BACKGROUND: Machine perfusion (MP) has evolved as a promising approach for the ex situ preservation in organ transplantation. However, the literature on the use of MP in human vascularized composite allografts is scarce. The aim of this study was to evaluate the effects of hypothermic MP with an acellular perfusate in human upper extremities and compare with the current gold standard of static cold storage (SCS).
METHODS: Six upper extremities were assigned to either MP (n = 3) or SCS (n = 3) conditions for 24 h. MP-extremities were perfused with oxygenated Steen solution at a constant pressu

Complex regional pain syndrome (CRPS) in paediatric patients is clinically distinct from the adult condition in which there is often complete resolution of its signs and symptoms within several months to a few years. The ability to compare the symptomatic and asymptomatic condition in the same individuals makes this population interesting for the investigation of mechanisms underlying pain and other symptoms of CRPS. We used fMRI to evaluate CNS activation in paediatric patients (9-18 years) with CRPS affecting the lower extremity. Each patient underwent two scanning sessions: once during an a

Benign acute childhood myositis (BACM) is a pediatric syndrome characterized by mild self-limiting sudden onset of muscle pain during or following recovery from a viral illness. The case discussed in this report is of an eight-year-old female diagnosed with the common cold after presenting to her primary care physician. Five days later, the patient presented to the emergency department with lower extremity pain. The patient was sent home with supportive care and mild analgesics. Twelve hours later, the patient was seen again in the emergency department with severe bilateral lower extremities p

Posttraumatic dystrophy (PTD) is a syndrome that involves both neurological symptoms and microvascular abnormalities. A total of 44 consecutive patients seen for the first time with PTD were investigated. In 42 patients X-rays were taken: of the cervical spine in the case of PTD of the hand, and of the cervical, thoracic, and lumbar spine in the case of PTD of the foot. Two patients refused this investigation. In all 44 patients microvascular functions were investigated by means of transcutaneous oximetry, laser Doppler flow metering and capillary microscopy. In 34 of 42 patients radiographica

Keraunoparalysis is a transient paralysis of the extremities, which results from close contact with lightning. In this case report, a 58-year-old man came in close contact with a bolt of lightning. His left foot was pulseless, pale, cold and with absence of capillary refill. His symptoms were initially interpreted as arterial occlusion, and therefore bypass surgery and even amputation were considered. However, his symptoms resolved within hours. Therefore, clinicians must consider keranoparalysis as a differential diagnosis in patients struck by lightning.

The purpose of this report is to describe the case of tuberculosis osteitis of the posterior vertebral arch in a 35-year-old man with recent history of pulmonary tuberculosis. Clinical findings were pain due to bilateral inflammation of the lumbar nerve roots, fistulised cold abcess and motor deficit in both lower extremities. The tomodensitometry demonstrated a lytic bone lesion involving the spinous process of the second lumbar vertebra in association with spondylitis and a large paravertebral abscess with calcification typical of tuberculosis. Cure was achieved by a single 12-month course o

Cyclists are vulnerable to weather and commonly experience problems as they relate to extremes in both heat and cold. Educating cyclists on prevention of sunburn, dehydration, heat illness, hypothermia, and frostbite is the key. Limited sun exposure and proper use of sunscreens are the mainstay of treatment for prevention of sunburn. Adequate fluid and electrolyte replacement is necessary to avoid problems with dehydration and heat illness. Wind- and water-resistant clothing is recommended to protect against hypothermia and frostbite. Adequate training and acclimatization are, of course, recom

Drosophila nepalensis is a cold-adapted drosophilid endemic to the Himalayan region. Its ability to survive in harsh, cold conditions makes it a valuable Drosophila model for investigating how adaptation to thermal extremes may influence species persistence under future climate change. Here, we report the first de novo genome assembly of D. nepalensis, based on a hybrid sequencing strategy that combines Illumina short reads and Oxford Nanopore long reads. Illumina sequencing generated 49.88 million 150 bp paired-end reads (∼14.96 Gbp), while Nanopore sequencing produced 1.35 million long reads

Non-freezing cold injury (NFCI), so called trench foot, is a condition characterised by a peripheral neuropathy, developing when the extremities are exposed for prolonged periods to wet conditions at temperatures just above freezing. Classically, military personnel are affected, with 14% of casualties in the Falklands conflict afflicted. Clinically, NFCI is characterised by a well-defined acute clinical picture and chronic sequelae. Little is known regarding the pathophysiology and treatment of this condition. Opinions vary as to the type of nerve fibres most susceptible to damage and proposed

