Early-Onset Myopathies: Clinical Findings, Prevalence of Subgroups and Diagnostic Approach in a Single Neuromuscular Referral Center in Germany.

BACKGROUND: Early-onset myopathies are a heterogeneous group of neuromuscular diseases with broad clinical, genetic and histopathological overlap. The diagnostic approach has considerably changed since high throughput genetic methods (next generation sequencing, NGS) became available. OBJECTIVE: We present diagnostic subgroups in a single neuromuscular referral center and describe an algorithm for the diagnostic work-up. METHODS: The diagnostic approach of 98 index patients was retrospectively analysed. In 56 cases targeted sequencing of a known gene was performed, in 44 patients NGS was per

The complexity of pain in osteoarthritis.

Chronic pain is the hall-mark symptom of osteoarthritis (OA) and although several therapies are available, a sizeable number of patients do not gain adequate pain relief from these therapies. Predicting those patients who will not respond to current therapies remains a challenge. Although psychosocial, sensitivity, inflammation and genetic factors have been identified as pain mechanisms that predict response to pain therapy, none of these is a sufficiently strong predictor alone. An emerging approach to this challenge is the use of machine-learning algorithms that integrate several pain mechan

Surgery scheduling based on large language models.

Large Language Models (LLMs) have shown remarkable potential in various fields. This study explores their application in solving multi-objective combinatorial optimization problems-surgery scheduling problem. Traditional multi-objective optimization algorithms, such as the Non-dominated Sorting Genetic Algorithm II (NSGA-II), often require domain expertise for designing precise operators. Here, we propose LLM-NSGA, where LLMs act as evolutionary optimizers, performing selection, crossover, and mutation operations. Results show that for 40 cases, LLMs can independently generate high-quality sol

An Improved Deep Semi-supervised JNMF Method for Biomarker Extraction of Alzheimer's Disease.

Imaging genetics is an approach that explores the underlying mechanisms of brain disorders such as Alzheimer's disease (AD) by analyzing the correlation between neuroimaging and genetic data. Traditional non-negative matrix factorization (NMF) algorithms are based on linear assumptions, which limits the potential of nonlinear feature extraction among multi-omics data. This study proposes a novel joint-connectivity-based deep semi-supervised non-negative matrix factorization (JCB-DSNMF) model to overcome this limitation and incorporate prior knowledge from both within and between different moda

Treatment of bipolar disorder: new perspectives.

Treatment of bipolar disorder (BD) has traditionally focused on alleviation of acute symptoms and prevention of future recurrences. Current treatment guide-lines advocate more or less similar treatment algorithms for all patients. Such approach largely ignores the clinical, genetic, and pathophysiological heterogeneity of BD, which makes certain patients more (or less) likely to respond to specific treatments. Variables such as family history, comorbidity, course of illness, quality and duration of previous remissions, physical and medical comorbidity, and side-effects may help in selecting th

Diagnosis of rare dementia syndromes: an algorithmic approach.

The etiology of dementia can be diagnosed in most patients using a standard clinical approach consisting of physical, neurologic, and mental status examinations, and laboratory testing, lumbar puncture, and neuroimaging. In some cases, however, the clinical presentation or historical data are unusual, or the results of the workup are inconclusive or atypical. A rare cause of dementia may then be present and a complicated evaluation may be necessary to identify the specific disease process. A potentially useful approach to the diagnosis of rare dementing disorders consists of a series of diagno

Study on optimization algorithm of tuned mass damper parameters to reduce vehicle-bridge coupled vibration.

A vehicle-bridge tuned mass damper (TMD) coupled dynamic analysis and vibration-control model was established to optimize TMD damping effects on a steel-box girder bridge bearing vehicle loads. It was also used to investigate optimization efficiency of different algorithms in TMD design parameters. This model simulated bridges and vehicles with the use of a 7 degrees of freedom curved-beam element model and a 7 degrees of freedom vehicle model, respectively. The TMD system was simulated with the use of multiple rigid-body systems linked with springs and dampers. Road surface condition, as a vi

Exploring potential gene signatures in dengue through machine learning and deep learning approaches.

Dengue is a major public health problem that affects millions of people globally. The present study used microarray data to identify differentially expressed genes (DEGs) during dengue clinical conditions. The microarray datasets GSE84331, GSE18090, GSE43777, and E-MTAB-3162 were downloaded and analyzed using statistical analysis (Unpaired t-test). This was followed by Machine Learning (ML) and Deep Learning (DL) techniques with recursive feature elimination and genetic algorithms implemented to identify the potential biomarkers. Further, functional enrichment, platelet signaling, and protein-…

Chromosome Image Classification Using Edge Fusion Attention Network.

