Compound heterozygosity of a novel missense variant and exonic deletion in hypomyelinating leukodystrophy 15.

Hypomyelinating leukodystrophy 15 (HLD15) results from biallelic pathogenic variants in EPRS1, but exonic deletions have not been reported. We describe a 40-year-old woman with mild intellectual disability, ataxia, dystonia, and MRI showing hypomyelination. Whole-exome sequencing identified a heterozygous missense variant in the prolyl-tRNA synthetase domain of EPRS1 (c.3430 C > G; p.Leu1144Val, NM_004446.3), without second variant. Whole-genome sequencing revealed a heterozygous 220-bp deletion spanning exon 15 (c.1743-30_1932del), and segregation analysis confirmed compound heterozygosity. R

Compound Heterozygosity of PTF1A Exonic and Enhancer Variants in a Japanese Boy With Pancreatic Hypoplasia.

A Japanese boy carried a paternally inherited single-nucleotide deletion in PTF1A exon 1 (c.775delC) and a maternally inherited nucleotide substitution in the distal enhancer region (g.23508356T>G). Both variants were hitherto unreported. The patient exhibited transient anemia in addition to typical clinical features of pancreatic hypoplasia, but no neurodevelopmental abnormalities. These results highlight the clinical importance and broad mutation spectrum of PTF1A variants as a cause of pancreatic hypoplasia. In addition, our data imply that phenotypes of PTF1A abnormalities are variable and

Exon 21 skipping in ARHGAP10: A splicing switch that governs breast cancer metastasis.

Rho GTPase-activating protein 10 (ARHGAP10) is recognized as a tumor suppressor, yet the functional impact of its alternative splicing isoforms on breast cancer metastasis remains unclear. This study aimed to elucidate the role and regulatory mechanism of ARHGAP10 exon 21 skipping in breast cancer progression. Our research results indicate that in metastatic breast cancer cells, the full-length isoform ARHGAP10-L is downregulated, whereas the truncated ARHGAP10-S is upregulated. The RNA-binding protein HNRNPA0 directly binds to intron 21 of ARHGAP10 pre-mRNA, promoting exon-21 skipping and ARH

An antisense method for efficient exon skipping and its application to Duchenne muscular dystrophy.

Antisense-mediated exon skipping has been used to treat Duchenne muscular dystrophy (DMD) by restoring the DMD reading frame to express partially functional dystrophin. However, difficulty in designing effective antisense oligonucleotides (ASOs) has hindered the development of clinically effective therapies. Here we describe a robust bipartite ASO design, termed 5' splice site decoy (5D)-ASO, in which a short tail sequence is employed to enhance the splicing repression exerted by an antisense moiety. The tail carried by the antisense moiety to an exon of interest in a target pre-mRNA interfere

Confirmation of frameshift variants in the last exon of FGFR1 as a cause of multiple epiphyseal dysplasia.

Multiple epiphyseal dysplasia (MED) is a genetically diverse skeletal disorder characterized by abnormal and delayed epiphyseal ossification, early-onset osteoarthritis, joint pain and mild short stature. MED is primarily caused by pathogenic variants in genes involved in cartilage and bone development, including COMP, MATN, COL9A1-3, CANT1 and SLC26A2. FGFR1 was suggested as a cause of MED in a study published in 2020, which reported a frameshift variant in the last exon of this gene. We assembled an international cohort of 13 individuals with MED carrying C-terminal frameshift variants in FG

Preclinical development of genome editing to treat Duchenne muscular dystrophy by exon skipping.

Duchenne muscular dystrophy (DMD) is caused by loss-of-function mutations to the gene encoding dystrophin. Restoring the reading frame of dystrophin by removing internal out-of-frame exons may address symptoms of DMD. Therefore, the principle of exon skipping has been at the center stage in drug development for Duchenne muscular dystrophy (DMD) over the past two decades. Antisense oligonucleotides (AONs) have proven effective in modulating splicing sites for exon skipping, resulting in the FDA approval of several drugs using this technique in recent years. However, due to the temporary nature …

Exon-Skipping Using Antisense Oligonucleotides for Laminin-Alpha2-Deficient Muscular Dystrophy.

Phosphorodiamidate morpholino oligomer (PMO)-mediated exon-skipping is among the more promising approaches available for the treatment of several neuromuscular disorders, including Duchenne muscular dystrophy. The main weakness of this treatment arises from the low efficiency and sporadic nature of the delivery of neutrally charged PMOs into muscle fibres, the mechanism of which is unknown. Recently, using wild-type and dystrophic mdx52 mice, we showed that muscle fibres took up PMOs more efficiently during myotube formation. Interestingly, we detected PMO mainly in embryonic myosin heavy chai

Exon Utilization Improves Risk Stratification for Advanced Heart Failure in Titin Cardiomyopathy.

Truncating variants in the titin (TTN) gene (TTN-TV) are the most common genetic cause of dilated cardiomyopathy (DCM) and confer a significant risk of progression to advanced heart failure (AHF). Penetrance of TTN-TV has been linked to the level of expression of the exon containing the TTN-TV, quantified using the percent spliced in (PSI). We performed long-read RNA sequencing on cardiac tissue from 8 unused organ donors and 14 DCM patients and identified expression of 16 TTN isoforms. We used these data to recalculate PSI (PSI-LR). PSI-LR reclassified 5% of exons compared to the original PSI…

Evolution and Breakthroughs in Exon Skipping and Splice Modulation: From Inception to Clinical Success.

