Superficial white onychomycosis revisited.

BACKGROUND: Superficial white onychomycosis (SWO) is a distinct pattern of fungal nail invasion, which is usually treated with topical antifungals. OBJECTIVE: This paper presents a case of SWO with deep penetration and records other similar cases. METHODS: The clues to deep invasion of the nail plate are twofold: an inability to clear the discoloration by scraping the nail and a clinical involvement of the nail plate in the proximal nailfold area. Histology of the nail keratin will confirm deep penetration beyond the superficial layers of the nail plate. RESULTS: In the light of this findin

Assessing onychial disorders in the older patient.

Disorders and diseases of the toenails (onychial) in the older patient are often the focus of the primary foot complaints. They may be a result of the aging process, the end result of repetitive microtrauma or historical major trauma, a complication or related clinical change associated with a multiple system disease or condition, or a localized foot problem. This article details the symptoms of and problems associated with onychial disorders in the elderly.

Tungiasis (tungosis) comes to the Czech Republic.

A case of a 39 year old patient in whom infection by the sand flea Tunga penetrans was diagnosed by histological examination is described. Clinical findings included small red lesions on the lower extremities, containing a pyodermic component and a central black dot resembling tattoo. A tropical parasitic disease was considered as the signs developed following the patient's return from Tanzania.

Idiopathic plantar hidradenitis.

We describe four cases of idiopathic plantar hidradenitis (IPH), a form of neutrophilic eccrine hidradenitis (NEH) localized on the feet of otherwise healthy young persons. Our patients were all males, 8-17 years old, in apparently good health, but presenting erythematous painful papules, plaques and nodules on the soles. They were afebrile and there were no constitutional symptoms. The differential diagnoses included erythema nodosum, vasculitis and traumatic plantar urticaria. The histological features of IPH are similar to those of NEH, except for the absence of syringosquamous metaplasia.

Onychomycosis associated with Onychocola canadensis: ten case reports and a review of the literature.

BACKGROUND: Onychocola canadensis is a nondermatophyte mold associated with onychomycosis particularly in temperate climates (eg, Canada, New Zealand, and France). The slow growth rate of O canadensis and lack of resemblance to any other known nail-infecting fungus may have delayed its discovery. We are aware of 23 mycologically confirmed cases of O canadensis in the literature. OBJECTIVE: We describe 10 previously unreported Canadian patients, specimens from whom grew O canadensis. We also review the literature on infections associated with this organism. METHODS: Cases of O canadensis onyc

Condyloma latum of the toe webs: an unusual manifestation of secondary syphilis. A report of two cases.

The incidence of primary and secondary syphilis in the United States peaked in 1990, exceeding the number of reported cases since 1948. Uncommon manifestations of secondary syphilis seen more frequently in the preantibiotic era may be less well recognized today. One such manifestation is the condyloma latum, or split papule of the toe web. Two patients with condyloma of the toe web are presented and the previous literature is reviewed. Condyloma latum is a common manifestation of secondary syphilis; however, condyloma latum of the toe webs is unusual. Failure to recognize toe web condyloma lat

[Painful nodular and plantar erythema in children].

INTRODUCTION: According to our knowledge, only twenty-three similar cases have been reported in the literature. Possible hypotheses for localised painful red nodules on the feet in children include erythema nodosum, neutrophilic eccrine hidradenitis, traumatic plantar urticaria, vasculitis and cold panniculitis. CASE REPORT: We report a typical case of painful erythematous plantar nodules of the child. A 13-yr-old boy was first seen by a paediatrician for intermittent fever. The fever was associated with red, painful nodules on the soles. He was unable to walk. Routine blood chemistry paramet

[Pitted keratolysis of hyperkeratotic form and isolation of the etiologic agent: Corynebacterium sp].

Two cases of "pitted keratolysis" with a very accentuated plantar hyperkeratosis, and the isolation on chocolate thelurite agar of the presumptive etiologic agent, Corynebacterium sp., is presented. In order to keep permanently in mind, for a proper diagnosis, the original description of the disease as "keratoma plantare sulcatum" (Castellani, 1910), we are proposing to distinguish two different clinical forms: The hyperkeratotic one and the common or usual form of "pitted keratolysis" with keratolysis as the main sign. The prosecution of our biochemical studies with a significant number of st

Nocardia brasiliensis skin infections.

Nocardia brasiliensis skin infections were diagnosed in three patients (two with mycetoma and one with lymphocutaneous syndrome) in our hospital during the last 6 years. The mycetomas were detected in otherwise healthy Yemenite immigrants who had acquired their infection while in Yemen. The patient with the lymphocutaneous syndrome--a man with well-controlled diabetes mellitus--acquired his illness in Israel. N. brasiliensis appears to be more common in our region than previously believed. This organism should be considered a possible etiologic agent in some cases of chronic unresponsive skin
