Fetal Instability and Resuscitative Management During Fetal Intervention: A Retrospective, Descriptive Analysis of 245 Fetal Cardiac and Spina Bifida Procedures.

OBJECTIVE: The heterogeneity of fetal surgery has led to variability in the nature and frequency of fetal instability. Understanding these differences is critical for optimizing management. METHODS: We reviewed all fetal atrial septal interventions (FASI) and fetoscopic myelomeningocele (fMMC) repairs with fetal instability performed at our institution between March 2012 and April 2025. We described and compared the frequency, timing, etiology, and management of fetal instability. RESULTS: During the study period, 28 unique patients underwent 35 FASI procedures, and 210 patients underwent fM

Diagnosis and Management of Spontaneous Twin Anemia Polycythemia Sequence during Early Second Trimester: A Case Report.

INTRODUCTION: Spontaneous twin anemia polycythemia sequence complicates about 3-5% of monochorionic pregnancies. This complication is associated with severe neurological morbidity and mortality in some cases. Prenatal diagnosis relies on Doppler assessment of the middle cerebral artery peak systolic velocity. However, there is ongoing debate regarding the appropriate gestational age to initiate surveillance. CASE PRESENTATION: We present a case of stage III twin anemia polycythemia sequence, first identified at 16 weeks of gestation, given discordant middle cerebral artery peak systolic veloc

Twin-twin transfusion syndrome and related monochorionic disorders: historical perspectives, current controversies, and evidence-based opportunities.

Twin-twin transfusion syndrome remains a major cause of morbidity and mortality in monochorionic twin pregnancies, arising from placental vascular anastomoses and representing a frequent indication for referral to fetal therapy centers. Despite advances in ultrasound assessment and fetoscopic laser therapy, variability in diagnostic thresholds, Doppler interpretation, and recognition of overlapping phenotypes-particularly twin anemia-polycythemia sequence and selective fetal growth restriction-continues to complicate clinical decision-making and limits meaningful comparison of outcomes across

Pre-Operative Predictors of Survival in Twin-Twin Transfusion Syndrome Undergoing Fetoscopic Laser Treatment.

INTRODUCTION: Limited data exist regarding the effect of pre-operative risk factors on fetal survival for patients undergoing fetoscopic laser photocoagulation (FLP) for twin-twin transfusion syndrome (TTTS). The primary objective of this study was to determine the pre-operative variables predictive of single and dual fetal survival at birth for subjects treated with laser for TTTS. The secondary objective was to determine the combined effect of multiple risk factors on single and dual fetal survival at birth. METHODS: This was a prospective cohort study of TTTS pregnancies treated with FLP b

Acute Fetal Metabolomic Changes in Twins Undergoing Fetoscopic Surgery for Twin-Twin Transfusion Syndrome.

Fetuses undergo major surgical stress as well as fluid shifts secondary to both twin-twin transfusion (TTTS) as well as the fetoscopic surgery for treatment of TTTS. While the pathophysiology of TTTS is understood, the acute metabolic changes that fetuses experience from fetoscopic surgery are not. We sought to evaluate the changes in recipient metabolomic profile secondary to TTTS surgery. Amniotic fluid was collected at the beginning and end of four TTTS surgical cases performed from 12/2022-2/2023. Samples were immediately processed and evaluated via NMR-based Metabolomics Facility protocol

Fetal endoscopic tracheal occlusion and pulmonary hypertension in moderate congenital diaphragmatic hernia.

OBJECTIVE: To study the role of fetal endoscopic tracheal occlusion (FETO) on resolution of pulmonary hypertension (PH) in fetuses with isolated moderate left-sided diaphragmatic hernia (CDH). METHODS: This retrospective study included fetuses with CDH evaluated between February 2004 and July 2017. Using the tracheal occlusion to accelerate lung growth (TOTAL) trial definition, we classified fetuses into moderate left CDH if O/E-LHR (observed/expected-lung head ratio) was 25-34.9% regardless of liver position or O/E-LHR of 35-44.9% if liver was in the chest. Postnatal echocardiograms were use

Relapse of Human Chorionic Gonadotropin-Induced Hyperthyroidism and Severe Hyperemesis Gravidarum Secondary to Twin-Twin Transfusion Syndrome, With Rapid Recovery Following Fetoscopic Laser Coagulation: Case Report.

BACKGROUND: Limited data have shown that, compared to uncomplicated twin pregnancies, pregnancies complicated by twin-twin transfusion syndrome (TTTS), a life-threatening condition, are associated with higher maternal serum levels of both human chorionic gonadotropin (hCG) and thyroid hormones. With the continuing expansion of assisted reproductive technologies, the rate of twin pregnancies, including those complicated by TTTS and associated hyperemesis gravidarum, is expected to increase further. Therefore, detailed descriptions of the maternal and fetal clinical outcomes of maternal thyrotox

Fetal tracheal occlusion for congenital diaphragmatic hernia.

Congenital diaphragmatic hernia (CDH) remains one of the most elusive birth defects to treat. Despite greater knowledge of disease and advances in technology, approximately one-third of CDH children born today still die. Consequently, clinicians and researchers have struggled to find the optimal treatment strategies for CDH. Without further innovations in postnatal treatment, many have focused an antenatal approach to improve pulmonary function. Fetoscopic Endoluminal Tracheal Occlusion (FETO) for CDH has evolved to the bedside after decades of research. While still under clinical investigatio

Update on twin-to-twin transfusion syndrome.

Twin-to-twin transfusion syndrome (TTTS) is a serious complication that affects 10-15% of monochorionic multiple pregnancies. Communicating placental vessels on the chorionic plate between the donor and recipient twin are responsible for the imbalance of blood flow. There is evidence for the superiority of fetoscopic laser ablation over serial amnioreductions regarding survival and neurological outcome for stages II-IV TTTS. However, the optimal management of stage I is still debated. The "Solomon" technique showed a significant reduction in recurrent TTTS and post laser twin anemia-polycythem

[Lower urinary tract obstruction (LUTO)--clinical picture, prenatal diagnostics and therapeutic options].

The aetiology of urinary tract obstructions (LUTO) is heterogeneous. The most common entities are isolated posterior urethral valves or urethral atresia in male foetuses. In female foetuses LUTO is frequently a part of complex malformations. The natural history of LUTO is characterised by high morbidity and mortality due to the development of severe pulmonary hypoplasia caused by oligo- or anhydramnios affecting the cannalicular phase (16-24 weeks of gestation) of pulmonary development. The degree of renal damage is variable and ranges from mild renal impairment in infancy to end-stage renal i

Fetoscopic laser therapy for twin-to-twin transfusion syndrome.

Twin-to-twin transfusion syndrome (TTTS) is a unique and complicated phenomenon occurring in 10-15% of monochorionic gestations. The chronic unbalanced distribution of blood volume across placental anastomoses between the donor and recipient fetuses leads to multisystem organ impairment including maladaptive changes in both fetuses. Fetoscopic selective laser photocoagulation (SLPC) is now established as the primary treatment modality for advanced stages of TTTS. SLPC is also associated with a risk reduction in fetal demise and long-term neurological impairment.
