Case reports: Painful limbs/moving extremities: report of two cases.

BACKGROUND: Painful limbs/moving extremities is a relatively rare condition characterized by aching pain in one limb and involuntary movemen … ical spondylosis and a defect of the contrast material at the foramen. He was treated with cervical foraminotomy. Patient 2 was a 26-year-old woman with a painful leg and moving toes. The pain and involuntary movement appeared 2 weeks after discectomy at L5/S1. Lumbar MRI and myelography showed no indications of nerve root compression. She was treated with a l …

[Lumbar spinal stenosis syndrome in the experience of a rheumatologic-geriatric clinic].

In the geriatric patient differential diagnosis of low back pain with sciatica includes degenerative lumbar spinal stenosis, affecting the c … s. In our elderly patients lateral stenosis was predominant, and therefore lateral decompression by foraminotomy, generally combined with flavectomy, was the most important surgical procedure. In cases with combined lateral and central stenosis, hemilaminectomy or facetectomy was also performed. In follow-up 1 to 5 years after the operation the success rate, d …

Comparative retrospective study of patients operated for cervical disc herniation and spondylosis.

173 patients suffering from symptoms of cervical disc herniation or spondylosis were surgically treated during 1975 through 1984 by Cloward's method via the anterior route or by total or hemilaminectomy with Epstein's foraminotomy via the posterior route. Of the 41 patients with disc herniation, 37 had radiculopathy as the main disorder and only 4 myelopathy. Of the 132 spondylosis cases radiculopathy was the main disorder in 92 and myelopathy in 40 cases. The operative route was selected according to the site of the osteophytes or disc herniation. The criteria for operative treatment were per…

Exploring Pathways for Pain Relief in Treatment and Management of Lumbar Foraminal Stenosis: A Review of the Literature.

BACKGROUND: Lumbar foraminal stenosis (LFS) involves the narrowing of neural foramina, leading to nerve compression, significant lower back pain and radiculopathy, particularly in the aging population. Management includes physical therapy, medications and potentially invasive surgeries such as foraminotomy. Advances in diagnostic and treatment strategies are essential due to LFS's complexity and prevalence, which underscores the importance of a multidisciplinary approach in optimizing patient outcomes. METHOD: This literature review on LFS employed a systematic methodology to gather and synth…

Novel Cervical Endoscopic Unilateral Laminoforaminotomy for Bilateral Decompression in Cervical Spondylosis Myeloradiculopathy: A Technical Note and Clinical Results.

Background: This study investigates the efficacy of the Cervical Endoscopic Unilateral Laminoforaminotomy for Bilateral Decompression (CE-ULFBD) technique in treating cervical myeloradiculopathy, primarily caused by degenerative spondylosis. Traditionally managed through multisegmental anterior cervical discectomy and fusion (ACDF) or laminoplasty combined with foraminotomy, this condition has recently experienced a promising shift towards minimally invasive approaches, particularly endoscopic spinal decompression. While empirical evidence is still emerging, these techniques show potential for…

Experience of using a 3-blade LES-Tri retractor over 5 years for lumbar decompression microdiscectomy.

BACKGROUND: Lower back pain is the fifth most common reason for visiting a physician in the United States. Degenerative disc disease, degenerative spondylolisthesis, arthritis, and facet arthrosis are leading causes for lumbar spinal stenosis. The previous gold standard involved open laminectomy combined with medial facetectomy and foraminotomy. The advent of minimally invasive surgery (MIS) and endoscopic technologies has led to less invasive and targeted interventions. In this study, the authors aim to show a five-year experience using a three-blade retractor for lumbar decompression and mic…

A novel endoscope-port unit for lumbar microendoscopic surgery: a single-center case series review.

Minimally invasive spinal surgery has shown benefits not only from a clinical standpoint but also in some cost-effectiveness metrics. Microendoscopic procedures combine optical advantages of endoscopy with the preservation of bimanual surgical maneuvers that are not feasible with full percutaneous endoscopic procedures. TELIGEN is a new endoscopic platform designed to optimize these operations. Our aim was to present a retrospective review of surgical data from the first consecutive cases applying this device in our institution and describe some of its technical details. 25 patients have under

Microendoscopic Surgery for Degenerative Disorders of the Cervical and Lumbar Spine: The Influence of the Tubular Workspace on Instrument Angulation, Clinical Outcome, Complications, and Reoperation Rates.

BACKGROUND: Long-term clinical outcomes with microendoscopic spine surgery (MESS) are poorly investigated. The effect of instrument angulation on clinical outcomes has yet to be assessed. METHODS: A total of 229 consecutive patients operated on via two MESS systems were analyzed. Instrument angulation for both MESS systems, which differ from each other regarding the working space for instruments, was assessed using a computer model. Patients' charts and endoscopic video recordings were reviewed to determine clinical outcomes, complications, and revision surgery rates. At a minimum follow-up o

Diabetic amyotrophy coexisting with lumbar disk herniation and stenosis: a case report.

BACKGROUND: Clinical differentiation of diabetic neuropathy from HLD or lumbar stenosis may be difficult. The issue of misdiagnosis has been discussed as a reason for poor outcome after lumbar spine surgery. The authors report a case of diabetic amyotrophy coexisting with, rather than misdiagnosis of, HLD or lumbar stenosis. CASE DESCRIPTION: This 68-year-old female diabetic patient had left sciatic pain for 3 months, and the pain was suddenly aggravated for the last 1 week. She underwent foraminotomy and diskectomy for herniated disk and stenosis at L4-5 level because of failure of conservat…

Lumbar spinal stenosis in elderly patients.

Over a 3-year period in our clinic, surgeons operated on 32 persons over 65 years old with lumbar spinal stenosis. This article presents the retrospective analysis of the clinical, radiological, and short-term surgical outcomes. The stenosis seen most commonly among the elderly develops focally at the intervertebral junctions as a result of a complex process of disc degeneration, facet arthropathy, ligamentum flavum hypertrophy, spondylosis, and sometimes spondylolisthesis. All patients underwent a midline decompressive laminectomy with foraminotomies at the affected levels, and discectomy was
