A review of the structure-function relationships in normal, diseased and reconstructed middle ears is presented. Variables used to describe the system are sound pressure, volume velocity and acoustic impedance. We discuss the following: (1) Sound can be transmitted from the ear canal to the cochlea via two mechanisms: the tympanoossicular system (ossicular coupling) and direct acoustic stimulation of the oval and round windows (acoustic coupling). In the normal ear, middle-ear pressure gain, which is the result of ossicular coupling, is frequency-dependent and smaller than generally believed. …

An aberrant internal carotid artery (a-ICA) in the middle ear is rare, but it may be injured during routine otological procedures. This review examines 48 publications on its embryological basis, clinical presentation, imaging diagnosis, and management. The a-ICA results from agenesis of the first embryonic segment of the internal carotid artery, with collateralisation through the inferior tympanic artery. Presentation is often non-specific; hearing loss (47.7%) and pulsatile tinnitus (30.0%) are the most frequent symptoms. Otoscopy may show a pulsatile retrotympanic mass in the anteroinferior…

Middle ear adenomatous neuroendocrine tumors (MEANT) are rare neoplasms frequently misdiagnosed due to their nonspecific clinical presentations. We present the case of a 25-year-old woman who reported a 2-month history of tinnitus, dizziness, and left-sided conductive hearing loss. Otoendoscopy and computed tomography revealed a well-circumscribed, vascular soft-tissue mass in the epitympanum extending to the mastoid antrum, which was subsequently managed with complete transcanal endoscopic excision. Histopathological evaluation confirmed a well-differentiated neuroendocrine tumor. A definitiv…

Tophaceous gout can rarely present in the middle ear as a mass-like lesion, causing conductive hearing loss. Noncontrast high-resolution computed tomography (HRCT) of the temporal bone plays a significant role in the diagnosis. Awareness of this condition among radiologists is important since it presents a distinctive appearance on HRCT. We present a case of tophaceous gout of the middle ear diagnosed with photon-counting computed tomography (PCCT). Teaching point: The presence of a partially calcified mass with a semolina-like appearance within the middle ear is highly suggestive of tophaceou…

Acute radiotherapy (RT)-induced external ear soft tissue changes start with erythema and dry desquamation and may progress to moist desquamation and epidermal ulceration. Chronic RT-induced changes include epithelial atrophy and subcutaneous fibrosis. Although RT-induced radiation dermatitis has been well studied, interventions for soft tissue disease involving the external auditory canal (EAC) warrant investigation. Medical management includes topical steroid treatment for EAC radiation dermatitis and topical antibiotic therapy for suppurative otitis externa. Hyperbaric oxygen and pentoxifyll…

A 28-year-old male presented with non-pulsatile tinnitus, rightsided hearing loss and an ipsilateral pulsating ear drum. A CT-scan revealed an aberrant internal carotid artery in the middle ear. This is an uncommon finding. Recognition of this birth analomy is important since ear manipulation or surgery can lead to life-threatening complications.

BACKGROUND: The reconstructed auricle projection is an essential element of ear reconstruction. The novel use of an ear-shaped film with one or two "legs" can successfully provide a healthy auricular contour, length, and width, hence improving the three-dimensional (3D) contour of the reconstructed auricle.
METHODS: Sixty-one patients (31 men and 30 women) with unilateral ear reconstruction (22 on the left and 39 on the right side) who underwent auricular reconstruction using the novel ear-shaped film at the Plastic Surgery Hospital of the Chinese Academy of Medical Sciences between February …

Congenital anomalies of the external auditory canal (EAC) are classically divided into congenital aural atresia (CAA) and congenital aural stenosis (CAS). CAA can present as an isolated anomaly, unilateral or bilateral, or in the setting of a craniofacial syndrome. Hearing testing (ABR with air and bone conduction thresholds for both ears) early in the perinatal period is important to document hearing thresholds. Hearing status thus informs parent counseling on options for hearing habilitation: Bone conducting technology is a must for children with bilateral CAA to support normal speech and la…

Branchial cleft anomalies (BCA) are among the most common congenital anomalies found in the pediatric head and neck. The embryology of these congenital anomalies is well understood, which allows clinicians to anticipate their diagnosis when a pediatric patient presents with a head or neck mass. The predictable anatomy of the various types of BCA allows for improved surgical planning to prevent recurrence and ensure complete resection. This report details an unusual location of a first BCA located in the ear lobule of a 10-month old male. There has been no documented first BCA at the ear lobule…

INTRODUCTION: Glial heterotopia is defined as the presence of normal glial tissue in an abnormal site.
CASE REPORT: We report the case of a 63-year-old man who presented with right hearing loss and tinnitus. ENT examination, computed tomography and magnetic resonance imaging showed signs of chronic otitis media. Right mastoidectomy demonstrated a solid mass of the right mastoid and middle ear with no associated dehiscence of the tegmen tympani. Histological examination of the mass revealed normal glial tissue.
DISCUSSION: Glial heterotopia is a rare entity, which must be distinguished from e…

This review describes congenital anomalies of the external ear, which are common and include a wide range of malformations. Attentiveness to other malformations in newborns with ear anomalies is important, as they often appear as part of syndromes such as Goldenhar syndrome, Treacher Collins syndrome and branchio-oto-renal syndrome. This review is an overview of the most common congenital anomalies of the external ear and their treatment, including microtia, constricted ears, preauricular pits, tags and prominent ears.

The external ear is often involved in facial trauma. This is an updated review of the major conditions involved in external ear traumas: the auricular haematoma, the auricular perichondritis and the auricular laceration. These conditions must be quickly diagnosed and treated due to a risk of a severely deformed ear if left untreated. This is also a brief summary of the possibilities in reconstructive surgery of the ear. There is no high-quality level of evidence regarding the management for the various types of auricular trauma, which is why, further research is needed in this field.

INTRODUCTION: Actinomycosis is a saprophytic infection caused by actinomycetes. Actinomycetes is a gram positive, anaerobic, non acid-fast, filamentous bacterium. Although actinomyce is considered as a part of the normal flora, it is rarely seen in middle ear and mastoid cavity.
AIM: is to report a rare case of Actinomycosis infection of the middle ear and mastoid cavity.
CASE REPORT: We presented A 24 years old male arrived to our outpatient department complaining of right-sided otalgia and hearing loss for 8 years, no history of otorreah, vertigo or dizziness. He had a history of right-sid…
