A controlled trial of scheduled replacement of central venous and pulmonary-artery catheters.

BACKGROUND: The incidence of infection increases with the prolonged use of central vascular catheters, but it is unclear whether changing catheters every three days, as some recommend, will reduce the rate of infection, It is also unclear whether it is safer to change a catheter over a guide wire or insert it at a new site. METHODS: We conducted a controlled trial in adult patients in intensive care units who required central venous or pulmonary-artery catheters for more than three days. Patients were assigned randomly to undergo one of four methods of catheter exchange: replacement every thr

Control of air contaminants at mixers and mills used in tire manufacturing.

The National Institute for Occupational Safety and Health (NIOSH) has completed an evaluation of control techniques for airborne vapors and particulates in tire manufacturing. As part of this evaluation, Banbury mixers and mills were studied. This equipment is used to mix elastomers, carbon black, oil and other ingredients into a homogeneous mixture. These mixing and milling operations normally generate aerosols. Control of these air contaminants may be accomplished by using the following: local exhaust ventilation, equipment configuration and material substitution. Results of personal samplin

The importance of sampling technique and rinse water for assessing flexible gastrointestinal endoscope reprocessing: A 3-year study covering 59 centers.

BACKGROUND: Various guidelines recommend several sampling techniques to verify endoscope reprocessing, but a comparative study of the efficiency for recovering microorganisms was rare. Our goal was to compare different sampling techniques for the postreprocessing endoscope to assess residual bacterial contamination and analysis of the critical factors affecting the endoscope reprocessing failure. METHODS: From 2016 to 2018, 3 techniques, the conventional flushing sampling method, flush-brush-flush sampling method (FBFSM), and pump-assisted sampling method (PASM), were compared covering all 59

The impact of aerosolized mucolytic agents on the airflow resistance of bacterial filters used in mechanical ventilation.

BACKGROUND/PURPOSE: In order to reduce the contamination in the ventilator, bacterial filters were placed on the expiratory limb of a ventilator circuit. Aerosolized mucolytic agents may increase the resistance of the ventilator. The goal of this study is to determine the impact of aerosolized mucolytic agents on the pressure change during mechanical ventilation. METHODS: A lung model was investigated with mucolytic inhaled agents of 10% acetylcysteine and 2% hypertonic saline. The agents were administered using a jet nebulizer every 45 minutes for 15 minutes. The pressure drop was measured a

A prospective randomized comparison of an attached silver-impregnated cuff to prevent central venous catheter-associated infection.

The VitaCuff catheter, a specialized central venous catheter (CVC) with an attached silver-impregnated cuff, is designed to permit percutaneous placement and prolonged venous access. A prospective randomized study was undertaken comparing the VitaCuff with standard triple lumen catheters to determine if the VitaCuff reduces infection during extended use. All consenting patients underwent percutaneous placement of subclavian lines. By study design, control and VitaCuff catheters could remain in site for up to 7 and 14 days, respectively. Cultures were obtained from the preinsertion skin site, a

Nosocomial transmission of congenital tuberculosis in a neonatal intensive care unit.

Congenital tuberculosis is uncommon, and nosocomial transmission from a congenitally infected infant to another infant has not been reported in the English literature. We report an investigation of 2 infants with tuberculosis who were cared for in the same neonatal intensive care unit. Isolates from both infants were genetically indistinguishable. Transmission between the 2 infants was likely due to contaminated respiratory equipment.

Deep Convolutional Approaches for the Analysis of COVID-19 Using Chest X-Ray Images From Portable Devices.

The recent human coronavirus disease (COVID-19) is a respiratory infection caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). Given the effects of COVID-19 in pulmonary tissues, chest radiography imaging plays an important role in the screening, early detection, and monitoring of the suspected individuals. Hence, as the pandemic of COVID-19 progresses, there will be a greater reliance on the use of portable equipment for the acquisition of chest X-ray images due to its accessibility, widespread availability, and benefits regarding to infection control issues, minimizing th

Perceptions of Fungal Contamination in Hospital Humidifiers Among Healthcare Providers in Jordan: A Cross-Sectional Study.

