Early results of a physiotherapist-delivered Ponseti service for the management of idiopathic congenital talipes equinovarus foot deformity.

We studied 24 children (40 feet) to demonstrate that a physiotherapist-delivered Ponseti service is as successful as a medically-led programme in obtaining correction of an idiopathic congenital talipes equinovarus deformity. The median Pirani score at the start of treatment was 5.5 (mean 4.75; 2 to 6). A Pirani score of > or =5 predicted the need for tenotomy (p < 0.01). Of the 40 feet studied, 39 (97.5%) achieved correction of deformity. The remaining foot required surgical correction. A total of 25 (62.5%) of the feet underwent an Achilles tenotomy, which was performed by a surgeon in the p

Surgical treatment of mild hallux valgus deformity: the state of practice among academic foot and ankle surgeons.

BACKGROUND: The purpose of this study was to determine the operative procedure of choice among academic foot and ankle surgeons practicing in the United States for treatment of mild hallux valgus deformity. METHODS: A hypothetical patient was created: a 60-year-old woman with a mild hallux valgus deformity (first intermetatarsal angle of 11 degrees and hallux valgus angle of 22 degrees). The patient complained of pain around the bunion, nonoperative treatment had failed, and she desired operative correction. The case was sent to academic foot and ankle surgeons to identify their operative tre

Early onset cerebellar ataxia with retained tendon reflexes: foot deformity in a first grade family member.

Early onset cerebellar ataxia with retained tendon reflexes (EOCA) is a clinical syndrome characterised by progressive cerebellar ataxia wit … mmer toes and peripheral neuropathy. The patient's father did not have any ataxia, but had the same foot deformities as his daughter and a slight peripheral neuropathy. The possible relationship between these clinical features is discussed.

Split anterior tibial transfer for spastic equinovarus foot deformity: retrospective study of 73 operated feet.

The aim of this study was to describe the long-term results of split tibialis transfer in patients with stroke, cerebral palsy, and brain damage due to trauma, tumor, or infection. An evaluation was made of 73 feet in 69 patients with tendon transfer or lengthening for an average follow-up period of 44 months (range, 1 to 14 years). The primary indications for surgery included pain caused by pressure of the foot or toes on the floor or in shoes, ankle instability due to varus deformity, or difficulty wearing orthopedic shoes or braces chi(2) tests were used to compare preoperative and postoper

Split hand/split foot deformity and LADD syndrome in a family: overlap between the EEC and LADD syndromes.

A mother and daughter are reported with apparently dissimilar syndromes. The mother has a split hand/split foot deformity and the daughter a condition consistent with a diagnosis of LADD syndrome. Absence of clefting and deficient formation of saliva and tears are the main signs that differentiate the LADD from the EEC syndrome. However, no distinct feature is constant between these two autosomal dominant disorders that show great phenotypic variability. This report emphasises the overlap between the LADD and the EEC syndromes.

The influence of mediolateral deformity, tibial torsion, and foot position on femorotibial load. Prediction of a musculoskeletal computer model.

The influence of mediolateral deformity, tibial torsion, and different centers of foot support was studied with a three-dimensional computer model that incorporates the significant muscles of the lower extremities needed for quasi-static walking. This theoretical method avoids the variability in gait pattern from the pain and discomfort associated with deformity in patients. The study illustrates the possible importance of the muscle force on the load across the knee and ankle. High strains in the medial gastrocnemius and the medial hamstring created particularly high loads in the medial compa

Deformity of the foot following anterior transfer of the posterior tibial tendon and lengthening of the Achilles tendon for spastic equinovarus.

Twenty-four children with spastic equinovarus deformity due to cerebral palsy were treated by anterior transfer of the posterior tibial tendon and Achilles tendon lengthening. In five patients, the operation was performed on both sides, making a total of 29 feet available for evaluation after an average follow-up of five years. Only 38 per cent of the results were graded "good" or "satisfactory." Sixty-two per cent were rated as "poor" because of valgus, calcaneus or equinus deformity severe enough to require re-operation. The post-operative deformity was generally evident one or more years af

A comprehensive weightbearing computed tomography study: the pathogenesis of first metatarsal pronation in sesamoid bone displacement, due to hallux valgus deformity and progressive collapsing foot deformity (PCFD).

PURPOSE: This study aimed to explore the biomechanical interrelationships in Progressive Collapsing Foot Deformity (PCFD), also known as flatfoot, with concurrent hallux valgus (HV), first metatarsal pronation and sesamoid bone displacement. The primary purposes were to quantify correlations between arch collapse, first metatarsal rotation, HVA (hallux valgus angle), and sesamoid displacement using weight-bearing computed tomography (WBCT), which provides superior three-dimensional insights compared to traditional radiographs. The central research question was: How is M1 rotation related to al

Treating symptomatic flexible flatfoot deformities. a novel technique: comparison of uc berkeley laboratory foot orthosis with and without kinesio taping in juvenil athletes.

