Imaging of cartilage in the athlete.

Since the development of radiography, we have been able to visualize the osseous alterations related to arthritis. These include productive changes, such as osteophyte formation, sclerosis, and buttressing, as well as erosive changes and subchondral cyst formation. However, because cartilage is radiolucent, it is not directly visible by either radiography or computed tomography. With careful attention to technique, both hyaline and fibrocartilage can be visualized by magnetic resonance imaging.

Mandibular exercises to control bruxism and deviation problems.

A discussion of bruxism, mandibular deviation and hypermobility problems producing temporomandibular joint (TMJ) and masticatory muscle pathology is presented. The pathophysiology of painful masticatory muscle hyperactivity, TMJ dysfunction and chondromalacia are presented. The role that pain mediators play in the degradation of TMJ fibrocartilage, acute synovitis and joint and muscle pain is analyzed. Isokinetic and stretching exercises are described to assist in correcting or preventing pathology and dysfunction of these structures.

Editorial Commentary: Magnetic Resonance Imaging Is Not Inferior to the Gold Standard of Diagnostic Arthroscopy for Identification of Wrist Ligamentous Pathology.

Accurate diagnoses of wrist ligamentous injuries are critical to guiding the proper treatment pathway for patients with these pathologies. Although advancements in imaging continue and magnetic resonance imaging is valuable, diagnostic arthroscopy remains superior, particularly if involvement of the noncentral triangular fibrocartilage complex, scapholunate interosseous ligament, or lunotriquetral interosseous ligament is suspected on the basis of history and physical examination and even in the face of negative magnetic resonance imaging findings.

Histological and biochemical evaluation of perichondrial transplants in human articular cartilage defects.

From 1986 to 1992, 88 patients with articular defects in the knee were treated with a perichondrial arthroplasty. In this study, we report on the results for 22 biopsies of grafted tissue with a mean follow-up of 21 months. Biopsies were obtained at routine arthroscopy after approximately 1 year or at arthroscopy or arthrotomy at a later stage when patients were operated on again because of recurrent complaints. Biopsies were taken only when a partial failure was present or when there was a clear failure resulting in fibrocartilage, a loose flap, or a loose body. The biopsies were analyzed his…

Arthrography of the wrist. Assessment of the integrity of the ligaments in young asymptomatic adults.

Fifty-two asymptomatic adults who were between twenty and thirty-five years old had arthrography of the wrist with use of a single injection into the radiocarpal joint. The purpose of the study was to evaluate the integrity of the triangular fibrocartilage, the scapholunate ligament, and the lunotriquetral ligament. Contrast medium was injected under fluoroscopic guidance, and posteroanterior and lateral radiographs of the wrist were made after the subjects had performed exercises of the wrist. No patient who had a history of trauma to the wrist, pain in the wrist, or inflammatory arthritis wa…

In vitro effect of caveolin-1 as a slow-release material on bone-tendon junction healing: A comparative study.

Bone tendon junction injury is hard to cure because of its special anatomical structure, and the treatment applied for bone-tendon junction injury cannot result in the perfect vascular regeneration and restoration of the fibrocartilage zone. In this article, we aim to explore the effect of caveolin-1 as a slow-release material on bone-tendon junction healing. Seventy-two New Zealand rabbits were randomly selected and assigned into the experimental, sham-operated and control groups (n = 24). Caveolin-1 microspheres and microcapsule were developed as drug delivery system. At the 4th, 8th, and 12…

Microfracture: indications, technique, and results.

The so-called "marrow stimulating" technique of microfracture uses an awl to penetrate the subchondral bone in cartilage defects. Disruption of the subchondral bone induces fibrin clot formation in the area of the chondral defect. This clot contains pluripotent, marrow-derived mesenchymal stem cells, which are able to differentiate into fibrochondrocytes, resulting in a fibrocartilage repair with varying amounts of type I, II, and III collagen content. Microfracture is a single-stage procedure that is ideally suited for small, well-contained, Outerbridge grade 3 to 4 cartilage lesions. Most cl…

Bone reinnervation after fracture: a study in the rat.

Reinnervation after tibial fracture in the rat was studied by analyzing the occurrence of growth-associated protein 43 (GAP-43), a marker for regenerating nerve fibers, and protein gene product 9.5 (PGP-9.5), a marker for mature nerve fibers, by immunohistochemistry. At 3 days postfracture, GAP-43--immunoreactive nerve fibers were first observed in the fracture hematoma and periosteum. At 7 days postfracture, abundant sprouting of GAP-43--positive fibers was seen in the callus, hyperplastic periosteum, and edge of fibrocartilage. In the latter region, the nerve fibers were nonvascular, showing…

Carbon fiber implants in osteochondral defects of the rabbit patella.

Carbon fiber implants were used to fill osteochondral defects created on the articular surface of the patella of 36 rabbits, for the purpose of studying the long-term histological changes of the repair process. Six months after surgery the defect was filled by fibrous tissue, where the superficial area was organized parallel to the joint surface. Fibrocartilage developed after 9 months and, after 12 months, the defects were covered by hyaline cartilage tissue.

Wrist Arthroscopy for Athletic Injuries.

Management of hand and wrist injuries for athletes often places emphasis on an expeditious return to sport. Arthroscopic techniques have the advantage of directly visualizing joint derangements and correcting them via a minimally invasive approach. This article discusses the evaluation and management of common wrist injuries treated with arthroscopy in athletes, including scapholunate and lunotriquetral injury, triangular fibrocartilage complex tears, hamatolunate impingement, and arthroscopic-assisted reduction of wrist fractures.

Upper Extremity Injuries in Gymnasts.

Gymnastics is a unique sport, which loads the wrist and arms as weight-bearing extremities. Because of the load demands on the wrist in particular, stress fractures, physeal injury, and overuse syndromes may be observed. This spectrum of injury has been termed "gymnast's wrist," and incorporates such disorders as wrist capsulitis, ligamentous tears, triangular fibrocartilage complex tears, chondromalacia of the carpus, stress fractures, distal radius physeal arrest, and grip lock injury.

Increased size of a gas-filled intradural cyst causing acute foot drop: a case report.

We describe the case of a 76-year-old man presenting with a gas-filled intradural cyst that increased in size over a 10-month period and caused acute bilateral foot drop. The gas-filled intradural cyst was resected from the adherent cauda equina, and histopathological examination identified cystic tissue with degenerated fibrocartilage. Leg pain disappeared immediately following surgery, and the bilateral foot drop resolved within 8 months.
