Meningitis and epiglottitis: a new immunisation against haemophilus influenzae type b infections.

Since 1985 the number of cases of systemic haemophilus influenzae type b (Hib) disease in children in the Oxford region has been recorded. This study shows that a cumulative incidence of children suffering systematic Hib infections before their fifth birthday is 1:600, 71 per cent of sufferers having meningitis. Because the incidence of haemophilus infections may be increasing in the UK and there is increasing resistance of the organism to antimicrobial therapy, the need for primary prevention through immunisation is becoming increasingly attractive. We have therefore undertaken a safety and i

Acute epiglottitis: decade of change--a 10-year experience with 242 children.

Acute epiglottitis is a life threatening infection of the supraglottic structures. It more commonly occurs in early childhood but affects all age groups. Haemophilus influenzae type B is the usual pathogenic organism. The disease may progress so rapidly that sudden upper airway obstruction will occur, resulting in hypoxic brain damage or death. A retrospective study was completed on all patients (242) admitted to the Hospital for Sick Children in Toronto with a diagnosis of acute epiglottitis from January 1976 to December 1985. Prior to 1976, tracheotomy was the more common form of airway cont…

Severe epiglottitis caused by Pasteurella multocida: A case report and the literature review.

Pasteurella multocida is a small, facultatively anaerobic coccobacillus commonly found in the upper respiratory tract of many mammals, birds, and especially felines. It can cause a wide range of infections in humans, often resulting from bites or scratches from pet animals. The literature has previously reported 15 cases of epiglottitis caused by Pasteurella multocida. Here we describe a case of severe epiglottitis complicated with neck area cellulitis in a previously healthy middle-aged woman. Blood cultures were positive for Pasteurella multocida, likely originating from a scratch by the pat…

Acute Epiglottitis with Concurrent Pneumonia and Septic Shock in an Alcoholic Adult Patient.

Historically epiglottitis has been considered a childhood disease. However, the introduction of the Haemophilus influenzae type B vaccine has decreased the incidence of epiglottitis in children. It is important to recognize modern epiglottitis as a disease of adults. This report describes a case of acute bacterial epiglottitis in an adult patient secondary to infection caused by Streptococcus pyogenes, a group A streptococcal infection. This case demonstrates the importance of early recognition of epiglottitis in adults, as they can experience rapid clinical decline. The progression of this di…

[Acute epiglottitis in adults. Diagnosis and treatment in our experience of 30 cases].

OBJECTIVE: We present the 15-year-long experience of 2 hospitals in our region regarding the therapeutic management of acute epiglottitis in adults. PATIENTS AND METHOD: Thirty patients with an average age of 46 years were diagnosed as having acute epiglottitis, either by indirect laryngoscopy or fibroscopy, and studied through a series of clinical parameters: age, sex, personal history, complementary tests, clinical symptoms, treatment, evolution, and average stay in hospital. RESULTS: We found an obvious predominance of this urgent pathology in males, with most patients reporting dysphagia…

Identifying acute epiglottitis in adults. High degree of awareness, close monitoring are key.

Epiglottitis can be a rapidly fatal condition in adults. Important clues that should raise clinical suspicion include the tripod sign, fever, stridor, sore throat, odynophagia, shortness of breath, and drooling. These features must be differentiated from those associated with common viral infections. The most helpful diagnostic studies are radiography of the neck and direct laryngoscopy. The patient's airway should be monitored during evaluation to avoid obstruction. Successful management requires teamwork between the primary care physician and personnel skilled in intubation as well as timely…

Epiglottitis in children, 1979 through 1992. Effects of Haemophilus influenzae type b immunization.

OBJECTIVE: To examine secular trends in the epidemiology, bacteriology, and clinical presentation of acute epiglottitis in children in the years surrounding the introduction of vaccine against Haemophilus influenzae type b. DESIGN: Retrospective chart review of patient series. SETTING: Large, urban, tertiary care pediatric hospital. SUBJECTS: One hundred forty-two children with epiglottitis admitted during a 14-year period (1979 through 1992). MAIN RESULTS: The average annual incidence of epiglottitis declined from 10.9 per 10,000 admissions before 1990 to 1.8 per 10,000 admissions from 19…

[Acute epiglottitis. 27 years of experience with and future development of nasotracheal intubation].