PURPOSE: Symptomatic flexible pes planus (SFPP) can cause pain and discomfort when walking or engaging in sportive activities in children and adolescents. SFPP can be treated conservatively with foot orthoses, such as the University of California Berkeley Laboratory (UCBL) foot orthosis, which can improve foot function and reduce pain. Kinesio Tape (KT) has also been used as an adjunct to foot orthoses in the treatment of pes planus. This study aims to compare the effectiveness of the UCBL foot orthosis with and without KT in the treatment of SFPP among amateur juvenile and adolescent athletes

The efficacy of split tibial tendon transfers on functional gait outcomes for children and youth with cerebral palsy and spastic equinovarus foot deformities.

To systematically review the efficacy of split tendon transfer surgery on gait-related outcomes for children and adolescents with cerebral p … tendon transfers are an effective treatment for children and youth with CP and spastic equinovarus foot deformities. Clinical data presented can be used for future study designs; a more standardized functional and patient-focused approach to evaluating outcomes of surgical intervention of gait may be warranted.

Functional Outcomes of Minimally Invasive Percutaneous Surgical Correction of Late-Presenting Severely Deformed Foot in a Patient with Myelomeningocele.

BACKGROUND: Congenital talipes equinovarus (CTEV), continues to rank among the most common congenital musculoskeletal deformities. The management of CTEV, particularly when it is part of a syndromic presentation or associated with conditions such as myelomeningocele (MMC), presents a significant challenge due to the deformity's tendency to be stiffer and more complex. Most children with CTEV may require surgical intervention. The current case report highlights the functional outcome of minimally invasive percutaneous surgical correction in a late-presenting, rigid, and severe foot deformity in

Long-term follow-up of the medial arch correction with calcaneal medialization osteotomy in progressive collapsing foot deformity.

PURPOSE: Medializing displacement calcaneal osteotomies are part of conservative surgical treatments and represent a reliable option in valgus flatfoot deformities. Favorable short-term results of this procedure is well-known. However, there are few series with follow-up beyond five years. This study reports the clinical outcomes of calcaneal medialization osteotomy with a minimum follow-up of five years. The primary objective was to compare functional scores and radiographic measurements at the preoperative stage, immediately post operative, and at the final follow-up. MATERIALS AND METHODS:…

Acquired Pes Planovalgus : Current Concepts - "From Adult Acquired Pes Planovalgus to Progressive Collapsing Foot Deformity".

The clinical disorder traditionally known as pes planovalgus due to posterior tibial tendon insufficiency or adult-acquired pes planovalgus has been the subject of several publications over the past two decades. Now, it is understood that the problem does not lie in the posterior tibial tendon per se and may even occur without tendon injury. Studies have brought new concepts and understanding that question the views on this subject, culminating in the replacement of existing classifications with one that is more assertive and discriminative of the potential presentation patterns of the deformi

An evaluation model for children's foot & ankle deformity severity using sparse multi-objective feature selection algorithm.

Foot & ankle deformity is a chronic disease with high incidence and is best treated in childhood. However, the current diagnostic procedures rely on doctor's consultation and empirical judgment, and lack objective and quantitative evaluation methods, resulting in low screening rates. To solve this problem, this paper aims to construct an evaluation model for children's foot & ankle deformity through data mining and machine learning technologies. Firstly, it proposes the grading rules for children's foot & ankle deformity severity based on analyzing the existing quantitative indexes and expert

Calcaneal Sliding Osteotomy Versus Calcaneal Lengthening Osteotomy for Valgus Foot Deformity Correction in Children With Cerebral Palsy.

BACKGROUND: Medial calcaneal sliding (CS) osteotomy and lateral column lengthening (LCL) are often performed to relieve pain and improve transverse plane alignment and gait stability for children with cerebral palsy (CP) and valgus foot deformities. The purpose of this study was to examine the effectiveness of these procedures in this population. METHODS: Retrospective medical record review (including 3D gait analysis data) of patients with CP who underwent LCL (26 subjects, 46 limbs) or CS (46 subjects, 73 limbs). Data extraction included complications (modified Clavien-Dindo system), change…

Configuration design and correction ability evaluation of a novel external fixator for foot and ankle deformity treated by U osteotomy.

As a basic osteotomy technique, U osteotomy can be applied for certain complex foot and ankle deformities. Gradual correction cases using Ilizarov apparatus and Taylor Spatial Frame have been reported. This paper proposes a novel parallel distraction apparatus for U osteotomy (PDA-Uos) to supplement the correction equipment for surgeon. Designed with an adjustable structure, the PDA-Uos can adopt different assembly shapes (joint connection points). However, the influence of the change in assembly shape on interference-free workspace and self-structural performance of the external fixator have

Surgery for foot deformity in patients with congenital myopathy (multicore disease, congenital fiber-type disproportion, and centronuclear myopathy).

Foot deformities associated with congenital myopathy are rarely treated surgically; therefore, few reports are available. We present cases of three patients who underwent surgery for foot deformities caused by congenital myopathy. Posteromedial release without talocalcaneal joint release is our surgical procedure. Surgical procedures in each case were determined on the basis of radiographic findings. Surgery resulted in plantigrade feet in the three patients, all of whom were able to walk without pain more than 3 years postoperatively.
