Relationship Between Atopic Dermatitis and Food Allergy in Children.

Atopic dermatitis (AD) is a chronic inflammatory skin condition characterized by a compromised skin barrier due to a variety of reasons, suc … onfirm allergies before recommending a stringent diet that could be hazardous to the patient. While elimination diets continue to be the cornerstone of the management of FA, they should only be carried out under the guidance of a specialist. Topical treatments are crucial for all individuals with AD. Early skin care with emollients, topical steroid treatment, and e …

Eosinophilic gastrointestinal disorders in children and adolescents: A single-center experience.

BACKGROUND: To date, few studies have been conducted in Italy on pediatric eosinophilic gastrointestinal diseases (EGIDs). AIMS: To assess … mp inhibitors (PPIs) alone or in combination with swallowed steroids. PPIs, oral steroids, and food-elimination diets were prescribed to patients with non-esophageal EGIDs. CONCLUSION: This is the first Italian study revealing an increased frequency of EGIDs in a pediatric population. Further studies are needed to characterize patients with these emerging diseases …

Oesophagitis in children: reflux or allergy?

The prevalence of eosinophilic oesophagitis appears to be increasing in many countries, sometimes rapidly, although this may be partly due t … itis typically fail to respond to acid suppressive medications but respond well to either elemental/elimination diets or aerosolised swallowed corticosteroids. Long-term uncontrolled oesophageal eosinophilic inflammation may lead to progressive subepithelial fibrosis, potentially resulting in strictures or oesophageal narrowing.

Eosinophilic esophagitis.

PURPOSE OF REVIEW: Eosinophilic esophagitis is a new disease commonly confused with gastroesophageal reflux disease. Because of different treatments, recognition of eosinophilic esophagitis is important. RECENT FINDINGS: Recent work further characterizes the allergic response, demonstrating a population of eosinophils different from gut and blood that specifically homes to the esophagus in eosinophilic esophagitis. Further data suggest a multi-step allergy process starting with an atopic skin response that later primes the esophagus on further allergen exposure. Diagnosis of eosinophilic esop

Synapse-to-neuron ratio in the lateral geniculate nucleus of rats exposed chronically to ethanol.

Effects of chronic ethanol exposure on synapse-to-neuron ratio in the rat lateral geniculate nucleus were investigated. Male Sprague-Dawley rats were exposed to ethanol, using the Lieber-DeCarli liquid diets, for 4 months starting at the age of 5 weeks. Brains were perfusion-fixed, and the region containing the dorsal lateral geniculate nucleus was cut into slabs (500 microns thick) that were epoxy resin-embedded. From each rat, three slabs containing the structure were serially sectioned for electron microscopy. Using the double disector method, the study shows an unaltered synapse-to-neuron

[Secondary carbohydrate and protein intolerances following gastroenteritis].

After acute gastroenteritis, delayed recovery and protracted diarrhoea may occur, particularly in very young infants, in bottle-fed malnourished infants, and after rota virus infection. Monosaccharide and disaccharide malabsorption have been demonstrated to contribute to postenteritis problems in these children. The contribution of secondary food protein intolerance to the perpetuation of diarrhoea after gastroenteritis is less well understood. Secondary sugar intolerance is diagnosed by estimation of stool pH, Clinitest, H2 breath testing and, in some cases, direct enzyme determination from b

The algorithmic epistemic inversion in gastroenterology: how social media and psychological vulnerability fuel secondary risks in IBD and IBS management.

The unchecked proliferation of medical misinformation on short-video platforms has crystallized into a critical digital public health crisis. For patients managing chronic gastrointestinal diseases such as IBD and IBS, recommendation algorithms routinely override evidence-based medicine (EBM), engendering a phenomenon we term "algorithmic epistemic inversion." This inversion amplifies unproven "miracle cures" and fear-based narratives, driving population-level secondary risks, ranging from indiscriminate probiotic abuse and extreme elimination diets, malnutrition, and orthorexia nervosa. Altho…

Food allergy is a common clinical problem in Saudi Arabia with multiple factors contributing to it including the changes in lifestyle, espec … nd eggs, but regional foods such as sesame and seafood are also responsible for allergic reactions. Elimination diets in children with food allergy may increase the risk of nutritional deficiencies. Improvement of Gastrointestinal, Metabolic, and Thyroid Biomarkers through Advanced Diagnostic Techniques and Functional Nutrition Approach in 40-Year-Old Female: A Case Report.

BACKGROUND: Functional nutrition is a holistic approach that uses advanced diagnostics to treat diseases based on a root cause analysis, followed by personalised dietary interventions, supplements, physical exercise, stress management, and environmental modifications. OBJECTIVES: This case report discusses the functional nutrition approach adopted to manage gastrointestinal and thyroid dysfunctions in a 40-year-old female. The approach included root cause analysis and advanced diagnostic tests such as gastrointestinal microbial array tests (GI-MAP) and pathogen-associated immune reactivity sc

Activation of PPARγ by endogenous prostaglandin J2 mediates the antileukemic effect of selenium in murine leukemia.

