The current role of elbow hemiarthroplasty compared to total arthroplasty in the treatment of distal humerus fractures in the elderly. Twice the trouble or half the hassle?

BACKGROUND: Unreconstructable distal humerus fractures in elderly patients are commonly treated with total elbow arthroplasty (TEA). However, TEA has the major disadvantage of lifelong weight-bearing restrictions. Elbow hemiarthroplasty (EHA) is an alternative that allows full weight-bearing, but it is less commonly performed, and there's a lack of high-quality original research directly comparing it to TEA. Therefore, a systematic review and meta-analysis of all available research were conducted to better understand the differences. METHODS: A systematic review and meta-analysis were conduct

Avulsion fracture of the supinator crest as an indication for a sustained posterolateral (sub)luxation of the elbow. A case report, anatomical evaluation and review of the literature.

The treatment of elbow injuries can be challenging because of the complexity of both anatomy and pathology. We present a rare traumatic avulsion fracture of the supinator crest of the ulna in a 37-year-old patient. Conservative treatment in a long arm cast for four weeks led to satisfactory results. Reproduction of the fracture on a cadaveric elbow clarified that the avulsed fragment holds the insertion of the lateral ulnar collateral ligament (LUCL). The mechanism of trauma that causes this fracture is a posterolateral (sub) luxation of the elbow, which usually causes the LUCL to rupture, but

Divergent elbow dislocation with radial shaft fracture, distal ulnar deformation, and distal radioulnar joint instability: an unclassifiable Monteggia variant.

Originally described by Monteggia and later classified by Bado, elbow dislocations with concurrent radial and ulnar shaft fractures with distal radioulnar joint (DRUJ) disruption are considered operative cases with high-energy injurious etiologies. Here, we present an unclassifiable Monteggia variant fracture suffered through a high axial load mechanism in a 47-year-old female. The fracture pattern initially exhibited included a divergent elbow dislocation, a radial shaft fracture, plastic deformation of the distal ulna, and DRUJ instability. Here we describe the pattern in detail, along with

The influence of type II coronoid fractures, collateral ligament injuries, and surgical repair on the kinematics and stability of the elbow: an in vitro biomechanical study.

PURPOSE: This study determined whether elbow stability could be restored with open reduction and internal fixation (ORIF) of type II coronoid fractures and evaluated the role of collateral ligament repair. METHODS: Passive varus and valgus and simulated active vertical motion were performed using an in vitro elbow motion simulator. Varus/valgus angle and internal/external rotation were measured with the coronoid intact, with 50% removed, and after ORIF. Testing was performed with the collateral ligaments detached and repaired. RESULTS: Vertical: stability was normal when both the lateral col

Open Reduction and Internal Fixation Versus Total Elbow Arthroplasty for Intra-articular Distal Humerus Fractures in Patients 65 Years or Older: A Meta-analysis.

BACKGROUND: Distal humerus fractures (DHF) in the elderly pose a challenge due to osteoporotic bone, comminution, and potential for poor functional recovery. Surgical management typically involves open reduction and internal fixation (ORIF) or total elbow arthroplasty (TEA). However, inconsistent findings across studies have led to uncertainty regarding which intervention optimizes outcomes, underscoring the need for a rigorous meta-analysis to guide clinical decision making. METHODS: A systematic literature search was conducted across PubMed, Scopus, Cochrane Library, and Google Scholar for

Using 3D Printing Technology to Design Split-Piece Sleeve Prosthesis in the Revision Surgery of Tumor-Type Total Elbow Prosthetic Fractures: A Case Report.

BACKGROUND: Revision of tumor-type prosthetic fractures is very challenging in clinical work. Traditional repair methods may not be able to meet the needs of complex cases or cause greater bone damage. Therefore, more effective and reliable solutions need to be found. CASE PRESENTATION: This study presents a novel revision technique for managing fractures of tumor-type total elbow prostheses. A 57-year-old female patient was diagnosed with a left distal humeral bone tumor accompanied by pathological fracture and underwent customized tumor-type total elbow prosthesis arthroplasty. After 5 year

The use of the anterolateral thigh vascular free flap in complex open elbow fractures after major trauma - An illustrated report of an interesting case.

