A novel nutritional index "simplified CONUT" and the disease risk index independently stratify prognosis of elderly patients with acute myeloid leukemia.

Elderly patients aged 65 or older with acute myeloid leukemia (AML) have poor prognosis. The risk stratification based on genetic alteration has been proposed in national comprehensive cancer network (NCCN) guideline but its efficacy was not well verified especially in real world elderly patients. The nutritional status assessment using controlling nutritional status (CONUT) score is a prognostic biomarker in elderly patients with solid tumors but was not examined in elderly AML patients. We performed prospective analysis of genetic alterations of 174 patients aged 65 or older with newly diagn

Characterization of Additive Gene-environment Interactions For Colorectal Cancer Risk.

BACKGROUND: Colorectal cancer (CRC) is a common, fatal cancer. Identifying subgroups who may benefit more from intervention is of critical public health importance. Previous studies have assessed multiplicative interaction between genetic risk scores and environmental factors, but few have assessed additive interaction, the relevant public health measure. METHODS: Using resources from CRC consortia, including 45,247 CRC cases and 52,671 controls, we assessed multiplicative and additive interaction (relative excess risk due to interaction, RERI) using logistic regression between 13 harmonized …

Genetic sonogram scoring index: accuracy and clinical utility.

We sought to evaluate the accuracy of the genetic sonogram scoring index in detecting Down syndrome fetuses in a high-risk population. Women referred for genetic sonogram scoring index based on increased risk for aneuploidy were prospectively evaluated. Each fetus was assigned a score based on our previously published genetic sonogram scoring index. A score of 1 or greater was an indication for karyotype determination. Of 1076 patients (1118 fetuses) in the study group, follow-up data were available for 1030 (92%), eight of whom had Down syndrome (prevalence 0.8%). Three fetuses with Down synd

The genetic architecture of postoperative delirium after major surgery and its relationship with nonpostoperative neurocognitive conditions: A genome-wide association study.

BACKGROUND: Postoperative delirium is the most common postoperative complication in older individuals. Genome-wide association studies (GWAS) can provide insights into how genetic factors influence postoperative risk. We examined the genetic architecture of postoperative delirium after major surgery and its relationship with related cognitive conditions (delirium of any type and Alzheimer's disease, including the APOE ε4 allele). METHODS AND FINDINGS: A case-control GWAS was performed in the UK Biobank to identify genetic variants associated with postoperative delirium, adjusted for age, sex,

Using Genetics to Inform Interventions Related to Sodium and Potassium in Hypertension.

BACKGROUND: Hypertension is a key risk factor for major adverse cardiovascular events but remains difficult to treat in many individuals. Dietary interventions are an effective approach to lower blood pressure (BP) but are not equally effective across all individuals. BP is heritable, and genetics may be a useful tool to overcome treatment response heterogeneity. We investigated whether the genetics of BP could be used to identify individuals with hypertension who may receive a particular benefit from lowering sodium intake and boosting potassium levels. METHODS: In this observational genetic

White matter microstructure is associated with hyperactive/inattentive symptomatology and polygenic risk for attention-deficit/hyperactivity disorder in a population-based sample of adolescents.

Few studies have investigated the link between putative biomarkers of attention-deficit/hyperactivity disorder (ADHD) symptomatology and genetic risk for ADHD. To address this, we investigate the degree to which ADHD symptomatology is associated with white matter microstructure and cerebral cortical thickness in a large population-based sample of adolescents. Critically, we then test the extent to which multimodal correlates of ADHD symptomatology are related to ADHD polygenic risk score (PRS). Neuroimaging, genetic, and behavioral data were obtained from the IMAGEN study. A dimensional ADHD c

Genomic Prostate Score (GPS) as a Prognostic Biomarker in Patients With Localized Prostate Cancer Undergoing Focal Therapy.

BACKGROUND: Focal therapy (FT) encompasses ablation of the prostate cancer (PCa) while preserving the surrounding benign tissue, aiming to minimize or spare patients from postoperative complications. Although FT has been proven effective, the multifocal and heterogeneous nature of PCa is a significant concern regarding the risk of disease progression. The Genomic Prostate Score (GPS) has been shown to aid in the risk stratification of patients with localized disease; however, its role in patients undergoing FT remains unclear. We evaluated the association between the GPS performed on pretreatm

Development of a Polygenic Risk Score to Predict Diverticulitis.

