Scoliosis Correction with One Ventricle: A Multispecialty Approach.

BACKGROUND: Patients with single-ventricle congenital heart disease may present for scoliosis correction. These patients undergo a series of cardiac operations that create a novel circulation that has a significant impact on the management of their spinal surgery. CASE DESCRIPTION: A 21-year-old man with severe scoliosis presented for posterior T4-L3 spinal fusion. He was born with complex congenital heart disease that resulted in his having a single functioning ventricle. He underwent a series of operations culminating in a Fontan procedure to palliate his heart disease. Both the surgical pr…

Location of acutely successful radiofrequency catheter ablation of intraatrial reentrant tachycardia in patients with congenital heart disease.

Intraatrial reentrant tachycardia (IART) is common after surgery for congenital heart disease (CHD). Radiofrequency (RF) catheter ablation of IART targets anatomic areas critical to the maintenance of the arrhythmia circuit, areas that have not been well defined in this patient population. The purpose of this study was to determine the anatomic areas critical to IART circuits, defined by activation mapping and confirmed by an acutely successful RF ablation at the site. A total of 110 RF ablation procedures in 88 patients (median age 23.4 years, range 0.1 to 62.7) with CHD were reviewed. Patien

Long-term Outcomes after Fontan Single Ventricle Palliation: A Systematic Review and Meta-Analysis of Regional Registry Data.

BACKGROUND: While several Fontan patient registries have been established, a systematic analysis of these collective data does not exist to provide insight into long-term outcomes. This meta-analysis aims to provide an updated assessment of published Fontan patient outcomes. METHODS: A systematic search identified registry studies reporting long-term outcomes following the Fontan procedure. A reconstructed individual patient data-based meta-analysis assessed survival and failure rates. Subgroup analyses evaluated outcomes across different Fontan techniques. RESULTS: Six registry studies, inc…

Semi-Automatic Planning and Three-Dimensional Electrospinning of Patient-Specific Grafts for Fontan Surgery.

This paper proposes a semi-automatic Fontan surgery planning method for designing and manufacturing hemodynamically optimized patient-specific grafts. Fontan surgery is a palliative procedure for patients with a single ventricle heart defect by creating a new path using a vascular graft for the deoxygenated blood to be directed to the lungs, bypassing the heart. However, designing patient-specific grafts with optimized hemodynamic performance is a complex task due to the variety of patient-specific anatomies, confined surgical planning space, and the requirement of simultaneously considering m

Ten-year outcomes of transcaval cardiac puncture for catheter ablation after extracardiac Fontan surgery.

BACKGROUND: Although they are at lower risk, patients with previous extracardiac conduit (EC) Fontan still may require catheter ablation for supraventricular arrhythmia. OBJECTIVE: The purpose of this study was to determine the optimal approach to pulmonary venous atrium (PVA) access after EC Fontan operation. METHODS: All electrophysiological procedures requiring PVA over a 10-year period at the UCLA Medical Center were reviewed. PVA was grouped by transcaval cardiac puncture (TCP) or direct conduit puncture. Procedural characteristics and outcomes were compared. RESULTS: Between June 2009

Incidence and factors influencing the spontaneous closure of Fontan fenestration.

INTRODUCTION: The Fontan operation is the final stage of single ventricle palliation in patients with complex congenital heart disease. Fenestration in the Fontan conduit, providing an atrial level right to left shunt, has been shown to reduce early postoperative morbidity. However, there is limited data on the long-term fate of this fenestration. The aim of this study is to define the rate of spontaneous closure of the fenestration in the Fontan conduit and factors predictive of the fate of the fenestration. METHODS: This was a retrospective study reviewing the medical records of the patient

Subaortic obstruction in univentricular heart: results using the double barrel Damus-Kaye Stansel operation.

OBJECTIVE: We review our experience with relief of subaortic obstruction in univentricular hearts following pulmonary artery banding (PAB) with double barrel Damus-Kaye Stansel procedure (DKS) and subsequent staged palliation to Fontan. The purpose was to determine if PAB alters semilunar valve function after the double barrel DKS procedure and if this staged approach negatively influences the achievement of Fontan palliation. METHODS: From January 1990 to March 2006, 27 patients underwent PAB (mean 22 days, range 1-150 days; 3.4kg) and coarctation as corrected simultaneously in 18 (18/27) 66

Portosystemic venous shunt in the patients with Fontan circulation.