During a 27-year period 295 patients in Copenhagen County were treated for acute epiglottitis. One hundred and eight-two were adults and 113 were children. Most children were treated by nasotracheal intubation while only a few adults required nasotracheal intubation in order to secure the airway. The incidence of acute epiglottitis in children less than five years old was calculated to be 8.7/100,000 with minor annual variation. Since vaccination against Haemophilus influenzae type b has been implemented in Denmark from May 1993, this figure will probably be markedly reduced, and the disease e…

Incidental finding of two epiglottic cysts: Case report and literature review.

INTRODUCTION AND IMPORTANCE: Epiglottic cysts are rare benign lesions of the larynx, frequently discovered incidentally. Though usually asymptomatic, they can occasionally result in life-threatening airway obstruction. This case report details the incidental discovery of two epiglottic cysts in an adult female during an elective surgical procedure and outlines their clinical presentation, diagnostic evaluation, and surgical management, supported by a literature review. The report adheres to the SCARE criteria. CASE PRESENTATION: A 43-year-old medically free female presented for elective umbil

Difficult endotracheal intubation due to a large epiglottic cyst: A case report.

RATIONALE: An epiglottic cyst is a type of benign tumor that is formed due to the obstruction of the mucinous duct and the retention of glandular secretion. In such cases, the glottis is not visible as it is covered by the enlarged epiglottic cyst. When conventional anesthesia is administered in such patients, they might have difficulty ventilation since the epiglottic cyst can easily form a flap and move with external pressure changes and can cause the blockage of the glottis due to the loss of consciousness and the relaxation of the throat muscles of the patient. If endotracheal intubation i

Resolution of Signs of Epiglottic Retroversion Following Medical Management of Hyperadrenocorticism in a Dog.

A 6 yr old spayed female Chihuahua was referred for a 10 mo history of chronic respiratory compromise. Decreased serum thyroxine and thyroid-stimulating hormone concentrations had been confirmed at a primary clinic, but no treatment was initiated. Serum biochemistries revealed elevated alkaline phosphatase and cholesterol concentrations. An adrenocorticotropic hormone-stimulating test revealed elevated preserum and postserum cortisol concentrations. Fluoroscopy revealed marked epiglottic retroversion (ER) during inhalation. Enlarged bilateral adrenal glands were found on abdominal ultrasonogra

Angioleiomyoma of the Epiglottis Mimicking Epiglottic Hemangioma: Clinical Experience and Literature Review.

We describe a case of laryngeal angioleiomyoma, discuss its characteristic features and management, and provide a review of the literature, to improve clinical diagnosis and treatment. We report the oldest patient with a laryngeal angioleiomyoma to date and analyze the clinicopathological features reported in the literature. To the best of our knowledge, a total of 36 cases have been described in the English and Chinese medical literature (including our case). The male-to-female ratio was 5:1 and the mean age was 53.89 years. The most common laryngeal site was the supraglottic region (23 cases

Necrotizing epiglottitis with necrotizing fasciitis in a child: A case report and review of literature.

Necrotizing epiglottitis (NE) is a rare but extremely serious variant of acute bacterial epiglottitis. So far, it has been reported mostly in immunocompromised adults and only seldomly in the pediatric population. We describe the case of a 15-month-old immunocompetent child who presented to our tertiary referral center with necrotizing epiglottitis. This manuscript emphasizes the need for rapid recognition and surgical management of this unusual condition, as well as the importance of laryngeal preservation when facing overwhelming necrosis around the laryngeal cartilaginous framework. A revie…

Epiglottopexy by external puncture for epiglottic prolapse in severe laryngomalacia. A novel technique.

INTRODUCTION: Laryngomalacia (LM) is the first cause of stridor in infants. 10 to 20% of patients with LM may require surgery due to the development of severe symptoms. Supraglottoplasty is the most commonly performed surgery for severe LM. However, it is insufficient for the rostrocaudal displacement of the epiglottis against the posterior pharyngeal wall. CASE SUMMARY: We report a case of a 2-month-old infant with severe laryngomalacia with a remarkable collapse of the epiglottis towards the glottis with secondary obstruction of the airway, alteration in swallowing and failure to thrive.
