Ventricular tachycardia due to cardiac ischaemia: assessment by exercise electrocardiography.

Although ventricular tachycardia is a well-known complication of myocardial ischaemia and may be provoked by exercise, many patients may app … artery; other vessels were usually also affected. Although beta-adrenergic blocking drugs increased exercise tolerance, ventricular tachycardia still occurred when the heart rate on exercise reached a level similar to that before treatment. In five patients coronary artery bypass surgery was performed because of angina and exercise-induced ventricular tachycardia. E …

Thermoregulation in the Aging Population and Practical Strategies to Overcome a Warmer Tomorrow.

As global temperatures continue to rise, improving thermal tolerance in the aged population is crucial to counteract age-associated impairments in thermoregulatory function. Impairments in reflex cutaneous vasodilation and sweating response can augment the vulnerability of older adults to heat-related injuries following exposure to heat stress. Mechanisms underlying a compromised cutaneous vasodilation are suggested to include reduced sympathetic neural drive, diminished cholinergic co-transmitter contribution, and altered second messenger signaling events. On the other hand, impairments in sw

Effects of water drinking on cardiovascular responses to supine exercise and on orthostatic hypotension after exercise in pure autonomic failure.

OBJECTIVE: Patients with pure autonomic failure (PAF) have an abnormal fall in blood pressure (BP) with supine exercise and exacerbation of orthostatic hypotension (OH) after exercise. This study assessed the pressor effect of water on the cardiovascular responses to supine exercise and on OH after exercise. METHODS: 8 patients with PAF underwent a test protocol consisting of standing for 5 min, supine rest for 10 min, supine exercise by pedalling a cycle ergometer at workloads of 25, 50 and 75 W (each for 3 min), supine rest for 10 min and standing for 5 min. The test protocol was performed

Effects of cardiac rehabilitation in patients with ventricular assist devices: a scoping review.

INTRODUCTION: Ventricular assist devices represent a treatment option for patients with advanced heart failure, offering control over various haemodynamic variables. Similarly, the prescription of exercise within a cardiac rehabilitation programme for heart failure patients is recommended to reduce symptoms, and hospitalisations, improve cardiorespiratory fitness, and increase exercise tolerance. Therefore, exercise prescription can impact those with ventricular assist devices. Given the limited evidence on exercise-based cardiac rehabilitation programmes for this population, this review aims …

Physical Exercise as Therapy for Frailty.

Longitudinal studies demonstrate that regular physical exercise extends longevity and reduces the risk of physical disability. Decline in physical activity with aging is associated with a decrease in exercise capacity that predisposes to frailty. The frailty syndrome includes a lowered activity level, poor exercise tolerance, and loss of lean body and muscle mass. Poor exercise tolerance is related to aerobic endurance. Aerobic endurance training can significantly improve peak oxygen consumption by ∼10-15%. Resistance training is the best way to increase muscle strength and mass. Although the …

Short-term aerobic exercise reduces nitroglycerin-induced orthostatic intolerance in older adults with type 2 diabetes.

AIMS/HYPOTHESIS: Older adults are at a high risk for syncope due to orthostatic intolerance (OI), and this risk increases with comorbid type 2 diabetes and vasoactive medications. Despite many benefits, previous investigations have shown worsening OI with aerobic training. We examined whether aerobic exercise reduced OI in older adults with type 2 diabetes who were given a short-acting vasoactive agent (nitroglycerin). METHODS: Forty older adults (25 males and 15 females, mean age 71.4 ± 0.7 years, ranging in age from 65 to 83 years) with type 2 diabetes were recruited. Subjects were randomiz

Pulmonary artery pressure-flow relations after prostacyclin in primary pulmonary hypertension.

Exercise tolerance improves within a few weeks after prostacyclin initiation in patients with primary pulmonary hypertension even in the absence of significant changes in resting pulmonary vascular resistance and/or in patients who fail to respond to an acute vasodilator challenge. We tested the hypothesis that this early effect of prostacyclin may be ascribable to an improved pressure-flow response of the pulmonary circulation to exercise. Pulmonary hemodynamic variables at rest and during exercise and the 6-min walking distance were determined before and after 6 wk of continuous intravenous …

Pain responses following resistance exercise with and without blood flow restriction in individuals with multiple sclerosis.

PURPOSE: Pain is a common symptom among individuals living with multiple sclerosis (MS). Exercise-induced hypoalgesia (EIH), a reduction in pain sensitivity following resistance exercise, typically requires high loads. Blood flow restriction (BFR) training may offer a lower-load alternative to elicit EIH. This investigation aimed to assess EIH responses following resistance exercise with and without BFR in individuals with MS. METHODS: 15 participants diagnosed with MS completed three exercise sessions consisting of isotonic, unilateral leg extensions with the current exercise recommendation

Strategies of muscle training in very severe COPD patients.

There is strong evidence that exercise training, constituting the cornerstone of pulmonary rehabilitation, improves exercise tolerance, dyspnoea sensations, functional capacity and quality of life in patients with severe chronic obstructive pulmonary disease. However, intolerable sensations of breathlessness and/or peripheral muscle discomfort may prevent such patients from tolerating high-intensity exercise levels for sufficiently long periods of time to obtain true physiological training effects. Accordingly, the major issue that arises is the selection of the appropriate training strategy, …

[Role of clinical evaluation and functional testing in the tailored follow-up of chronic heart failure patients].

Clinical assessment is crucial to monitor chronic heart failure (HF) patients. It allows to tailor follow-up based on clinical severity, sym … al data should be complemented by objective measures of functional capacity using exercise testing. Exercise tolerance, a comprehensive index of body function, is a pivotal prognostic predictor. The 6-min walking test is simple, well accepted by patients, and provides an objective documentation of exercise tolerance in subjects who cannot perform a maximal stress te …

Pulmonary dysfunction secondary to mandibular retrognathia in Marfan's syndrome.

Referrals to pulmonary physicians for polysomnography to evaluate snoring or the possibility of sleep apnea syndrome often evolve into a multidisciplinary clinical problem. We present a young woman with two congenital abnormalities (Marfan's syndrome and retrognathism) which both may affect her decreased exercise tolerance, daytime hypersomnolence, and fatigue. Polysomnography and pulmonary exercise testing documented improvement in her upper airway diameter and exercise capacity. Her improved sleep pattern and her understanding of her exercise performance limitations afforded her a new outloo…

Evolution of Physical Status From Diagnosis to the End of First-Line Treatment in Breast, Lung, and Colorectal Cancer Patients: The PROTECT-01 Cohort Study Protocol.

Background: Cancer cachexia and exacerbated fatigue represent two hallmarks in cancer patients, negatively impacting their exercise tolerance and ultimately their quality of life. However, the characterization of patients' physical status and exercise tolerance and, most importantly, their evolution throughout cancer treatment may represent the first step in efficiently counteracting their development with prescribed and tailored exercise training. In this context, the aim of the PROTECT-01 study will be to investigate the evolution of physical status, from diagnosis to the end of first-line t…