Portosystemic venous shunt (PSVS) is a vascular anomaly between the portal and systemic veins, resulting in several critical complications. Although PSVS is often associated with congenital heart diseases, the clinical association between Fontan circulation and PSVS has not been elucidated. This study aimed to investigate the clinical features of Fontan patients with PSVS. Two hundred thirteen patients who underwent Fontan procedure are being followed up at Adult Congenital Heart Disease clinic in Kyushu University Hospital. Among them, 139 adult patients underwent cardiac catheterization betw…

The Fontan patient: inconsistencies in medication therapy across seven pediatric heart network centers.

Patients who have undergone the Fontan procedure are at risk for thrombosis, ventricular dysfunction, and valve regurgitation, but data to guide the medical treatment and prevention of these adverse outcomes in this population are lacking. This analysis examined medication usage among Fontan patients by putative indication and by study center. The medical history and current medications of 546 Fontan subjects, ages 6-18 years, were assessed in a Pediatric Heart Network multicenter cross-sectional study. Cardiac imaging was performed within 3 months of enrollment. The majority of the subjects (…

[Fontan's operation: results, immediate and long-term postoperative complications].

The Fontan operation and its modifications can be relatively safely performed in the treatment of many complex congenital heart diseases, such as univentricular heart and tricuspid atresia. The main postoperative complications following the Fontan procedure and the incremental risk factors involved in their development are investigated. Between January 1984 and January 1988 eleven patients (6 females and 5 males), ranging in age from 2 to 15 years (mean age: 7.3 +/- 3.7) and in weight from 10.8 to 50 Kg (mean weight 22.3 +/- 12.7) underwent the Fontan operation in our Department. Pulmonary haemodynamics in Fontan physiology after lobectomy in a patient with a single ventricle associated with pulmonary sequestration.

A 2-year-old girl with a functionally univentricular heart associated with a pulmonary sequestration underwent right lower lobectomy after which increased lung volume with low mean pulmonary artery pressure and pulmonary vascular resistance was documented. A cardiac catheterisation performed after a subsequent total cavopulmonary connection demonstrated favourable Fontan haemodynamics. Lobectomy may have induced compensatory lung growth, contributing to the maintenance of haemodynamics favourable for the long-term success of the Fontan procedure.

[General Anesthesia for Magnetic Navigation System-guided Catheter Ablation in a Patient with Persistent Atrial Tachycardia and an Extracardiac Total Cavopulmonary Connection].

Atrial arrhythmia is an important prognostic factor in patients undergoing the Fontan operation. Although catheter ablation is considered to … ed as sched- uled. The number of catheter ablation procedures in patients with arrhythmia after the Fontan procedure is expected to increase as patients with complex congen- ital heart defects are surviving longer. It is important to clarify the issues associated with this procedure and establish a safe anesthetic management strategy based on hemodynamic data.

Pulsed Field Ablation for Atrial Fibrillation in Fontan Palliation.

INTRODUCTION: Pulsed-field ablation (PFA) is a novel and effective modality for ablation in patients with atrial fibrillation (AF). Available case reports and small series showed promising results in patients with congenital heart disease. However, PFA ablation in Fontan patients has not been reported yet. METHODS AND RESULTS: We present a 35-year-old patient with a history of Fontan palliation for hypoplastic left heart who underwent successful ablation for AF with PFA. We describe procedure characteristics and illustrate the potential benefits of PFA in this patient population. CONCLUSION:

Total cavopulmonary connection for functionally single ventricle with pulmonary atresia and abnormal arborization of pulmonary arteries--exclusion of overwhelmed area by collateral arteries from Fontan circulation.

Functionally single ventricle with major aortopulmonary collateral arteries and pulmonary atresia is very rare. Surgical indication for this combination of abnormalities is still unclear and Fontan procedure is still a crucial challenge for the patient who has this anomaly. We report a case with asplenia, functionally single ventricle, pulmonary artesian and pulmonary abnormal arborization who successfully underwent staged Fontan operation. In this case, the left upper lobe area, where the flow from a Glenn shunt was overwhelmed by the collateral arterial blood flow, was excluded from the Font…

Adequacy and Safety of Liver Biopsy Performed During Cardiac Catheterization in Patients With Fontan and Non-Fontan Heart Disease.

BACKGROUND AND AIMS: Liver biopsy is the gold standard for diagnosis of hepatic fibrosis. Biopsy performed during cardiac catheterization is a unique approach performed at few centers. It allows for simultaneous hepatic and cardiac assessment and may be useful in the surveillance of Fontan-associated liver disease, a recognized complication in Fontan patients. This study reviews the indications, outcomes, and adequacy of liver biopsy performed during cardiac catheterization, with special attention to patients with Fontan-associated liver disease. METHODS: A multidisciplinary, retrospective re
