product labels from Drugs@FDA were reviewed

Experimental Quantification of Interactions Between Drug Delivery Systems and Cells In Vitro: A Guide for Preclinical Nanomedicine Evaluation.

A major component of designing drug delivery systems concerns how to amplify or attenuate interactions with specific cell types. For instance, a chemotherapeutic might be functionalized with an antibody to enhance binding to cancer cells ("targeting") or functionalized with polyethylene glycol to help evade immune cell recognition ("stealth"). Even at a cellular level, optimizing the binding and uptake of a drug carrier is a complex biological design problem. Thus, it is valuable to separate how strongly a new carrier interacts with a cell from the functional efficacy of a carrier's cargo once

Off-label use of antidepressants, antipsychotics, and mood-stabilizers in psychiatry.

Off-label drug prescribing in psychiatry is increasing. Many psychotropic drugs are approved for psychopathologic syndromes rather than based on international standard diagnostic classification systems which might facilitate the clinical decision for off-label prescriptions. The objective of this study was to analyze the prevalence and category of off-label use of psychotropic drugs. The study was conducted in 10 psychiatric hospitals in Germany over a period of 2 years. Prescription data of all patients were retrospectively analyzed after identification of antidepressants, antipsychotics, and

The prevalence and consequences of support for off-label Ozempic prescriptions.

Ozempic and related semaglutide drugs represent a popular new strategy to address obesity in the United States, yet uptake of these medications has sparked opposition highlighting concerns about off-label drug use policies, drug safety, supply shortages and cost. Public attitudes towards off-label prescribing by physicians broadly, and towards Ozempic in particular, in light of this opposition are unclear. To better understand public sentiment on this topic, we analysed data from a representative survey of 3,420 US adults conducted from 13 to 22 June 2023. Public attitudes towards off-label pr

A phase IV randomised, open-label pilot study to evaluate switching from protease-inhibitor based regimen to Bictegravir/Emtricitabine/Tenofovir Alafenamide single tablet regimen in Integrase inhibitor-naïve, virologically suppressed HIV-1 infected adults harbouring drug resistance mutations (PIBIK study): study protocol for a randomised trial.

BACKGROUND: Currently recommended boosted protease-inhibitor (bPI) regimens may be associated with increased risk of cardiovascular or chronic kidney diseases; in addition, boosted regimens are particularly associated with drug-drug interactions. Since both cardiovascular and renal disease, and polypharmacy, are common in ageing people with HIV, there is a need for alternative efficacious regimens. bPI-based regimens are often the treatment of choice for individuals with pre-treatment or treatment-acquired resistance but it is plausible that carefully selected HIV-positive individuals with dru

Pharmacokinetics, Safety, and Dosing of Antiseizure Medications in Patients with Renal or Hepatic Impairment.

Ten newer antiseizure medications (ASMs) approved since 2000 and selected for this review (cenobamate, brivaracetam, eslicarbazepine acetate, lacosamide, perampanel, fenfluramine, ganaxolone, cannabidiol, stiripentol, and rufinamide) have broadened treatment options for drug-resistant epilepsy. However, concurrent renal or hepatic impairment alters drug disposition through diminished clearance, shifted protein binding equilibria, accumulation of active metabolites, and variable dialytic removal that standard dosing fails to accommodate. Despite the rising clinical overlap of epilepsy with chro

A gamma-secretase inhibitor decreases amyloid-beta production in the central nervous system.

OBJECTIVE: Accumulation of amyloid-beta (Abeta) by overproduction or underclearance in the central nervous system (CNS) is hypothesized to be a necessary event in the pathogenesis of Alzheimer's disease. However, previously, there has not been a method to determine drug effects on Abeta production or clearance in the human CNS. The objective of this study was to determine the effects of a gamma-secretase inhibitor on the production of Abeta in the human CNS. METHODS: We utilized a recently developed method of stable-isotope labeling combined with cerebrospinal fluid sampling to directly measu

Regulatory Readiness for Paper Replacement in Global Pharmaceutical Electronic Labeling: Cross-Sectional Comparative Policy Analysis.

