Blackout of my nights: Contentless, timeless and selfless report from the night in patients with central hypersomnias.

At the extreme spectrum of consciousness during sleep, some patients with rare hypersomnias reported experiencing a specific night 'blackout' when sleeping, i.e., an absence of experiences or recall of them from sleep onset to offset. Thus, we explored through questionnaires the conscious experiences (dreaming experience, mind, self) during the night in 133 patients with idiopathic hypersomnia, 108 patients with narcolepsy, and 128 healthy controls. The night blackout was more frequent in idiopathic hypersomnia than in narcolepsy and control groups. Patients with idiopathic hypersomnia and fre

From Continuity to Contiguity: A Response to the Fraught Temporality of Gender.

This paper examines current psychoanalytic engagements with the use of hormone blockers in transsexual children and the underlying premises concerning our understanding of the child's process of coming into his or her gendered self. Rather than taking sides in the debate, I explore how the "hormones question" becomes entangled in a series of misreadings and displacements through which the child's request could potentially be missed. In examining psychoanalytic conceptualizations of the trans child's agency, autonomy, and future and the relation between the natal body and gender, I ask, how is

Autobiographical memory and the self in a single-case of chronic unilateral spatial neglect.

Unilateral spatial neglect (USN) is mainly defined as a condition affecting perception and the mental representation of the environment. However, nothing is known about its impact on the ability to mentally represent one's past and on personal identity. We addressed these questions in a case of chronic USN, DR, a 59-year-old right-handed woman, who underwent a variety of measures exploring the self and autobiographical memory (AM). DR showed preserved self-images and her AM performance was only preserved when memories were prompted by her own self-images and not by self-unrelated cues. Our fin

Self, agency and the surgical collective: detachment.

In this article, I describe the socio-technical organisation of surgical rehabilitation. After having gone through surgical intervention, patients are implicated within various types of medical work aimed at adjusting their bodies to post-operative social and material environments. My argument is that the process of re-establishment of a 'self' is mediated through a re-disposition of agency in the socio-technical ensemble upon which the patient depends immediately after surgery. Drawing on one year of ethnographic fieldwork in a neurosurgical clinic, and theoretical resources from science and

Auditory verbal hallucinations--breaking the silence of inner dialogue.

Auditory verbal hallucinations (AVHs) are usually defined as perceptions of speech that occur in the absence of any appropriate external stimulus. This definition, we argue, is false. We maintain that AVHs are disorders of self-consciousness that are best understood as the becoming conscious of inner dialogue. Normally, subconscious interior conversations are experienced as a sense of partnership between distinct parts: we feel these parts as distinct, but also integrated and collaborating with each other in decision-making and in self-representation. AVHs attest to a breakdown in this process

Attachment and dysthymia: the contributions of preoccupied attachment and agency of self to depression in women.

The present research evaluated a conceptual model that links preoccupied attachment to dysthymic disorder in women. From an original community sample of 420 women, 129 women were identified with depressive symptomatology as assessed by the Centre for Epidemiological Studies Depression scale (CES-D). Twenty-four of these 129 women were diagnosed as dysthymic disordered according to the Structured Clinical Interview for DSM-III-R-non-patient edition (SCID-NP). Attachment patterns were assessed using the Adult Attachment Projective (AAP). The results indicate that preoccupied attachment was assoc

Neuropsychology and the self-concept.

The biological basis of the self is discussed in relation to neuropsychological findings in cases with focal brain injury. This material demonstrates that the self is deposited at an early phase in the process of object realization and distributes into the private space of imagery and the external space of object perception. A disruption or truncation of this process results in an erosion of the self that is similar across the different perceptual modalities. From the pattern of pathological breakdown, it is concluded that the self is a categorical, relational entity that achieves autonomy in …

Grief as a crisis: psychotherapeutic interventions with elderly bereaved.

This paper describes a psychotherapeutic intervention of grief in elderly subjects. The technique rapidly and efficiently facilitated grief work. The paper further illustrates the feasibility and usefulness of psychotherapy with the aged, contrary to earlier reports that the modality was not an effective treatment for geriatric patients.

Resolved and unresolved bioethical authenticity problems.

