INTRODUCTION: The coronary cusps (CCs) are utilized as an alternative vantage point for radiofrequency catheter ablation (RFCA) of left ventricular summit ventricular arrhythmias but are sometimes a challenge despite favorable activation timing and pace mapping.
METHODS: Ex vivo experiments were performed in 12 intact porcine hearts submerged in a 37°C saline bath. Radiofrequency (RF) applications were delivered with an irrigated contact force sensing catheter oriented 45° to the endocardial left ventricular outflow tract (LVOT) surface and nadir of the CCs using different dosing parameters.

BACKGROUND: Endomyocardial biopsy is the gold standard and has a definite role in the surveillance of cardiac allograft rejection. Its role in other cardiac diseases is limited. However, it is required for conclusive diagnosis of a few entities in which it can influence patient management. There is no reported data regarding the utility of endomyocardial biopsy in the Indian population. Thus, this study was undertaken in a tertiary care center in India to assess the utility of endomyocardial biopsy in various cardiac diseases in the context of clinical diagnoses.
METHODS: All endomyocardial b

An unusual case of endomyocardial fibrosis is reported complicating an idiopathic hypereosinophilic syndrome. Persisting hypereosinophilia, degranulated eosinophils in the blood, and myocardial activity have been found accompanying the fibrotic phase of endomyocardial disease. This occurrence supports the unitarian theory on tropical and temperate endomyocardial disease and suggests in such a condition the use of steroids or cytotoxic drugs in addition to surgery.

Transient entrainment was demonstrated during 59 pacing events in 18 episodes of sustained uniform ventricular tachycardia (VT) while recording electrograms from the site of origin of tachycardia (LE-SOO). During entrainment, the morphology of the initial component of the LE-SOO was identical to the morphology observed during the tachycardia in 13 VTs (group I), but in five VTs (group II), the initial component changed at a "critical" paced cycle length. The presence of the proposed surface electrocardiographic criteria for entrainment--fixed fusion and a first postpacing complex without fusio

The fibrous skeleton of the human heart is composed of several parts which are formed from different, mainly extracardiac sources. The atrioventricular valve rings are formed by invagination of sulcus tissue at the atrioventricular and bulboventricular transitions. The atrial part of the central fibrous body is formed from an ingrowth of tissue from the dorsal mesocardium, the ventricular part from ingrowth of sulcus tissue towards an endocardial structure, the left bulbar ridge. During valve development the atrioventricular endocardial cushions, initially situated between the developing atria

The problem whether Löffler's endocarditis and endomyocardial fibrosis belong to the same disease spectrum or are separate entities is still under debate. Until recently it was believed that endomyocardial fibrosis was a disorder restricted to tropical areas. Three Danish patients are presented, two showing a continuous disease spectrum, one patient showing the fully developed endomyocardial fibrosis which is indistinguishable from endomyocardial fibrosis described from the tropical areas. Eosinophilia was present in all three patients. The findings described in these three patients lend suppo

The authors report a patient with a rare manifestation of invasive septic Erysipelothrix rhusiopathiae infection without endocardial involvement. Our patient presented with progressive paraparesis and subacute fever for ten days. He had underlying diabetes mellitus and alcoholic cirrhosis. Magnetic resonance imaging (MRI) of the lumbosacral spine showed a psoas abscess with vertebral osteomyelitis and discitis at level of L23 of the lumbar spine. His blood culture grew E. rhusiopathiae. Transthoracic echocardiography demonstrated normal endocardium. Surgical drainage and debridement with conco…

Cardiac cachexia remains a phenomenon seen more particularly in New York Heart Association Classification class III or IV heart failure patients. This is frequently missed and written off as "old age." It has a mortality rate of 50% within 18 months of diagnosis, so it is imperative to diagnose cardiac cachexia promptly. It remains elusive in the complexity of its underlying pathophysiology, from cytokine release from both the endocardium and the gut, with systemic implications. This review provides an analysis of cardiac cachexia: (1) definition and diagnostic criteria; (2) prevalence and mor…