BACKGROUND: Despite its prevalence and associated morbidity, we remain limited in our ability to predict the course of a patient with diverticular disease. Although several clinical and genetic risk factors have been identified, we do not know how these factors relate to one another. OBJECTIVE: Our aim was to determine whether a polygenic risk score could improve risk prediction for diverticulitis and recurrent diverticulitis compared with a model using only clinical factors. DESIGN: This is an observational study. SETTING: The study examines the predictive ability of a polygenic risk score

Multi-Trait GWAS Analyses of Blood Pressure Traits Provide Limited Gains in Locus Discovery or Polygenic Risk Score Performance Compared to Single-Trait GWAS.

High blood pressure (BP) represents a major cause of life-threatening cardiovascular conditions. GWAS have identified >2000 BP-associated signals. BP-GWAS usually analyze each BP trait individually. However, multi-trait GWAS may yield novel loci and improve polygenic risk scores (PRS) prediction. We explored whether multi-trait BP-GWAS can: (i) find novel loci undetected by single-trait GWAS; (ii) increase heritability estimates; (iii) improve PRS prediction of hypertension within the Lifelines Cohort Study. We investigate the utility of multi-trait GWAS approaches of correlated BP traits (sys

Challenges to case-only analysis for interaction detection using polygenic risk scores: model assumptions and biases in large biobanks.

Understanding gene-environment and gene-gene interactions is important for studying complex diseases. Case-only analysis has been proposed to improve power for detecting interactions. However, case-only analysis relies on key assumptions, including correct specification of the disease risk model and marginal independence between variables. In this study, we systematically investigate the challenges of case-only analysis using polygenic risk scores (PRS) as genetic variables in large biobanks. Through simulations, we demonstrate that the false positive control of PRS-based case-only analysis de

Prevalence and predictors of low bone mineral density in pediatric inflammatory bowel disease.

OBJECTIVES: Bone health is at risk in children with inflammatory bowel disease (IBD). This study examined the prevalence and predictors of low bone mineral density (BMD) in a cohort of children and young adults with IBD. METHODS: This single-center retrospective study included patients with IBD, ages 3.5-22 years, with completed dual x-ray absorptiometry (DXA) scans from 2006 to 2019. Demographic, clinical, and laboratory data were collected. Logistic regression analysis identified predictors associated with low BMD (Z-scores ≤ -2 standard deviations [SDs]) for three outcomes. A multiancestry polygenic risk score improves stratification in patients with hypertrophic cardiomyopathy.

Hypertrophic cardiomyopathy (HCM) has traditionally been considered a Mendelian disease driven by pathogenic or likely pathogenic variants in sarcomere-encoding genes (SARC-HCM-P/LP). However, these variants explain only one-third of cases, and variable penetrance suggests additional polygenic contributions. Existing HCM polygenic risk scores (PRSs), largely derived from European-ancestry cohorts, have limited generalizability. Here we develop a multiancestry PRS using summary statistics from the BioBank Japan, Million Veteran Program and a meta-analysis of seven European-ancestry cohorts and

Linking polygenic risk scores to dopaminergic neuron loss using neuromelanin-sensitive imaging.

Parkinson's disease is characterized by the degeneration of dopaminergic neurons in the substantia nigra pars compacta (SNc), and neuromelanin-sensitive MRI provides a biomarker to track this neuronal loss. Isolated REM sleep behaviour disorder (iRBD), associated with cognitive decline, may represent a distinct subtype of synucleinopathy. Polygenic risk scores for these conditions may be associated with neuronal degeneration. This study investigates whether genetic risk scores for Parkinson's disease (PGS000903) or iRBD (PGS003414) are associated with neuromelanin signal loss in the SNc in the…

Performance of routine risk scores for predicting cirrhosis-related morbidity in the community.

BACKGROUND & AIMS: Models predicting an individual's 10-year risk of cirrhosis complications have not been developed for a community setting. Our objectives were to assess the performance of existing risk scores - both with and without genetic data - for predicting cirrhosis complications in the community. METHODS: We used a 2-stage study design. In stage 1, a systematic review was conducted to identify risk scores derived from routine liver blood tests that have demonstrated prior ability to predict cirrhosis-related complication events. Risk scores identified from stage 1 were tested in a U

Leveraging polygenic risk scores to infer causal directions in genotype-by-environment interactions between complex traits.

Most existing genotype-by-environment interaction (G×E) methods assume a known causal direction as an assumption that often does not hold and can lead to biased estimates and spurious findings. To address this, we introduce the Genetic Causality Inference Model (GCIM), a novel approach designed to infer causal directions in G×E studies. GCIM integrates polygenic risk scores (PRS) for both the exposure and the outcome to strengthen causal inference and reduce spurious interaction signals. We evaluated GCIM using simulated data across varying genetic and residual correlation settings and compare