Respect for autonomy is a central moral principle in bioethics. It is sometimes argued that authenticity, i.e., being "real," "genuine," "true to oneself," or similar, is crucial to a person's autonomy. Patients sometimes make what appears to be inauthentic decisions, such as when (decision-competent) anorexia nervosa patients refuse treatment to avoid gaining weight, despite that the risk of harm is very high. If such decisions are inauthentic, and therefore non-autonomous, it may be the case they should be overridden for paternalist reasons. However, it is not clear what justifies the judgme

Deep Brain Stimulation, Authenticity and Value.

In this article, we engage in dialogue with Jonathan Pugh, Hannah Maslen, and Julian Savulescu about how to best interpret the potential impacts of deep brain stimulation on the self. We consider whether ordinary peoples' convictions about the true self should be interpreted in essentialist or existentialist ways. Like Pugh, Maslen, and Savulescu, we argue that it is useful to understand the notion of the true self as having both essentialist and existentialist components. We also consider two ideas from existentialist philosophy-Jean-Paul Sartre and Simone de Beauvoir's ideas about "bad faith

A self psychological and relational approach to group therapy for university students with bulimia.

Bulimia is a complex disorder that can significantly interfere with psychosocial functioning. It is a particularly significant problem among adolescents and young women in college and university settings. The preponderance of literature on group therapeutic approaches for this disorder is from a cognitive-behavioral perspective. In this paper, we present a psychodynamic group therapy approach for female university students, informed specifically by self psychological and relational frameworks. Through clinical illustrations, we demonstrate how the group provides selfobject experiences, includi

The computational self.

Your brain is never at rest. Shifting patterns of activity course through your brain at night as you review the events of the day and plan the next day before falling asleep. Rumination is a reflection of the Self that is not directly driven by sensory stimuli. When we record from single neurons in the brain, we discover that even in the absence of sensory stimulation, neurons are continuously active. This is called maintained, or spontaneous, activity, and although it is well documented, it has not been as well studied. Most experiments are designed to look for signals that are elicited by se

The place of annihilation anxieties in psychoanalytic theory.

Survival-related clinical reports are abundantly found in the works of classical, object-relational, and self psychological writers, but are underrepresented in major theoretical formulations on anxiety. Fears of being overwhelmed, merged, penetrated, fragmented, and destroyed, as contents of unconscious and conscious fantasies, are regularly interrelated with the typical dangers. Fifteen preliminary propositions invite closer study of such apprehensions and provide definitional components. Annihilation anxieties are triggered by survival threat; are found early but can be engendered throughou

The self: clues from the brain.

Can we find a way of thinking about the self that is compatible with modern neuroscience? I think we can. First of all, we have to recognize that "the self" is not the same as "the conscious self," since much of who we are as individuals takes place out of conscious awareness. Second, we have to accept that some aspects of the self, especially the unconscious aspects, occur in and can be studied in other species, allowing us to relate these aspects of the self to detailed brain mechanisms. Finally, it also helps to think of the self in terms of memory. Obviously, much of who we are is based on

Internal containment in the treatment of patients with dissociative disorders.

Both external and internal containment techniques are useful in treating patients with multiple personality disorder and other dissociative disorders. The author focuses on internal containment using various individualized strategies for helping these patients cope with life and work through past traumas. Because the visual imagery typically used in these strategies varies significantly from patient to patient, the author does not intend to provide an exhaustive description. The techniques are illustrated by case examples from the author's clinical practice.

Paranoid character and the intolerance of indifference.

This paper suggests that aspects of paranoid character are structured around fantasies of magical and concrete connectedness to objects; these fantasies serve to avoid the terrors of object inconstancy. The authors describe how these fantasies are expressed in the psychoanalytic situation and explore their relation to common paranoid phenomena. The paranoid person must maintain these fantasies of connectedness at all cost or risk experiencing unbearable indifference between self and object. Paradoxically, the sacrifice of self and object boundaries inherent in these fantasies makes object cons

The Differential Diagnostic Technique, a visual motor projective test.

The Differential Diagnostic Technique is a projective drawing test in which the individual is asked to copy 14 figures, presented one at a time. Research indicates the usefulness of the test as an objective measure of personality and points to its diagnostic use for certain neuropsychological problems and in research.