Major trauma may result in severe open elbow fractures with significant soft tissue injury and skin loss. Reconstruction of those defects can be complicated and inadequate cover can result in severely limited functional outcome. The free anterolateral thigh flap (ALT) is one of the ways to reconstruct those defects. Its utilisation in severe complex open elbow fractures is recently being increased due to its advantages. The purpose of this article is to present an interesting case where the ALT flap was used with success in a challenging situation of a severe elbow bony, ligamentous and soft t…

Delayed surgery leads to reduced elbow range of motion in children with supracondylar humeral fractures managed at a referral hospital in sub-Saharan Africa.

BACKGROUND: Supracondylar humeral fractures (SHFs) in children are associated with morbidity due to elbow stiffness. Timely operative management and/or physiotherapy are thought to reduce this complication, but pose challenges in settings with limited resources for health. METHODS: This prospective cohort study included 45 pediatric patients with isolated SHF at a large tertiary hospital in Nairobi, Kenya. Patients were managed non-operatively or operatively with varying wait times to surgery, with or without physiotherapy. The measurement of elbow ROM was done up to 12 weeks after removal of

A systematic review, and meta-analysis of pain outcomes following total elbow replacement and hemi-replacement for unreconstructible acute distal humerus fractures in adults.

BACKGROUND: Unreconstructible distal humerus fractures have been managed with total elbow replacement (TER), but there is increasing use of hemi-replacement (DHH). Pain has been identified as the most important outcome by patients. This study aims to systematically review the literature on reported pain outcomes in adult distal humerus fractures treated with TER or DHH. METHODS: Medline, Embase and Central were searched using OVID, from January 2000 to September 2023, for studies in adults undergoing DHH or TER for acute closed distal humerus fractures. The primary outcome was patient-rated p

The Bryan-Morrey triceps-sparing approach to open reduction of T-condylar humeral fractures in adolescents: cybex evaluation of triceps function and elbow motion.

Nine patients who underwent open reduction of a T-condylar distal humerus fracture through a Bryan-Morrey triceps-sparing approach were evaluated for triceps function and elbow motion. The average follow-up was 3 years 5 months. The average range of motion was -8 degrees to 136 degrees. Measured by Cybex testing, the average triceps deficit compared with the uninvolved arm was 6% at 60 degrees/sec, 7% at 120 degrees/sec, and 10% at 180 degrees/sec. Compared with another study that tested triceps function after open reduction with the Campbell triceps-splitting approach, no statistically signif

Visualization of Median Nerve Entrapment After Reduction of an Elbow Fracture Dislocation Using 3-Dimensional Magnetic Resonance Imaging: A Case Report.

CASE: A 10-year-old girl presented after closed reduction of an elbow fracture dislocation. She demonstrated intact vascularity but a dense median nerve palsy. Preoperative magnetic resonance neurography (MRN) precisely mapped the median nerve entrapped within the medial epicondylar fracture. Intraoperatively, the median nerve was freed preceding reduction and fracture fixation. Postoperatively, neurological symptoms completely resolved, and she regained full elbow function. CONCLUSION: Median nerve injury can present without associated vascular injury. In this case, MRN was helpful in preope

Management of a Rare Complex Triad Injury of the Elbow Joint: Radial Head Fracture, Triceps Tendon Avulsion, and Rupture of the Medial Collateral Ligament.

BACKGROUND: Isolated triceps tendon injuries are infrequent, and their combination with a medial ligament avulsion is even rarer. Eccentric force on the triceps tendon following a fall on the outstretched hand, combined with a valgus torque at the elbow joint, exposes the medial side to stretching forces, potentially disrupting the ulnar collateral ligament (UCL). Furthermore, compressive forces on the lateral side could result in injury to the radial head. Currently, there is no standardized surgical approach for managing such combined injuries. INDICATIONS: The complex triad injury presente

Comprehensive analysis of the effect of structural parameters on erosion wear, structural stress, and deformation of high-pressure double-elbow in shale-gas fracturing.

Источник пометил публикацию как отозванную

In field hydraulic fracturing operation of shale gas development, the high pressure and large displacement liquid-particle two-phase fracturing fluid can be forced to change direction many times through high-pressure double-elbow, and be transported from the outlet pipeline of the fracturing pump to the main pipeline. The high-pressure double-elbow is prone to be affected by erosion wear and Fluid-Structure Interaction (FSI), resulting in perforation and fracture, posing a potential safety threat to field operation. In this study, we conducted the erosion wear experiments on 35CrMo steel used
