Osseous manifestations of 'handball goalie's elbow'.

The radiographic manifestations of musculoskeletal stress at the elbow associated with European team handball are reported in 46 amateur handball players with 74 elbows injured by blocking shots. All the 92 elbows were evaluated by clinical and radiological examinations using the 18 asymptotic elbows as a control. Lateral and anterioposterior radiographs were obtained of both elbows in addition to stress radiographs with manual valgus force. The mean age of the athletes was 23.0 (range 16-44) years. The players had an athletic experience of 11.6 years (range 3.5-34). Their elbow pain had at in…

Translational injuries in posterior elbow dislocation.

We describe a new plain radiographic finding in posterior elbow dislocation: a defect in the nonarticular posterior surface of the capitellum caused by impaction of the radial head during dislocation. The defect was visible on a lateral view unobscured by overlying bony structures in the unreduced state and confirmed on both an axial view after reduction and by magnetic resonance imaging. We propose that this lesion is analogous to the Hill-Sachs/Bankart lesion complex seen in translational glenohumeral injury.

Above-elbow arm replantation during pregnancy.

We report a successful replantation of an above-elbow amputated left arm in a 26-year-old female with a good, functional, long-term result. The patient was at 18 weeks estimated gestational age at the time of surgery. Major upper extremity replantation during pregnancy has not been previously described in the literature. Current operative and perioperative aspects, including considerations that favored microsurgical replantation in this unusual case, are discussed.

Elbow arteriovenous fistulas for chronic haemodialysis.

Arteriovenous fistulas (AVFs) were created at the elbow, rather than using prosthetic grafts, when forearm vessels were inadequate. A total of 272 such fistulas were constructed: 49 between the brachial artery and a forearm vein, 91 between the brachial artery and the elbow perforating vein, 52 between the brachial artery and the cephalic vein and 80 between the brachial artery and the basilic vein. In each case the vein was sutured end to side to the artery. The immediate failure rate was 11.8 per cent. The 1- and 4-year cumulative patency rates were 68.7 and 51.2 per cent for brachial artery…

The modified Liverpool total elbow prosthesis.

We present the results of the modified Liverpool total elbow joint replacement. The humeral component of the original prosthesis was modified because of a high incidence of loosening. 19 patients who had total elbow joint replacement for rheumatoid arthritis were reviewed 8 years after surgery. 14 patients had the modified prosthesis implanted. There was dramatic relief of pain and improvement in function following surgery. There was a reduced fixed flexion deformity and an increased range of movement with the modified prosthesis. None of the humeral components of the modified prosthesis have …

Dynamic electromyography to assess elbow spasticity.

Control of elbow motion was evaluated in 45 extremities of adults with spasticity resulting from traumatic brain injury with use of dynamic electromyography. Simultaneous recording of elbow motion was obtained using a double parallelogram goniometer. Thirty-four male and 9 female patients were studied. Mean elbow flexion was 85 degrees and mean extension was 20 degrees. The average time of elbow flexion was 1.8 seconds. Extension time was prolonged to a mean of 4.0 seconds. Dynamic electromyography revealed a consistent pattern of muscle activity. Severe spasticity was noted in the brachioradi…

[Conservative treatment of pediatric elbow fractures].

Between 1978 and 1982, 146 children with elbow fractures were treated at the Department of Traumatology of the Freiburg University Hospital. Forty-two children received conservative treatment and in 104 an operation was performed. The possibilities for conservative treatment of the elbow joint in children are discussed together with our results. Out of 47 supracondylar fractures of the humerus, 25 were treated conservatively (28%, dislocation grade 1; 48%, grade 2). Good or very good results were achieved in more than 76%. Unstable grade 3 fractures often involve rotation of the elbow; in 35% …

Elbow injury: a new imaging approach.

The authors describe a new technique for evaluating traumatic conditions to the elbow: the radial head-capitellum view. This projection has proved useful in demonstrating minimally displaced or nondisplaced fractures of the radial head, capitellum, and coronoid process. The radial head-capitellum view not only delineates the fracture, it also defines the extent of displacement.

Tennis elbow--a radial tunnel syndrome?

The hypothesis that the chronic tennis-elbow syndrome is caused by an entrapment neuropathy of the radial nerve (Roles and Maudsley 1972) was put to the test prospectively in ten patients. Detailed orthopaedic, neurological and neurophysiological examinations did not reveal any involvement of the radial nerve. It is concluded that this syndrome cannot be explained by an entrapment neuropathy.

Safe Ultrasonography-Assisted Elbow Arthroscopy Technique.

Elbow synovectomy has traditionally been performed using direct visual observation. However, recent advancements in elbow arthroscopy have increased the popularity of arthroscopic synovectomies. This minimally invasive procedure has garnered attention because of its efficacy in halting joint deterioration while minimizing patient discomfort and invasiveness. Conversely, the technical demands of arthroscopic elbow synovectomy pose challenges, often resulting in notable complications, such as nerve damage. Considering the utility of real-time nerve visualization for injury prevention, we suggest…

Radiographic assessment of medial elbow stability.

BACKGROUND: Diagnosing simple valgus instability of the elbow currently involves time- and cost-intensive imaging modalities such as magnetic resonance imaging or magnetic resonance arthrography. Previous studies have demonstrated that stress radiography represents an alternative diagnostic tool for such conditions. The aim of this study was to investigate whether standardized valgus stress radiography can identify soft-tissue lesions of the medial elbow. METHODS: A telos stress device (telos GAIII/E; telos Arzt- und Krankenhausbedarf GmbH, Woelfersheim-Bernstadt, Germany) was used to apply 5…

Coronoid fractures and traumatic elbow instability.

The coronoid process is key to concentric elbow alignment. Malalignment can contribute to post-traumatic osteoarthritis. The aim of treatment is to keep the joint aligned while the collateral ligaments and fractures heal. The injury pattern is apparent in the shape and size of the coronoid fracture fragments: (1) coronoid tip fractures associated with terrible triad (TT) injuries; (2) anteromedial facet fractures with posteromedial varus rotational type injuries; and (3) large coronoid base fractures with anterior (trans-) or posterior olecranon fracture dislocations. Each injury pattern is as…

Elbow stiffness: Arthritis and heterotopic ossification.

In elbow stiffness, pre-operative assessments should identify the articular and peri-articular tissues involved and, more specifically, they should determine how preserved the articular surfaces and osteo-articular congruity are. We will focus on the most important conditions and tissue reactions after trauma in order to understand the causes of joint stiffness. A logical surgical planning is based upon a deep knowledge of the anatomical obstacles and of the associated lesions that the trauma provoked with. The peri-articular soft tissue contractures. The osteo-articular incongruity.
