Recognizing and responding to hyperglycaemic emergencies.

Diabetic ketoacidosis (DKA) and hyperglycaemic hyperosmolar state (HHS) are both diabetic emergencies associated with hyperglycaemia and can be fatal if healthcare professionals fail to recognize and intervene appropriately. While many students and qualified nurses may be able to recall common signs and symptoms related to DKA and HHS - for example polyuria, polydipsia and elevated blood sugars - understanding the physiological mechanisms behind abnormal observations and restoring homeostasis through appropriate management is far more complex. Health educators can play a significant role in co…

Psychiatric emergencies in the geriatric population.

Failure to recognize psychiatric conditions in any age group particularly in the elderly could lead to a very complicated clinical picture and may lead to increased health expenditures. Psychiatric emergencies can be secondary to numerous factors: geriatric depression and suicide, behavioral disturbance secondary to underlying organic conditions, substance abuse, elder abuse, and medication-induced adverse events. This article discusses mainly geriatric depression, elder abuse, and conditions emanating from iatrogenic causes.

[Emergencies in thyroid surgery. Our experience].

The rate of complications after thyroid surgery is about 5% and among these real emergencies account for less than 1%, consisting in intraoperative and postoperative bleeding, bilateral recurrent palsy, severe hypoparathyroidism and, rarely, laryngeal oedema and tracheomalacia. Between 2000 and 2004 849 patients were submitted to total thyroidectomy for various thyroid pathologies in our institution. Complications observed were postoperative bleeding in 13 patients (1.5%), laryngeal recurrent nerve palsy in 18 (monolateral in 14 - 8 transient and 6 permanent - and bilateral in 4, 3 of which tr…

[Cardiac emergencies in infancy and childhood].

There are relatively few situations where cardiac emergencies can occur in a child. A special focus is directed to the cyanotic newborn. In these cases a very specific treatment plan is necessary which is limited to a few dedicated centres. The importance lies in the timely diagnosis of a cyanotic heart defect and the rapid and safe transport from the referring hospital. An outline is given on the causes and treatment of congestive heart failure in infancy. Management guidelines are given for the most frequent types of cardiac arrhythmias and the few occasions, where an arrhythmia can lead to …

Neuro-ophthalmic emergencies for the neurologist.

A variety of acute neurologic disorders present with visual signs and symptoms. In this review the authors focus on those disorders in which the clinical outcome is dependent on timely and accurate diagnosis. The first section deals with acute visual loss, specifically optic neuritis, ischemic optic neuropathy (ION), retinal artery occlusion, and homonymous hemianopia. The authors include a discussion of those clinical features that are helpful in distinguishing between inflammatory and ischemic optic nerve disease and between arteritic and nonarteritic ION. The second section concerns disc ed

Colorectal cancer presenting as surgical emergencies.

Colorectal Cancer (CRC) is a major cause of cancer morbidity and mortality. Surgery is considered the first line therapy for CRC, and is generally encountered by many surgeons under elective conditions. Unfortunately, colorectal cancer may present acutely as a surgical emergency. These conditions include, but are not limited to perforation, hemorrhage, and obstruction. The presentation of a patient with these conditions can lead to higher morbidity and mortality. The choice of operation depends mainly on the site of the disease (left-sided versus right-sided), the patient's physical condition,

The vasculopathic patient: uncommon surgical emergencies.

Recognition of the patient who has an emergent vascular disorder and the early initiation of simple management steps in the ED can significantly impact patient outcome. The vasculopathic patient presents with significant comorbid conditions and a small window of opportunity to alter the prognosis favorably. The critical role of the EP lies in prompt use of this opportunity and appropriate direction of further care.

Abdominal emergencies: diagnostic and therapeutic laparoscopy.