Since the discovery of RNA splicing in 1977, significant strides have been made in understanding its role in human development and disease. Splice modulation, particularly exon skipping and exon inclusion, have emerged as a promising therapeutic strategy to correct genetic disorders caused by recessive loss-of-function mutations. Exon skipping and exon inclusion involve the use of antisense oligonucleotides (AOs) to mask specific splice sites or regulatory elements on pre-mRNA, enabling the exclusion or inclusion of exons and restoration of the correct reading frame. This process has gained si…

EGFR mutations in sinonasal squamous neoplasms: Novel hotspot for exon 20 insertions.

EGFR mutations and oncogenic high-risk (HR) HPV have been suggested to be mutually exclusive pathways in pathogenesis of sinonasal squamous neoplasms which include sinonasal papilloma and squamous cell carcinoma (SCC). In Indian patients, HR-HPV association is rare; however, there is no data on EGFR mutations in these tumors. One-hundred-eleven cases of sinonasal squamous neoplasms were interrogated for EGFR exon 19 and 20 mutations, including 48 inverted papillomas (IP), 15 SCC arising in the background of inverted papilloma (IP-SCC) and 48 de novo SCC. HR-HPV association was determined by p1

Erythromelalgia caused by the missense mutation p.Arg220Pro in an alternatively spliced exon of SCN9A (NaV1.7).

Erythromelalgia (EM), is a familial pain syndrome characterized by episodic 'burning' pain, warmth, and erythema. EM is caused by monoallelic variants in SCN9A, which encodes the voltage-gated sodium channel (NaV) NaV1.7. Over 25 different SCN9A mutations attributed to EM have been described to date, all identified in the SCN9A transcript utilizing exon 6N. Here we report a novel SCN9A missense variant identified in seven related individuals with stereotypic episodes of bilateral lower limb pain presenting in childhood. The variant, XM_011511617.3:c.659G>C;p.(Arg220Pro), resides in the exon 6A

Detecting and Validating MAPT Mutations in Neurodegeneration Patients and Analysis of Exon Splicing Consequences.

Mutation of MAPT has been observed in patients with parkinsonism, progressive supranuclear palsy, and corticobasal degeneration and is a significant cause of frontotemporal dementia. In this chapter, we discuss considerations for next-generation sequencing analysis to identify MAPT mutations in patient genomic DNA and describe the validation of these mutations by Sanger sequencing. One of the most common effects of MAPT mutations is differential splicing of exon 10, which leads to an imbalance in the proportion of 3-repeat and 4-repeat tau isoforms. We describe how to investigate the effect of

Three exonic variants in the COL4A5 gene alter RNA splicing in a minigene assay.

BACKGROUND: X-linked Alport syndrome (XLAS) is an inherited renal disease caused by rare variants of COL4A5 on chromosome Xq22. Many studies have indicated that single nucleotide variants (SNVs) in exons can disrupt normal splicing process of the pre-mRNA by altering various splicing regulatory signals. The male patients with XLAS have a strong genotype-phenotype correlation. Confirming the effect of variants on splicing can help to predict kidney prognosis. This study aimed to investigate whether single nucleotide substitutions, located within three bases at the 5' end of the exons or interna…

Znrf3 exon 2 deletion mice do not recapitulate congenital adrenal hypoplasia.

Wnt/β-catenin signaling is essential for adrenocortical development. Zinc and ring finger 3 (ZNRF3), an E3 ubiquitin ligase that attenuates Wnt/β-catenin signaling, is negatively regulated by R-spondin via an extracellular domain that is partially encoded by exon 2 of ZNRF3. We recently identified ZNRF3 exon 2 deletions in three individuals with congenital adrenal hypoplasia. ZNRF3 exon 2 deletion impairs R-spondin binding, thereby attenuating β-catenin expression and eventually leading to the development of congenital adrenal hypoplasia. To elucidate the influence of ZNRF3/Znrf3 exon 2 deleti

Quantitative Evaluation of Exon Skipping in Urine-Derived Cells for Duchenne Muscular Dystrophy.

Antisense oligonucleotide (ASO)-based exon skipping therapy is thought to be promising for Duchenne muscular dystrophy (DMD). For the screening or assessing patient eligibility before administering ASO to patients, in vitro testing using myoblasts derived from each DMD patient is considered crucial. We previously reported state-of-the-art technology to obtain patient primary myoblasts from MYOD1-induced urine-derived cells (UDCs) as a model of DMD. We hypothesize that the myoblasts may potentially reflect specific pathological phenotypes, leading to a path for precision medicine in DMD patient

Use of Glycine to Augment Exon Skipping and Cell Therapies for Duchenne Muscular Dystrophy.

Antisense oligonucleotide (AO)-based exon-skipping and cell therapies are the main therapeutic approaches for Duchenne muscular dystrophy (DMD). Insufficient systemic delivery leading to low therapeutic efficacy limits the former; low transplantation efficiency hampers the latter. Here we describe how glycine can address these issues by augmenting satellite proliferation and muscle regeneration, resulting in enhanced AO uptake in regenerating myofibers and cell transplantation efficiency in dystrophic mice. The dual functionality of glycine demonstrated in AO-based exon-skipping and cell thera

The Dilemma of Choice for Duchenne Patients Eligible for Exon 51 Skipping The European Experience.

Antisense oligonucleotide (ASO) mediated exon skipping aims to reframe dystrophin transcripts for patients with Duchenne muscular dystrophy (DMD). Currently 4 ASOs have been approved by the Food and Drug Administration targeting exon 45, 51 and 53 based on low level dystrophin restoration. Additional studies to confirm functional effects are ongoing. Furthermore, efforts are ongoing to increase muscle specific delivery of ASOs. Consequently, there are 5 clinical trials ongoing or planned for exon 51 skipping ASOs in Europe. While exon 51 skipping applies to the largest group of patients, DMD e
