Surgical Plating vs Closed Reduction for Fractures in the Distal Radius in Older Patients: A Randomized Clinical Trial.

IMPORTANCE: The burden of injury and costs of wrist fractures are substantial. Surgical treatment became popular without strong supporting evidence. OBJECTIVE: To assess whether current surgical treatment for displaced distal radius fractures provided better patient-reported wrist pain and function than nonsurgical treatment in patients 60 years and older. DESIGN, SETTING, AND PARTICIPANTS: In this multicenter randomized clinical trial and parallel observational study, 300 eligible patients were screened from 19 centers in Australia and New Zealand from December 1, 2016, until December 31, 2

Treatment of grossly dislocated supracondylar humerus fractures after failed closed reduction: a retrospective analysis of different surgical approaches.

INTRODUCTION: The supracondylar humerus fracture (SCHF) is one of the most common pediatric injuries. Highly displaced fractures can be very challenging. If closed reduction fails, the therapy algorithm remains controversial. MATERIALS AND METHODS: In total, 41 patients (21 boys and 20 girls) with irreducible Gartland type III SCHF, treated with open reduction through three different approaches and cross-pin fixation, were retrospectively evaluated. The mean follow-up was 46 months (min.: 12, max.: 83, SD: 23.9). The Mayo elbow performance score (MEPS) as well as the quick disabilities of arm

Biomechanical Comparison of a Closed-Loop Double Endobutton to a Lag Screw in Fixation of Posterior Malleolar Fractures.

Anteroposterior (AP) lag screw, posteroanterior (PA) lag screw, or posterior buttress plate are usually performed for posterior malleolar fixation, but the biomechanically strongest technique is unclear. The aim of our study was to biomechanically compare 3 different fixation methods for posterior malleolar fractures; AP lag screw, PA lag screw, and closed-loop double endobutton. Fracture models were created using a thin blade power saw after drawing the fracture line. The resultant fracture involved 30% of the joint on the distal tibial joint surface and extends with an angulation of approxim

The Outcome Of Closed Reduction And Cast Immobilization Of Colle's Fractures Using Radiological Parameters In The Emergency Department.

BACKGROUND: Distal radius fractures account for most of the injuries presenting in trauma centers. Colle's fractures are defined as fractures within 2.5 cm of the wrist with dorsal angulation of the distal fragment. The cornerstone of treatment remains immobilization of the wrist in a cast. The objective of the study was to determine the outcome of closed reduction and plaster cast immobilization of Colle's fractures in adults using Radiological parameters. It was a descriptive case series study, conducted at the Department of Orthopaedics Sheikh Khalifa Bin Zayed Al Nahyan /CMH Muzaffarabad A

Arthroscopy-assisted closed reduction and percutaneous nail fixation of unstable ankle fractures: description of a minimally invasive procedure.

When one is surgically managing an unstable ankle fracture, anatomic reduction of the syndesmosis is typically accomplished using an open surgical approach. We propose an arthroscopically assisted technique that restores normal anatomy while using a percutaneously placed intramedullary nail to fix the fibula. The patient is positioned supine, and the ankle is placed under traction by use of a tensor bandage. Standard anteromedial and anterolateral arthroscopy portals are used. The joint is examined for bony, ligamentous, and chondral injury. Lateral malleolus fracture reduction is accomplished

A new method to correct rotational malalignment for closed reduction and percutaneous pinning in pediatric supracondylar humeral fractures.

Open reduction is often necessary for the treatment of supracondylar humeral fractures in children in which rotational malalignment cannot be corrected by closed means. We introduce a new closed reduction technique for the correction of this deformity using a Kirschner wire as a joystick.

Proposed method for closed reduction of impacted nasal bone fractures using a Kirschner wire and a C-arm.