BACKGROUND: Laparoscopy has been practiced in the management of emergencies resulting from inflammatory conditions, lumenal obstruction, perforation, vascular occlusion, and trauma. This article identifies and discusses controversial areas in the field, in particular surrounding the efficacy, cost effectiveness, and perceived advantages of laparoscopy in the evaluation and treatment of patients with acute abdominal conditions. MATERIALS AND METHODS: Review and commentary on pertinent articles in the English language literature are presented. RESULTS: Prospective randomized trials have been r…

Neonatal surgical emergencies: stabilization and management.

The transitional period following birth can be complicated by the presence of congenital anomalies requiring emergent surgical management. Newborns with congenital diaphragmatic hernia require immediate intubation and gastric decompression to minimize gastric distention, as well as cautious ventilation to avoid pneumothorax. Newborns with omphalocele and gastroschisis are at risk for bacterial contamination, as well as heat and evaporative losses from exposed viscera. These newborns benefit from the use of a protective bowel bag. Newborns with meningomyelocele require meticulous care to avoid

General pediatric emergencies. Malignant hyperthermia syndrome.

Anesthesiologists always undertake the challenge of keeping themselves informed about advances in medical knowledge aggressively; however, they have not been particularly active in the areas of patient education and advocacy. Malignant hyperthermia syndrome is a unique condition that anesthesiologists as clinicians understand well--in most cases, better than other specialists. They are positioned best to inform patients initially about the significance of malignant hyperthermia syndrome susceptibility and to provide an ongoing resource for counseling. The physician-patient relationship, accord

Uterine emergencies. Atony, inversion, and rupture.

Uterine atony, inversion, and rupture are potentially fatal events that may occur in pregnancy. They are obstetric emergencies that require immediate attention. Although all women may experience these complications, identification or known risk factors allow the obstetric team to prepare for rapid diagnosis and intervention. This article includes management options to help prepare for these uncommon events.

Selected pediatric emergencies in community practice.

In children, the signs and symptoms of serious infection often mimic those observed with minor, self-limiting diseases. One of the most important steps in making a diagnosis of an infection of the central nervous system is to suspect that an infection may be present. Acute epiglotitis predominantly affects children 2 to 7 years of age; delays in its diagnosis or treatment may result in death within a matter of hours. Pharmacists should be aware of the signs and symptoms of child abuse and neglect and understand the reporting procedures and requirements.

[Emergencies in human immunodeficiency virus infection].

Emergencies during HIV infection consist mainly in opportunistic infections. They are varied and the physician should keep an "internist's eye" on the condition; they also require emergency complementary examinations for confirmation of diagnosis. If such diagnostic tests cannot be rapidly made, treatment should be started on the basis of presumption and frequency. In France, the most frequently observed opportunistic infections are: pneumocystosis, cerebral toxoplasmosis and candidiasis. Tuberculosis is a major concern of those responsible for public health care. Surgical complications follow…

[Diagnosis and treatment of respiratory emergencies].

The following six points should be kept in mind for the management of respiratory emergencies in the emergency room. 1 Gram's stain of a sputum smear may reduce the use of broad-spectrum antibiotics. 2 Routine use of a portable pulse oxymeter substantially reduce the number of arterial blood gas tests. 3 In critical illnesses, such as tension pneumothorax, diagnosis should be made and treatment should be begun without a chest X-ray film. 4 Bag-valve-mask systems should be used with a reservoir bag or a gas pocket to increase the concentration of oxygen. 5 To help prevent hypoxemia during endot…

[Neurologic emergencies in acute alcoholic intoxication].

Neurologic complications of acute alcohol intake are less well known to the general practitioner than those punctuating the course of chronic alcohol dependency. The state of ebriety is a poor incentive to precise neurologic analysis and follow-up. Nevertheless, only such surveillance in acute intoxication can distinguish a specific neurological complication, a complication following a fall or prolonged immobilisation, or rapid decompensation of alcoholic encephalopathy. Among the specific complications, myopathies and strokes are described here because of their rarity compared to the more com