Supplementation with nontoxic doses of micronutrient selenium has been shown to alleviate chronic myelogenous leukemia (CML) via the elimination of leukemia stem cells (LSCs) in mice. This treatment provides a new and novel method for eliminating the LSCs that are otherwise not targeted by existing therapies. The antileukemic effect of selenium was dependent on the production of endogenous cyclopentenone prostaglandins (CyPGs), Δ-12 prostaglandin J2 (Δ12-PGJ2), and 15-deoxy-Δ12,14-prostaglandin J2 (15d-PGJ2). Here, we show that these endogenous CyPGs, produced by mice maintained on selenium-su

Role of food hypersensitivity in irritable bowel syndrome.

A significant proportion of IBS patients attribute their symptoms to adverse food reactions. Dietary elimination and re-challenge studies support the role of diet in the pathogenesis of IBS. The aetiopathogenesis of IBS is thought to be multifactorial involving an interaction between diet, infection, antibiotics and psychosocial factors. Serum IgE and IgG4 antibodies are elevated in food hypersensitivity induced atopic conditions and a similar mechanism has been postulated in IBS. Increased number of mast cells is present in the ileocaecal region of IBS patients. Once sensitized, they are capa

Nutrition versus toxicology of manganese in humans: evaluation of potential biomarkers.

Manganese intake can vary greatly with food choices, water composition, and supplement use. Thus, individuals consuming Western diets consume from < 1 to > 10 mg Mn/d. The levels of manganese intake associated with adverse effects (both deficient and toxic) are debatable. Moreover, many of the symptoms of manganese deficiency (growth retardation, changes in circulating HDL cholesterol and glucose levels, reproductive failure) and manganese toxicity (growth depression, anemia) are non-specific. The bone deformities observed in manganese-deficient animals and neurological symptoms of individuals

Are adverse food reactions linked to irritable bowel syndrome?

OBJECTIVE: We undertook to determine whether adverse food reactions play a role in irritable bowel syndrome (IBS). METHODS: A systematic review of the literature using Medline (1980-1996), targeting IBS and adverse food reactions, was performed. All clinical trials whereby dietary exclusion was followed by food challenge were selected. Each study was reviewed using a structured format to examine methodological issues and study outcomes. RESULTS: Of the seven studies included, the positive response to an elimination diet ranged from 15% to 71%; double-blind placebo-controlled challenges ident

Diuretic therapy in congestive heart failure for the elderly patient.

The incidence of congestive heart failure (CHF) in the elderly increases with age as 80% of patients hospitalised with CHF are older than 60 years of age. In this age group CHF may result from several factors such as coronary artery disease, hypertension, valvular disease or intrinsic myocardiopathies. Important alterations of renal physiology have been observed in this condition: decreases in renal plasma flow and glomerular filtration rate, and increases in renal venous pressure. Natriuretic hormone inhibition occurs and renin-angiotensin-aldosterone system activation and antidiuretic hormon

Nutritional Status and Metabolism in Celiac Disease: Narrative Review.

This review summarizes findings from studies assessing the nutritional status of patients with celiac disease (CD). Malnutrition, including over- and undernutrition, may be present in CD, both at diagnosis and while under treatment. Underweight and growth retardation in children, which mostly reflect malabsorption as a consequence of intestinal inflammation, are not a rule. Clinical presentations of CD can vary widely, and each manifestation has its own characteristics. Evaluating various nutritional parameters can be beneficial for CD patients and may improve health outcomes by facilitating a

Rickets and gross motor delay in a child with atopic dermatitis.

We report a case of a 14-month-old boy with atopic dermatitis (AD) who presented to our hospital with hypocalcemic tetany and gross motor delay. Further laboratory and imaging confirmed the diagnosis of vitamin D deficiency and rickets. He was breastfeeding and on a restricted diet due to presumed multiple food allergies. He received calcium and vitamin D supplementation which corrected his hypocalcemia. The patient developed Staphylococcus aureus bacteremia and superficial septic thrombophlebitis for which he was treated with antibiotics and anticoagulation. An elimination diet should be avoi

Treatment of end-stage pharyngeal strictures after laryngectomy with fasciocutaneous microvascular reconstruction.

Pharyngeal strictures can develop after laryngectomy for larynx cancer, particularly in combination with radiation. Some patients develop intractable strictures where conservative swallowing therapy and pharyngeal dilations fail. Secondary reconstruction of these patients has not been described in the literature. We report our experience with completion pharyngectomy and fasciocutaneous free tissue reconstruction for this patient population. Five subjects who had undergone laryngectomy and radiation were successfully reconstructed with fasciocutaneous free tissue transfer using the radial fore