Identification of chromosome pairs is important for karyotype generation and genetic disease prediction. Conventional methods tend to fail with structural variations of the chromosomes and imprecise boundaries. To solve these issues, we propose a deep learning architecture, Edge Fusion Attention Network (EFANet), for chromosome classification. It is a new architecture that combines the Adaptive Edge Preserve Fusion (AEPF) algorithm with the Feature Focused Attention Network (F2ANet). The AEPF algorithm clearly identifies chromosome boundaries and highlights the morphological differences. It im

Pathway-Based Kernel Boosting for the Analysis of Genome-Wide Association Studies.

The analysis of genome-wide association studies (GWAS) benefits from the investigation of biologically meaningful gene sets, such as gene-interaction networks (pathways). We propose an extension to a successful kernel-based pathway analysis approach by integrating kernel functions into a powerful algorithmic framework for variable selection, to enable investigation of multiple pathways simultaneously. We employ genetic similarity kernels from the logistic kernel machine test (LKMT) as base-learners in a boosting algorithm. A model to explain case-control status is created iteratively by select

Optimal adaptive heuristic algorithm based energy optimization with flexible loads using demand response in smart grid.

Demand response-based load scheduling in smart power grids is currently one of the most important topics in energy optimization. There are several benefits to utility companies and their customers from this strategy. The main goal of this work is to employ a load scheduling controller (LSC) to model and solve the scheduling issue for household appliances. The LSC offers a solution to the primary problems faced during implementing demand response. The goal is to minimize peak-to-average demand ratios (PADR) and electricity bills while preserving customer satisfaction. Time-varying pricing, inte

What does not look like celiac disease and instead it is.

The celiac disease (CD) diagnosis sometimes is challenging and diagnostic process cannot always follow a simple algorithm but it requires a close collaboration between histo-pathologists, clinicians, laboratory and genetic experts. The genetic predisposition for CD is related to HLA-DQ2 and/or DQ8 but other HLA haplotypes and non-HLA genes may be involved in genetic predisposition. In particular DQ7 may represent an additive and independent CD risk associated haplotype. We describe an unusual case of a female 42 year old with a previous diagnosis of Hodgkin lymphoma, who has a clinical present

An Efficient Multi-Dimensional Resource Allocation Mechanism for Beam-Hopping in LEO Satellite Network.

Low Earth Orbit (LEO) satellite communication networks have become an important means to provide internet access services for areas with limited infrastructure. Compared with the Geostationary Earth Orbit (GEO) satellites, the LEO satellites have limited on-board communication caching and calculating resources. Furthermore, the distribution of traffic requests is dynamically changing and uneven due to the relative movement between the LEO satellites and the ground. Therefore, how to schedule the multi-dimensional resources is an important issue for the LEO satellite communication networks. Bea

FreeSurfer-based segmentation of hippocampal subfields: A review of methods and applications, with a novel quality control procedure for ENIGMA studies and other collaborative efforts.

Structural hippocampal abnormalities are common in many neurological and psychiatric disorders, and variation in hippocampal measures is related to cognitive performance and other complex phenotypes such as stress sensitivity. Hippocampal subregions are increasingly studied, as automated algorithms have become available for mapping and volume quantification. In the context of the Enhancing Neuro Imaging Genetics through Meta Analysis Consortium, several Disease Working Groups are using the FreeSurfer software to analyze hippocampal subregion (subfield) volumes in patients with neurological and

Unsupervised class labeling of diffuse lung diseases using frequent attribute patterns.

PURPOSE: For realizing computer-aided diagnosis (CAD) of computed tomography (CT) images, many pattern recognition methods have been applied to automatic classification of normal and abnormal opacities; however, for the learning of accurate classifier, a large number of images with correct labels are necessary. It is a very time-consuming and impractical task for radiologists to give correct labels for a large number of CT images. In this paper, to solve the above problem and realize an unsupervised class labeling mechanism without using correct labels, a new clustering algorithm for diffuse l

Primary Hyperparathyroidism in Adults: Recent Developments in Diagnosis and Management.

Primary hyperparathyroidism has a prevalence of around 1% in the general population. Diagnosis requires biochemical testing of serum (for calcium and parathyroid hormone) and urine (for calcium excretion), as well as wider screening for complications including osteoporosis, renal disease, abdominal symptoms and neuropsychiatric disturbance. In selected individuals, genetic testing is a key consideration because up to 10% have hereditary primary hyperparathyroidism. Surgery is curative, with a minimally invasive approach preferred after identifying the abnormal parathyroid glands on preoperativ

The implications of revised WHO classification (2008) of chronic myeloid neoplasms.

Myeloid disorders constitute a subgroup of hematological malignancies of myeloid lineage. In the revised 2008 WHO classification system, the nomenclature for myeloproliferative entities has been changed from "chronic myeloproliferative diseases" to "myeloproliferative neoplasms", the definition criteria has changed, and and the MDS/MPN has emerged as a well defined entity the subgroup formerly known as "myelodysplastic/myeloproliferative diseases" has changed to "myelodysplastic/myeloproliferative neoplasms".