PURPOSE: Closed reduction is commonly used for the treatment of nasal bone fractures. If the fracture is impacted, accurate reduction is difficult. Therefore open reduction is often required. MATERIALS AND METHODS: Between January 2010 and December 2011, we used a Kirschner wire and a C-arm intraoperatively for accurate closed reduction of impacted nasal bone fractures. RESULTS: During the follow-up period of 4 weeks to 6 months, all patients had satisfactory cosmetic results. CONCLUSIONS: The effectiveness of open reduction was obtained without noticeable scarring with our method of closed

Reamed versus minimally reamed nailing: a prospectively randomised study of 100 patients with closed fractures of the tibia.

It is generally accepted that in tibial fractures the results of reamed intramedullary nailing are better than those of unreamed. However, it is not known whether the clinical effects of reaming are cumulative or if minimal reaming would induce the same beneficial effects as more extensive reaming. This international multicentre study has investigated the effects of different degrees of reaming. 100 patients with closed diaphyseal tibial fractures were prospectively randomised in two centres. Method of treatment was reamed nailing up to 12 mm inserting an 11 mm tibial nail (n: 50), and minimal

[Studies with 99mTc-pyrophosphate of reparative osteogenesis during treatment of closed oblique fractures of the long tubular bones].

Altogether 56 patients aged 20-55 with closed oblique fractures of the upper and lower limb bones were observed. Reparative osteogenesis was studied using 99mTc-pyrophosphate. An elevated RP accumulation at the ends of the bone splinters was found from the 1st day after fracture. By the end of the 1st month after fracture the RP activity increased up to 750% in the absence of splinter dislocation. In dislocation it was 50-80% less. The RP maximal activity in the place of fracture was noted on the 7-8th week, in the dislocation of splinters on the 9-10th week. By the results of radionuclide stu

Suggested method for closed treatment of fractures of the carpal scaphoid: hypothesis supported by dissection and clinical practice.

The anatomy and kinetics of the carpus with special reference to the fractured scaphoid are described. Clinical, radiological and post-mortem studies of the wrist show that the scaphoid can be immobilized and compressed in its long axis, with the wrist held in full supination, mid-dorsiflexion and full ulnar deviation. Immobilization of the upper limb in this position for four weeks has achieved union of those fractures of the scaphoid which are often problematical, i.e. the displaced fracture, fractures of the proximal pole, and those exhibiting delay in union of many months. The method of ma

Preoperative Risk Score for Predicting Delayed Wound Healing After Plate Fixation in Patients With Closed Distal Fibular Fractures.

BACKGROUND: Delayed wound healing after open reduction and internal fixation (ORIF) of distal fibular fractures can impair recovery and increase infection risk. The purpose of this study was to develop a simple preoperative scoring system to predict delayed wound healing after plate fixation in patients with closed distal fibular fractures. METHODS: This retrospective cohort study reviewed 84 patients who underwent plate fixation for closed distal fibular fractures between 2018 and 2024. Delayed wound healing was defined as a surgical wound rated as Grade 2 or higher according to the Southamp

Evaluation of a 3D-Printed Model for Simulating Closed Reduction and Percutaneous Pinning of Unstable Distal Radius Fractures.

INTRODUCTION: We evaluated construct validity of a 3D-printed displaced, unstable distal radius fracture (DRF) model for closed reduction and percutaneous pinning (CRPP). We hypothesized that clinical DRF experience levels would correlate with performance metrics during model CRPP. METHODS: Thirteen volunteers across a spectrum of DRF experience participated, including orthopaedic surgery residents and hand surgery specialists (fellows and attendings). Participants completed a survey assessing confidence and experience with DRF CRPP; watched a video demonstrating model CRPP and evaluation pro

Eccentric Hexapod external fixator assisted closed reduction and intramedullary nail fixation for the treatment of femoral shaft fractures.

OBJECTIVE: Closed reduction and intramedullary nail (IMN) fixation remains the gold-standard treatment for femoral shaft fractures (FSFs). However, technical challenges persist in achieving and maintaining anatomical alignment during the closed reduction process. This study aimed to evaluate the clinical efficacy of Hexapod external fixator (HEF)-assisted closed reduction and IMN fixation in the management of FSFs. METHODS: A retrospective cohort study was conducted on 25 patients undergoing closed reduction and IMN fixation with eccentric HEF assistance between June 2021 and January 2023. Op
