OBJECTIVES: Reconstruction with autologous costal cartilage remains the gold standard surgical treatment for total or partial ear reconstruction. The different auricular subunits are sculpted from costal cartilage and assembled. The assembly requires a particular skillset and training, including a thorough three-dimensional (3D) understanding of auricular anatomy. The correct positioning of the ear framework can be particularly difficult in patients with hemifacial microsomia because of facial asymmetry. We developed a digital workflow that can help the surgeon in the correct positioning of th

Due to the variety in the shape of dysmorphic cartilage, tragus reconstruction is one of the most challenging goals in otoplasty. The authors describe a method to reconstruct a prominent tragus in a simple way suitable for accounting for the size, shape, and location of the remaining ear. We present a case of tragus deformity in an 11-year-old female patient after a previous excision of pretragal skin tags. There was a small remnant of the deeply located dystopic cartilage in a horizontal orientation. The dystopic cartilage was used to reconstruct the tragus using a chondrocutaneous transposit

OBJECTIVES: The aim of the study was to determine if methimazole applied in a transdermal formulation to the internal pinna will cross to the external pinna in an in vitro Franz cell model.
METHODS: The ears from six cats were harvested soon after death. Whole ears were mounted onto Franz-type diffusion cells with the stratum corneum of the inner pinnae uppermost. A commercial transdermal preparation containing methimazole (0.1 ml/10 mg) was applied to the inner pinnae. At 1, 2, 4, 6, 8, 12, 18, 24 and 30 h, a 200 µl sample of reservoir solution was removed to determine the methimazole concen

BACKGROUND: Postoperative auricular perichondritis is a problematic and embarrassing surgical complication after an endaural approach tympanoplasty. However, a search through the literature showed only a few papers regarding postoperative auricular perichondritis, and the reported treatment experience is scarce. This study reviews the clinical aspects of postoperative auricular perichondritis and the different treatment methods.
METHODS: The medical records of 8 patients with postoperative auricular perichondritis over a 5-year period were reviewed. Five patients were treated using the wide e

Auricular cartilage reconstruction in microtia remains a formidable challenge in regenerative medicine. Although cartilage tissue engineering holds substantial promise, achieving reliable and efficient chondrogenesis remains a pressing challenge owing to the intrinsically quiescent phenotype of chondrocytes. We present engineered platelet-derived extracellular vesicles (PEVs) that enhance auricular chondrocyte chondrogenesis in a 3D-printed hydrogel, enabling safe and efficient ear regeneration. Initially, we demonstrated PEVs activated auricular chondrocytes, and sequencing identified miR-92a

The objective of the present study was to evaluate the possibility of the application of the cyanoacrylate-based glue for the strengthening of the reconstructed elements of the middle ear and its influence on the regeneration of the cartilaginous tissue. We used the cartilaginous tissue from the auricles of the male California rabbits as a model. The cartilage was destroyed in a standard press. Half of the cartilage thus fragmented was implanted into the left auricle. The remaining part was mixed up with the cyanoacrylate glue and implanted into the right auricle of the same animal. The implan

Some modified surgical techniques are described for constructing a deep conchal cavity and pseudomeatus and obtaining high auricular projection in congenital microtia. At the primary operation, a rather small portion of the microtic vestige is utilized for the lobule by switching, sparing the skin for the concha, with no free skin graft used. For higher projection of the auricle, three-dimensional transposition of a retroinfraauricular flap together with cartilage pieces underneath is applied to the cephaloauricular sulcus. A deep conchal cavity is constructed by further removal of the soft ti

Constricted ears are characterized by four features: (a) lop deformity, (b) protrusion, (c) low ear position, and (d) decreased ear size. These deformities, resulting from inadequate length of the helix, have been described by Tanzer (1975) as a purse-string closure of the ear. Constricted ears are classified into types 1, 2, and 3 according to the severity of the deformity. Type 3 constricted ear, the most severe, with decreased size and loss of the upper half of the normal ear contour, is classified currently as a mild form of microtia. Therefore, autogenous rib cartilage was used to reconst

BACKGROUND: Congenital auricular deformities are common diseases in newborn infants. We compared the efficacy of 2 ear molding systems in the nonsurgical management of newborn auricular deformities in Chinese infants.
METHODS: A total of 462 newborns with auricular deformities were treated with either the EarWell or the LiangEar ear molding systems. The posttreatment outcome was graded as excellent, fair, and poor. The differences in effectiveness between the 2 ear molding systems and factors that may affect the posttreatment outcome were analyzed.
RESULTS: Both ear molding systems showed su

A total of five ears with perichondritis secondary to burns was treated using a method of excising cartilage. This treatment resulted in the need for general anesthesia, frequent repeat debridement and frequent loss of structure of the ear with subsequent deformity. The presentation, etiology, pathology and present modes of therapy are presented. Our experience stresses the importance of early recognition and treatment and further consideration for the use of drains with continuous antibiotic and proteolytic enzyme administration.

BACKGROUND: The authors established the hypothesis that chest wall deformity can be absolutely avoided by leaving the perichondrium completely intact at the donor site for cartilage regeneration. As described in Part I, on the new method of costal cartilage harvest, the authors were able to confirm regeneration of cartilage visually and with palpation at 6 months postoperatively.
METHODS: The regenerated cartilage of four patients at 6, 8, 10, and 12 months, respectively, after the first-stage operation was harvested for morphological and histological evaluation and analyses during the second

BACKGROUND: The authors performed two-stage ear reconstruction using autogenous costal cartilage alone or combined with canaloplasty of the acoustic meatus to improve the functional results with minimal operative stages.
METHODS: In the first stage, lobule transposition, fabrication of the cartilage framework, and implantation of the framework were performed. In the second stage, elevation of the auricle, coverage with the mastoid fascia flap and skin graft, and concha excavation were performed. Canaloplasty was combined simultaneously in 27 patients with radiologic and audiometric evidence o

The authors present their combined experience with large cartilage-perichondrial composite grafts used to reconstruct total tympanic membrane perforations in 294 ears. Patients chosen for this procedure had failed earlier tympanoplasty surgery or were identified as poor candidates for conventional facial tympanoplasty because of the perforation size. Successful perforation closure was achieved in 97% of ears with chronic otitis media characterized by absence of the tympanic membrane, including portions of the anterior annular ligament. Hearing results in general were good, considering the adva

Ear reconstruction surgeries for congenital deformities and trauma are common, highlighting the need for improved cartilage regeneration. Lactoferrin (LF), a natural and cost-effective protein, is promising due to its anti-inflammatory, antimicrobial, and prochondrogenic properties. This study investigates the effects of LF on the viability, proliferation, and chondrogenesis of rabbit auricular chondrocytes. For in vitro studies, auricular chondrocytes were cultured for three passages, after which 3D pellets were formed. LF significantly increased chondrocyte metabolic activity by 1.5 times at

A rhinoplasty technique involving lower lateral cartilage (LLC) repositioning and grafting of ear cartilage allows for nasal lengthening and nasal tip mobility. For better esthetic outcomes in patients with cleft lip nasal deformity (CLND), we combined cosmetic nasal tip plasty with standard techniques. We performed open rhinoplasty combined with reverse-U incision and V-Y plasty in 62 patients with secondary unilateral CLND from October 2013 to June 2018. Both LLCs were isolated, repositioned, approximated medially, and advanced with a Medpor strut, followed by tip suture technique. To furthe…