BACKGROUND: Accurate and up-to-date medicinal product information is essential for the safe and effective use of medicines. Electronic labeling (e-labeling) has emerged as a potential alternative to paper-based package inserts by enabling timely updates, improving accessibility, and supporting integration with digital health systems. However, regulatory approaches to pharmaceutical e-labeling remain fragmented across jurisdictions. OBJECTIVE: This study examined how e-labeling policies have developed across International Coalition of Medicines Regulatory Authorities (ICMRA) member jurisdictio

Sun protection and sunscreen labeling--an update.

Sun exposure is today well recognized as having an adverse effect on human skin. Part of sun radiation, ultraviolet radiation A (UVA) and B (UVB), can modify skin structures and induce short-term skin changes (sunburn, tanning, hyperkeratinization, brown spots) and long-term skin damages (accelerated skin aging and skin cancers). Protection against both UVA and UVB is very important, therefore sun protection by clothes, avoiding sun exposure and correct use of sunscreens are important means to reduce short- and long-term solar radiation effects. The recommendation of appropriate sunscreen by d

The Best Pharmaceuticals for Children Act and Pediatric Research Equity Act reach the age of majority-An oncology perspective.

The scarcity of adequate pediatric drug labeling information has long been problematic in the pediatric population, which may place children at risk for adverse drug effects. The ontogeny of infants, children, and adolescents over the course of the first two decades of life pose complex pharmacokinetic, dosing, administration, effectiveness, and toxicity-related questions that require specific investigation. Here, we review the history that led to the passage of the Best Pharmaceuticals for Children Act (BPCA) and Pediatric Research Equity Act (PREA), and provide commentary on issues relevant …

Drug-induced QT Prolongation Atlas (DIQTA) for enhancing cardiotoxicity management.

Drug-induced prolongation of the QT interval is common in a variety of pharmaceutical treatments and can lead to serious clinical outcomes. Although substantial efforts have been made to prevent drug-induced QT interval prolongation, the lack of a centralized data source remains the main obstacle to further study of the underlying mechanism and the development of effective prediction strategies. To fill this gap, we propose a schema for stratifying the risk of marketed QT prolonging drugs based on US Food and Drug Administration (FDA)-approved drug labeling and developed a Drug-Induced QT Prol…

Done EAZY: An Automated Procedure for 89Zr-Radiolabeling and Size-Exclusion Chromatography Purification of Nanoliposomal Anticancer Therapeutics.

Liposomal nanomedicines offer a clinically proven platform for enhanced drug delivery, yet their behavior in vivo remains difficult to characterize. Noninvasive positron emission tomography (PET) imaging with radiolabeled liposomes enables quantitative insight into their biodistribution and pharmacokinetics, supporting the rational design and clinical translation of nanocarrier-based therapeutics. Zirconium-89 ([89Zr]Zr) is a positron emitter with a suitable half-life (t1/2 = 78.4 h) for tracking long-circulating nanocarriers such as PEGylated liposomes by PET imaging. However, existing manual

Unlicensed and off-label uses of medicines: definitions and clarification of terminology.

The terms 'licensed', 'unlicensed', and 'off-label', often used in relation to marketing and prescribing medicinal products, may confuse UK prescribers. To market a medicinal product in the UK requires a Marketing Authorization ('product licence') for specified indications under specified conditions, regulated by the Medicines and Healthcare products Regulatory Agency (MHRA). The Marketing Authorization includes the product's agreed terms of use (the 'label'), described in the Summary of Product Characteristics (SmPC). Prescribing a licensed product outside those terms is called 'off-label' pr

Inadequately written prescriptions: prescriber rationales for "take as needed" and "take as directed.".

An audit of a large sample of prescriptions written by 39 physicians revealed considerable variation in the rate at which they specified adequate dosage instructions. When asked for their rationales for saying "Take as needed" or "Take as directed" on prescriptions, the physicians cited several circumstances where these instructions may be indicated. "Take as directed;; is defended 1) when the patient has a history of compliance with the regimen being prescribed; 2) when the prescription is accompanied by verbal or auxiliary written instructions; 3) when oral contraceptives are ordered; and 4)