BACKGROUND: Cold agglutinin disease (CAD) is an autoimmune hemolytic anemia that induces blood coagulation and hemolysis upon exposure to cold temperatures. Strict temperature control is essential to mitigate these effects, especially during surgical procedures where hypothermia is possible.
CASE PRESENTATION: A 57-year-old male, 165 cm and 72 kg, diagnosed with CAD, underwent cerebral vascular anastomosis. Intraoperatively, mean arterial pressure was maintained at or above 65 mmHg with phenylephrine administration, while body temperature was rigorously controlled between 36.5 °C and 37.5 °C

An 83-year-old woman was admitted to the emergency department for a 7-day history of fatigue and progressive cyanosis in the feet and hands after cold exposure despite physical protective measures. Upon arrival, the patient presented with necrotic cutaneous lesions in both hands and distal lower extremities. Upon admission, hemoglobin was 7.6 g/dL and laboratory tests were consistent with cold agglutinin disease (CAD), the presence of monoclonal IgM, and flow cytometry consistent with lymphoplasmacytic lymphoma, but MYD88 L265P mutation was negative. The patient required blood transfusion, res

Microbially induced carbonate precipitation (MICP) has been widely used to immobilize Cadmium (Cd) in contaminated soils in mining-affected regions. However, its remediation efficacy under low-temperature stress, as well as the nucleation process that regulates Cd biomineralization via carbonate precipitation by psychrophilic bacteria, has yet to be investigated. Here, we isolated Pseudomonas sp. J-6, a novel urease-producing strain from tailings in high-altitude cold regions, exhibiting unparalleled cold adaptability at 5 °C and achieving 95.85 % Cd removal efficiency by MICP at 10 °C. BACKGROUND: Cryoconite holes, a dynamic habitat in Antarctica and other glaciers, are glacial biological hotspots with high biogeochemical turnover rates, significantly influencing carbon and nitrogen cycling within climate-sensitive glacial ecosystems. They are found on glaciers worldwide, highlighting their global importance, and are likely to become more widespread as glaciers continue to retreat. Despite their global occurrence and sensitivity to climate warming, untargeted metabolomic studies of their dissolved organic matter (DOM) remain scarce, largely due to very low biomass, harsh sam

As extreme temperature events increase globally, the influences of non-optimal temperature on health outcomes are of increasing concern. However, whether there is a correlation between non-optimal apparent temperature (AT) and acne is unknown. To illustrate the association between non-optimal AT and acne, data of daily acne outpatient visits, meteorological factors, and air pollutants from 2015 to 2019 in Chongqing, China were obtained. A multi-center study using distributed lag non-linear model (DLNM) was employed to explore the possible association between short-term non-optimal AT and outpa

INTRODUCTION: Increasing smoking cessation among American Indian (AI) persons is a critical equity goal.
AIMS AND METHODS: During 2022, AI persons (N = 46) were interviewed on perceptions of existing and novel cessation approaches. Intention to use each approach was assessed and questions started with "cold turkey" and then queried about the following: nicotine patches, gum, lozenges (NRT); prescription medications; plant-based medications; personalized medicine; e-cigarettes; and nicotine reduction standard (NRS).
RESULTS: Perspectives informed the identification of six crosscutting themes

Cold exposure that leads to frostbite puts patients at high risk for extremity amputations. Recent treatment advances, such as thrombolytic administration, have decreased amputation rates. However, little is known about patient outcomes with early mobilization of affected limbs. A retrospective review of 41 patients admitted for lower extremity frostbite was performed at an urban hospital burn unit. All patients received the institution's standard frostbite treatment protocol. The study group was allowed to bear weight within 72 hours, while the control group was immobilized beyond this time f

A 77-year-old man was admitted because of muscle weakness in both upper and lower extremities. Diabetes mellitus was diagnosed in 1988 and he had been treated by oral hypoglycemic agents. He had a common cold at the end of January, 1997. Muscle weakness appeared in the upper extremities, followed by the lower extremities at the end of February. No sensory disturbance or dysuria was recognized. Nerve conduction study revealed distally dominant demyelinating polyneuropathy. Guillain-Barré's syndrome was diagnosed and he recovered completely following immunological absorption therapy (IAT). Howev

Background: Paranasal sinuses and mastoid air cells have been attributed to multiple functions-such as voice resonance, cranial lightening, and pressure regulation-yet their potential role in local thermal homeostasis remains underappreciated. The thermoregulatory hypothesis, first proposed in the mid-twentieth century, was largely abandoned after the mid-century, when anthropological findings of climate-correlated variation seemed contradictory. Hypothesis: We propose that pneumatized skull regions form a three-component craniofacial biothermal system that maintains thermal stability in the o
