Multiple endocrine neoplasia type 1 (MEN1) is a rare but misleading disease. The diagnosis is evocated when two main lesions are present (parathyroid, endocrine pancreas, pituary gland) but also when a family tree shows recurrent lesions. Other lesions must be taken into account (adrenal glands, neuroendocrine thymic or bronchic lesions, cutaneous lesions, lipomas, nervous central system tumors). In an autopsy study of 503 cases the parathyroid glands were dissected, and the number of glands in each case and the anatomic distribution of the glands were recorded. In 18 cases (3%) only three glands were found. In these cases the lower combined weight suggested that a fourth gland had been missed.
