Development of a topical polymeric mucoadhesive ocular delivery system for azithromycin.

PURPOSE: Azithromycin is an azalide class of antibiotic with pharmacodynamics that have made it a valuable agent in the treatment of soft tissue infections. In ophthalmology, oral administration of azithromycin has been proven effective for the treatment of trachoma. However, topical formulations of azithromycin to treat ocular surface infections have been challenging to develop because of the drug's hydrophobicity and instability in aqueous solutions at pH levels that are comfortable in the eye. The design of a polycarbophil polymer-based delivery system for a topical formulation of azithromy

Discontinuation of highly active antiretroviral therapy leads to cryptococcal meningitis/choroiditis in an AIDS patient.

BACKGROUND: Cryptococcus neoformans is an opportunistic pathogen that can manifest in immunocompromised patients with acquired immune deficiency syndrome. Prevention of cryptococcosis and other opportunistic diseases is an objective in the management of human immunodeficiency virus (HIV)-infected patients. The goal of highly active antiretroviral therapy (HAART) is to reduce the viral loads and enhance CD4 counts in HIV-infected patients. These 2 mechanisms keep HIV-infected patients healthier and enhance their immune systems, thus reducing and often preventing opportunistic infections such as

Identifying Corneal Infections in Formalin-Fixed Specimens Using Next Generation Sequencing.

PURPOSE: We test the ability of next-generation sequencing, combined with computational analysis, to identify a range of organisms causing infectious keratitis. METHODS: This retrospective study evaluated 16 cases of infectious keratitis and four control corneas in formalin-fixed tissues from the pathology laboratory. Infectious cases also were analyzed in the microbiology laboratory using culture, polymerase chain reaction, and direct staining. Classified sequence reads were analyzed with two different metagenomics classification engines, Kraken and Centrifuge, and visualized using the Pavia

Sitting at the window to the world--ocular parasites.

Parasitic infections cause significant ophthalmic disease, both in developing countries and in the Western world. The parasitic infections Acanthamoeba keratitis, ocular toxoplasmosis, and ocular toxocariasis are responsible for a significant proportion of ocular pathology. Especially in light of the recent increase of immunocompromised (i.e. using immunosuppressants or HIV) and aged populations, parasitic infections of the eye are rising in number. This reviews aims to describe the pathogenesis, symptoms, diagnosis and management of infection, as well as preventative measures for these three

A Diagnostic Challenge: Periorbital or Orbital Cellulitis?

The authors present the case of a 39-year-old male who presented to the hospital with worsening eye pain, swelling, and blurred vision of the left eye. His symptoms grew worse despite initial over-the-counter medication and a trip to the urgent care center. A physical exam was concerning for a possible orbital cellulitis given the appearance of the eye and the amount of discomfort, as well as their immunocompromised status, necessitating imaging and workup to confirm the diagnosis. The patient was ultimately diagnosed with periorbital cellulitis and bacterial conjunctivitis, and he received in

Boston keratoprosthesis for visual rehabilitation in porphyria cutanea tarda.

A 56-year-old man, presented with bilateral chronic visual loss associated with generalised skin lesions. He had undergone multiple penetrating keratoplasties in his right eye for recurrent corneal infections and perforations. On ocular examination, his left eye was phthisical and his right eye had light perception vision owing to a failed and vascularised corneal graft. Dermatological evaluation revealed multiple hyperpigmented and hypopigmented lesions along with thickening of skin on nose, scalp and dorsum of hands. Skin biopsy showed focal areas of deposition of faint periodic acid Schiff-

Management of the red eye for the primary care physician.

For the primary care physician, the occurrence of a red eye is a frequent and prominent finding of a disease process in patients. A careful history and simple examination with the observation of typical clinical signs are important for the management of this common disorder. The causes can be classified as painful red eye, trauma, and other common conditions. The most frequent causes of a red eye, such as dry eye, conjunctivitis, keratitis, iritis, acute glaucoma, subconjunctival hematoma, foreign bodies, corneal abrasion, and blunt or penetrating trauma, are described in this article. Simple

[Emergency penetrating keratoplasty in infective lesions of the cornea at the site of sutures].

Urgent perforating keratoplasty was carried out in 52 patients with infective involvement of the cornea at the site of sutures after cataract extraction, perforating keratoplasty, and treatment of penetrating wounds of the cornea. The inflammation was most often caused by Pseudomonas aeruginosa (32%), cocci (33.6%), and fungi (14.1%). The technique of perforating keratoplasty for cases with different localization and extent of purulent infiltration is described and results of pathohistological examination of removed disks presented. The authors emphasize the importance of timely removal of mon

Human immunodeficiency virus disease: changing patterns of intraocular inflammation.

PURPOSE: To evaluate and put into perspective five articles in this issue of the AMERICAN JOURNAL OF OPHTHALMOLOGY that discuss ocular inflammatory disorders in patients with human immunodeficiency virus (HIV) disease. METHODS: We drew upon recent observations concerning the effect of HIV disease on the immune system in an attempt to understand the current reports describing intraocular inflammation. RESULTS: Intraocular inflammation appears to be dependent on several factors, including specific antigenic stimuli and the state of the host immune system. During dynamic changes in these factor

Infectious bovine keratoconjunctivitis: evidence for general immunity.

A study was conducted to determine whether acquired immunity to Moraxella bovis during episodes of infectious bovine keratoconjunctivitis (IBK) was local or general. Twelve calves (principals) were exposed to M bovis in the left eye by conjunctival sac instillation on day 1 and in both eyes on day 21. Six calves exposed in both eyes at day 1 and six others exposed at day 21 served as controls. The mean duration of infection in the right eyes of principal calves was significantly less (P less than 0.001) than that in the left eyes of principal calves, as well as those of the control calves. Adenoviral keratoconjunctivitis.

Acute epibulbar infections in humans are one of the most frequently diagnosed eye diseases. The majority of these are thought to be caused by the adenovirus. This review focuses on the features of the human adenovirus eye diseases, mode of transmission, signs and symptoms, available interventions, and preventive measures. It is concluded that infection control is still widely accepted to be the key to the management of human adenovirus infection in order to prevent the spread, individual suffering and economic damage.
