Three-Dimensional Quantitative Magnetic Resonance Imaging Cartilage Evaluation of the Hand Joints of Systemic Sclerosis Patients: A Novel Insight on Hand Osteoarthritis Pathogenesis-Preliminary Report.

BACKGROUND: Osteoarthritis of the hand joints in systemic sclerosis (SSc) patients might be an independent manifestation leading to limitation of upper extremity function. There is no publication quantitatively assessing the thickness of articular cartilage within the hand joints of SSc patients by MRI. The purpose of our study was to quantify the condition and thickness of hand joints cartilage with three-dimensional quantitative MRI (3D q-MRI). METHODS: The study was conducted in twenty people: ten patients with SSc and ten healthy individuals. All participants were examined with the 3D q-M

Multiple Bony Mallet Finger Injuries in One Hand of a 14-Year-Old Boy.

Bony mallet finger injuries, commonly seen as isolated incidents, typically occur in active individuals. We report a rare case of simultaneous avulsion fractures at the distal phalangeal bases of the second, third, and fourth fingers on the right hand of a 14-year-old boy following a forced passive flexion injury during a football game. The patient initially received conservative management with a finger extension splint for the distal interphalangeal (DIP) joints. However, one week after the injury, we performed surgical fixation on all affected digits using the K-wire extension block method

Pollicization of the index finger for reconstruction of the congenitally hypoplastic or absent thumb.

Between 1962 and 1980, 30 pollicizations of the index finger for congenital absence of the thumb were performed. None of the 30 index fingers were lost during the intraoperative or postoperative period. Of the 14 cases followed up on a long-term basis, six required secondary procedures, three for scar revisions; one, bone shortening; one, tenolysis of flexor tendons; and one, tenolysis of extensor tendons. All new thumbs showed normal sensibility. In four cases opposition was possible with all three residual fingers; in five cases, with the long and the ring finger; and in five cases, with the

Clinical replantation of digits and its problem.

571 replanted digits in 395 patients were critically evaluated from the point of view of the surgical technique, level of amputation, post-operative complications and the eventual functional end result. When the period of ischaemia exceeded 10 hours, the risk of failure was higher. Bone union was delayed in the digits in which the period of ischaemia was longer than 5 hours. Single thumb and multiple finger amputations were functionally most useful regardless of the level of amputation, the range of mobility or the sensory recovery. Single finger replantations resulted in poor function especia

Open Double Mallet Lesion of the Ring Finger with Concomitant Little Finger Fracture: A Case Report.

Background and Clinical Significance: Mallet finger is a common injury of the extensor mechanism at the distal interphalangeal (DIP) joint; however, open double mallet lesions are rare and may present a complex reconstruction challenge. Case Presentation: A 15-year-old male high school student who sustained an open injury to the left ring and little fingers after a high-energy buggy accident. The ring finger showed an open double mallet lesion in which the extensor tendon remained attached to a tiny avulsion fragment, and a separate dorsal base fragment was also present. The adjacent little fi

The sprained finger that isn't.

Six athletic injuries commonly misdiagnosed as a sprained finger are three types of closed tendon injuries, two types of intra-articular fractures involving the middle joint and a volar plate avulsion of the middle joint. The proper diagnosis can usually be made with a good history, a thorough examination and anteroposterior, lateral and oblique x-rays of the finger. Reconstructive procedures can be performed for missed injuries, but the results are inferior to those obtained with early treatment.

Treatment for complete extensor tendon rupture: A case report on extensor pollicis longus tendon transfer and tenodesis procedure to radius for a patient with rheumatoid arthritis.

The complete loss of finger extension leads to significant inconvenience in daily life and often requires surgical treatment. Despite some disadvantages, the Boyes method, which uses the flexor digitorum superficialis tendon, is commonly performed for complete extensor rupture. We report the case of a 73-year-old woman living alone diagnosed with a subcutaneous rupture of all extensor tendons from the index to the little finger. The favourable range of motion of her wrist allowed us to perform extensor tenodesis. Additionally, the patient had a dislocated thumb interphalangeal (IP) joint, enab

[Transfer of the distal phalange of the ring finger as a method of reconstruction of the partially amputated thumb--preliminary report].

