Slipped upper femoral epiphysis (SUFE) is not an uncommon hip disease in adolescence. Usually, diagnosis of SUFE is delayed, has high risk of sequela and poor long-term results. This paper reviews the literature with a focus on epidemiology, aetiology, diagnosis, treatment and results regarding SUFE.

Posterior lumbar apophyseal ring fractures are rare in adolescents. We report 4 such cases in Chinese adolescents. Two of the patients had a slipped capital femoral epiphysis; 3 of them were overweight/obese. All presented with low back pain and radicular pain. Apophyseal fractures of the upper lumbar spine usually involve the lower end plate, whereas those of the lumbar sacral spine usually involve the upper end plate. The authors retrospectively reviewed seven cases of progressive slipped capital femoral epiphysis after screw fixation. All seven patients initially presented with chronic symptoms, and five had an acute exacerbation of symptoms with the appearance of an acute-on-chronic slip. The purpose of this review was to determine whether the literature supports in situ prophylactic pinning of the hip contralateral to a hip with a slipped capital femoral epiphysis (SCFE). Three hundred twenty-five articles on SCFE between 1931 and 1998 were reviewed. Two hundred six studies were used to establish normative data. This study evaluated long-term clinical and radiologic results of Dunn's osteotomy carried out in 50 consecutive cases for severe slipped capital femoral epiphysis. The results of all the patients were analyzed with 4 years 6 months of follow-up. The clinical results were very good and good in 90% of the patients and were fair or poor in 10% of the patients. Seven patients had an important complication (total necrosis or chondrolysis), and the clinical evolution was studied. Dunn's procedure seems to be adequate to correct severe slips.

We evaluated the prevalence of slipped capital femoral epiphysis in the contralateral hip of 169 children who had been managed with pinning in situ and thirty who had been managed with immobilization in a spica cast. We studied the epidemiology of physiolysis of the hip (or slipped capital femoral epiphysis) between 1946 and 1992 in 175 patients from a well-defined population. The incidence of physiolysis of the hip was calculated using the attack rate method, i.e., the sum of the annual incidences in each 1-year group. We report on a 2-year old boy with the characteristic features of Di George syndrome. The diagnosis was confirmed by history, biochemical, immunologic and radiologic findings. After physical exercise he developed an unilateral epiphysiolysis capitis femoris, rarely seen in this age and not known to be a complication of hypoparathyroidism.

Thirty six patients with a chronic slip of the capital femoral epiphysis and a femoral headneck angle of more than 30 degrees, as measured in a lateral radiograph, were treated by intertrochanteric osteotomy. The authors describe two cases of proximal epiphyseal detachment of the tibia complicated by lesions of the popliteal artery. Following reduction and stabilization by Kirschner wires, vascular repair was done, in one case by the Fogarty method, and in the other by resection and substitution with a vein graft. A case of primary hypothyroidism with pituitary enlargement, slipped capital femoral epiphysis and cystic ovaries is reported. The pituitary abnormality and cystic ovaries disappeared dramatically after thyroid hormone therapy. Hip pinning was performed. The recognition of these associations may eliminate unnecessary surgery and lead to the choice of hormone replacement therapy.

In adolescents, on lateral radiographs of the vertebral column, the avulsion of a lumbar posterior ring epiphysis is a good argument suggesting a posterior herniation of the intervertebral disk. Two such cases are reported. The pathogeny of this tearing, unusual in adults, is discussed. This avulsion should be compared with the retromarginal anterior disk herniations, often observed at this age.
