A series of 100 Zimbabwean children aged between 5 months and 13 years with culture positive typhoid fever is presented. The disease was found to be fairly mild with a low prevalence of complications, and no patient in the series died. Possible explanations for the relative mildness of typhoid in this paediatric population are discussed.

Q fever endocarditis, which is seen most often in Great Britain and Australia, has been rarely observed in the United States. A patient with an eight month febrile illness who had signs and symptoms of endocarditis and serologic studies diagnostic of Q fever endocarditis is reported. A history of extensive travel makes it unclear where he originally contracted the disease. Q fever endocarditis is probably underdiagnosed and should be looked for in any case of culture negative endocarditis or chronic fever of unknown origin.

The authors have studied the progress of labour and the neonatal consequences for 100 patients who started labour with a high temperature. A control group consisted of 100 patients with normal temperature and the comparison showed that there was a raised risk of dystocia and of fetal distress during labour. The maternal and neonatal complications were studied in relationship to the aetiology, which was most frequently premature rupture of the membranes. The authors suggest the way these cases should be managed to lessen the maternal and neonatal risk which is associated with pyrexia in labour.

Cimetidine is a rare cause of drug-induced fever. In a patient without evidence of hypersensitivity (the lymphocytes did not react to the drug in vitro) fever was the sole manifestation of an adverse reaction to cimetidine that was confirmed by challenge with the drug.

The classic dramatic clinical presentation of acute rheumatic fever (ARF), with high fever, severe carditis and acute polyarthritis, is changing to an asymptomatic, milder form. The cardiac damage potential of the disease remains high. It is the responsibility of the primary care nurse practitioner to 1) provide primary prevention of ARF through appropriate treatment of streptococcal pharyngitis; 2) be cognizant of the manifestations of the disease; and 3) provide secondary prevention of subsequent attacks through prophylaxis.