Loss of a thumb accounts for 60% decrease of functional value of the hand, thus any attempt of it's reconstruction is a priority in the field of hand surgery. Optimal reconstructive solution for subtotal thumb amputations has not been settled. The results of partial pollicization of ring finger in 5 adult men is presented. Four patients sustained distal subtotal thumb amputation (MCP joint level), one total amputation. In three patients the distal phalange of the ring finger was transferred with the DIP joint, FDP tendon and a fragment of extensor tendon. An effective movement of a newly creat

[Trigger digits].

Trigger finger is an entity seen commonly by hand surgeons. It is produced by a size mismatch between the flexor tendon and the A1 pulley, which causes pain, clicking, catching, and loss of motion of the affected finger. The diagnosis is usually easy but other pathological processes (extensor apparatus instability, locked metacarpo-phalangeal joint) must be excluded. Treatment modalities in trigger finger include splinting, corticosteroid injection and/or surgery. Indication depends on the clinical form of trigger finger.

Traumatized index finger pollicization for thumb reconstruction-A case report.

When the index finger is injured or severed with a traumatic thumb amputation, transfer of the injured index finger can restore the function of the thumb. The purpose of this study was to evaluate the result of the transfer of an injured index finger for the traumatic loss of the thumb. A patient had a traumatic amputation at the first metacarpal level with the bone defect of the second metacarpal and proximal part of index finger. She was treated with a pedicled transfer of the injured index finger to the ipsilateral thumb. Postoperative evaluations included thumb range of motion, opposition

Functional improvement by body-powered 3D-printed prosthesis in patients with finger amputation: Two case reports.

RATIONALE: The most common upper limb amputations are finger amputations, resulting in functional limitations that lead to problems with activities of daily living or job loss. For many years, prosthetic options for finger amputations have been limited to passive prostheses. In many countries including South Korea, body-powered finger prostheses have rarely been prescribed due to high cost, lack of experience of physicians and prosthetists, low interest and no coverage by insurance benefits. We report 2 cases of work-related finger amputations in patients who received body-powered 3D-printed f

Camptodactyly resulting from paradoxical action of an anomalous lumbrical muscle.

A 17 year old girl presented with camptodactyly resulting from an anomalous insertion of the lumbrical muscle into the flexor digitorum superficialis tendon. The fourth anomalous lumbrical muscle paradoxically caused flexion of the proximal interphalangeal (PIP) joint of the little finger when the patient extended the fingers fully. She was operated on, and the lumbrical tendon was cut at its insertion into the flexor digitorum superficialis tendon. The tight fascial band and skin contracture at the PIP joint were released, and the skin defect covered with a transposed flap. She made a good re

Radiographic changes in the hands following childhood frostbite injury.

Three young adult patients who had sustained severe frostbite of the hands as children were recently evaluated for progressive deformity and joint pain in the fingers. Characteristic radiographic abnormalities including dwarfing of the middle and distal phalanges, irregular and malapposed articular surfaces, malalignments at the proximal and distal interphalangeal joints, and evidence of degenerative arthritis in the interphalangeal joints were observed. The metacarpal phalangeal joints and wrists were spared in all patients, and fingers which had been protected from the initial cold injury we

Factors influencing final range of motion in the fingers after fractures of the hand.

We evaluated 123 patients in which we treated 150 fractured fingers primarily. Sixty-seven percent of these fractured fingers required open reduction with internal fixation; the remainder had closed reduction with internal or external fixation, or immediate arthrodeses. Active and passive range-of-motion (ROM) exercises were usually begun within 3 days after injury. The active and passive ROM of all joints of each finger in the involved hand were measured at regular intervals until no further change in ROM occurred. An associated crush injury, a flexor or extensor tendon injury, or a skin loss

Focal hand dystonia: lack of evidence for abnormality of motor representation at rest.

OBJECTIVE: To examine whether the corticospinal system emanating from the primary motor cortex may be organized to facilitate generation of dystonic movements. METHODS: In this cross-sectional observational study, finger movement (FM) representations were assessed in 10 patients with focal hand dystonia (FHD) and 10 matched healthy controls by transcranial magnetic stimulation during rest. Evoked finger movements of the right hand were recorded using an instrumented data glove. Patterns of finger joint movements were analyzed using cluster analysis. Principal component analysis and centers of