BACKGROUND: Hospital-acquired infections remain a critical concern globally. Fungal contamination of reusable hospital humidifiers, especially in intensive care units (ICUs), represents an under-recognized threat. These devices, if improperly maintained, may become reservoirs for opportunistic pathogens such as Aspergillus and Fusarium, thereby compromising patient safety. OBJECTIVE: The objective of this study is to explore the perceptions of fungal contamination in hospital humidifiers across Jordan and describe associated disinfection practices, protocol presence, and staff awareness, with

Multigeneration cross contamination of mail with Bacillus species spores by tumbling.

In 2001, envelopes loaded with Bacillus anthracis spores were mailed to Senators Daschle and Leahy as well as to the New York Post and NBC News buildings. Additional letters may have been mailed to other news agencies because there was confirmed anthrax infection of employees at these locations. These events heightened the awareness of the lack of understanding of the mechanism(s) by which objects contaminated with a biological agent might spread disease. This understanding is crucial for the estimation of the potential for exposure to ensure the appropriate response in the event of future att

Nonsterile microbial production of chemicals based on Halomonas spp.

The use of extremophile organisms such as Halomomas spp. can eliminate the need for fermentation sterilization, significantly reducing process costs. Microbial fermentation is considered a pivotal strategy to reduce reliance on fossil fuel resources; however, sustainable processes continue to incur higher costs than their chemical industry counterparts. Most organisms require equipment sterilization to prevent contamination, a practice that introduces complexity and financial strain. Fermentations involving extremophile organisms can eliminate the sterilization process, relying instead on cond

The Role of Patient Care Items as a Fomite in Healthcare-Associated Outbreaks and Infection Prevention.

Patient-care items can serve as a source or reservoir for healthcare-associated pathogens in hospitals. We reviewed healthcare- associated outbreaks from medical equipment and provide infection prevention recommendations. Multiple healthcare-associated outbreaks via a contaminated patient-care item were identified, including infections with multidrug-resistant organisms. The type of patient care items implicated as a fomite causing healthcare-associated infections (HAIs) has changed over time. Patient populations at risk were most commonly critically ill patients in adult and neonatal intensiv

Plasmonic Biosensor Controlled by DNAzyme for On-Site Genetic Detection of Pathogens.

On-site predetection of pathogens could significantly decrease of a disease outbreak or national loss in most of the countries. However, conventional detection techniques are limited in use for on-site detection due to the necessity of specialized skill or equipment. Therefore, it is necessary to develop a new technique that can predetect pathogens in the field without special skills or equipment. Here, a DNAzyme strategy to control a plasmonic biosensor for rapid and simple visual detection of Salmonella choleraesuis is adopted. Multicomponent DNAzyme formed by target addition can cleave the

Assessing Viral Transfer During Doffing of Ebola-Level Personal Protective Equipment in a Biocontainment Unit.

BACKGROUND: Personal protective equipment (PPE) protects healthcare workers (HCWs) caring for patients with Ebola virus disease (EVD), and PPE doffing is a critical point for preventing viral self-contamination. We assessed contamination of skin, gloves, and scrubs after doffing Ebola-level PPE contaminated with surrogate viruses: bacteriophages MS2 and Φ6. METHODS: In a medical biocontainment unit, HCWs (n = 10) experienced in EVD care donned and doffed PPE following unit protocols that incorporate trained observer guidance and alcohol-based hand rub (ABHR). A mixture of Φ6 (enveloped), MS2

Microbial contamination of contact lenses, lens care solutions, and their accessories: a literature review.

PURPOSE: A contact lens (CL) can act as a vector for microorganisms to adhere to and transfer to the ocular surface. Commensal microorganisms that uneventfully cohabitate on lid margins and conjunctivae and potential pathogens that are found transiently on the ocular surface can inoculate CLs in vivo. In the presence of reduced tissue resistance, these resident microorganisms or transient pathogens can invade and colonize the cornea or conjunctiva to produce inflammation or infection. METHODS: The literature was reviewed and used to summarize the findings over the last 30 years on the identif

The effect of alcohol swabs and filter straws on reducing contamination of glass ampoules used for neuroaxial injections.

We have investigated the incidence of contamination of the contents of glass ampoules used for neuraxial injections, and whether this was reduced by wiping the outsides of the ampoules with isopropyl alcohol or using a filter straw. One hundred fentanyl and diamorphine ampoules used for routine regional anaesthesia were either wiped or not wiped with alcohol before their contents were aspirated, and the residual contents were swabbed and incubated. None of the swabs from the wiped ampoules grew organisms compared with nine (18%) from non-wiped ampoules (p = 0.004). In a second, laboratory stud
