Physical therapy and rehabilitation issues for patients supported with extracorporeal membrane oxygenation.

Extracorporeal Membrane Oxygenation (ECMO) is widely used to support patients who have failed conventional therapies for cardio-respiratory failure. Patient immobility during management of critical illness and ECMO support can result in physical impairment that can lead to prolonged hospitalization and poor functional outcomes for ECMO survivors. Although little information regarding the role of physical therapy in improving functional outcomes for ECMO patients is available, early intervention with physical therapy may decrease duration of hospitalization and improve functional outcomes for p

The juvenile gangliosidoses: A timeline of clinical change.

BACKGROUND: The gangliosidoses are rare inherited diseases that result in pathologic accumulation of gangliosides in the central nervous system and other tissues, leading to severe and progressive neurological impairment and early death in the childhood forms. No treatments are currently approved for the gangliosidoses, and development of treatments is impaired by limited understanding of the natural history of these diseases. OBJECTIVE: The objective of this study is to improve understanding of the juvenile gangliosidoses phenotypes and the late-infantile phenotypic subtype. METHODS: Throug

The nature of in vivo mechanical signals that influence cartilage health and progression to knee osteoarthritis.

Knee osteoarthritis is a disease that can be initiated along multiple pathways that ultimately leads to pain, loss of function and breakdown of the articular cartilage. While the various pathways have biological and structural elements, the mechanical pathways play a critical role in the development of the disease. The forces and motions occurring during ambulation provide mechanical signals sensed at the scale of the cell that are critical to healthy joint homeostasis. As such, ambulatory changes associated with aging, obesity, or joint injury that occur prior to the development of symptoms o

Surgical Considerations for the Acute and Chronic Charcot Neuroarthropathy of the Foot and Ankle.

Charcot neuroarthropathy (CN) of the foot/ankle is a devastating complication that can occur in neuropathic patients. It is a progressive and destructive process that is characterized by acute fractures, dislocations, and joint destruction that will lead to foot and/or ankle deformities. Early diagnosis is imperative, and early treatment may be advantageous, but the condition is not reversible. There is no cure for CN but only treatment recommendations. Ultimate goals of care should include providing a stable limb for ambulation and no ulcerations.

Role of ambulation to prevent venous thromboembolism in medical patients: where do we stand?

Venous thromboembolism (VTE) encompasses the spectrum of manifestations of deep venous thrombosis and/or pulmonary embolism and is a common, … t to the influence of different levels of immobility on the magnitude of VTE risk and the role that early ambulation as an isolated factor plays in prevention of such events. Understanding these limitations is a prerequisite for the proper use and interpretation of VTE risk assessment tools and for indicating the best strategy for preventing venous thrombosis in h …

Comparison of lateral quadratus lumborum and lumbar plexus blocks for postoperative analgesia following total hip arthroplasty: a randomized clinical trial.

INTRODUCTION: Effective analgesia after total hip arthroplasty must minimize pain and optimize early ambulation. Lumbar plexus blocks (LPBs) provide analgesia but may cause motor weakness. Quadratus lumborum blocks (QLBs) may provide analgesia with preserved motor strength. METHODS: This trial randomized subjects scheduled for elective hip arthroplasty to receive an LPB or lateral QLB for postoperative analgesia. The primary outcome was opioid consumption at 12-hour postoperative. Non-inferiority of lateral QLBs compared with LPBs was conducted using a one-sided two-sample t-test. Secondary o…

Functional Recovery From Prolonged Severe Paraparesis in Patients With Spinal Dural Arteriovenous Fistula: A Report of Two Cases.

A spinal dural arteriovenous fistula (sdAVF) is a rare spinal vascular condition often associated with delayed diagnosis and progressive myelopathy. Long disease duration and severe motor deficits before treatment initiation are generally associated with poor functional outcomes. We report two cases of sdAVF in patients presenting with prolonged symptom duration and severe lower limb paralysis. Despite these unfavourable prognostic indicators, both patients underwent early postoperative rehabilitation aimed at activating the central pattern generator through intensive weight-bearing exercises

Myxomavirus Serp-1 Protein Ameliorates Inflammation in a Mouse Model of Duchenne Muscular Dystrophy.

Duchenne muscular dystrophy is an X-linked disease afflicting 1 in 3500 males that is characterized by muscle weakness and wasting during early childhood, and loss of ambulation and death by early adulthood. Chronic inflammation due to myofiber instability leads to fibrosis, which is a primary cause of loss of ambulation and cardiorespiratory insufficiency. Current standard of care focuses on reducing inflammation with corticosteroids, which have serious adverse effects. It is imperative to identify alternate immunosuppressants as treatments to reduce fibrosis and mortality. Serp-1, a Myxoma v

Intensity Modulated Exoskeleton Gait Training Post Stroke.

Stroke is a leading cause of long-term disability. While major advances have been made in early intervention for the treatment of patients post stroke, the majority of survivors have residual mobility challenges. Recovery of motor function is dependent on the interrelationship between dosing, intensity, and task specific practice applied during rehabilitation. Robotic exoskeleton (RE) based gait training utilizes progressive repetitive task-oriented movements to promote functional recovery. The purpose of this investigation was to demonstrate the utilization of intensity modulated exoskeleton

Preventing muscle loss through early mobilization in non-critically ill older adults admitted to surgical intensive care units.

Intensive care unit-acquired weakness impairs recovery among older adults. We evaluated whether an early mobilization protocol reduces loss of muscle strength and mass compared with usual care in postoperative surgical intensive care unit (SICU) patients. In a before-and-after cohort at Siriraj Hospital, Thailand, 90 patients aged ≥ 65 years received usual care (June 2018-October 2019) and 90 subsequent patients followed a protocol-driven early-mobilization program (October 2019-November 2021). Daily assessments included muscle strength, Medical Research Council sum score (MRC-SS) and handgrip
