Nurses' Perceptions of Factors Influencing Elder Self-neglect: A Qualitative Study.

PURPOSE: Elder self-neglect is a global public health issue and should be taken seriously at large. Nurses, usually working directly with elderly patients, have a better understanding of what factors may cause elder self-neglect. In this qualitative study, we explored the influencing factors of elder self-neglect from the perception of nurses in the context of Chinese culture. METHODS: Face-to-face, in-depth interviews were conducted from November 2018 to December 2018. Purposive sampling was used. Twenty one participants recruited from eight geriatric wards of a general hospital located in W

[Risk factors for mortality in geriatric hospitalized patients with respiratory syncytial virus: the FAMHous study].

The real burden of respiratory syncytial virus (RSV) in older people is probably underestimated, in spite of improvements in virological diagnosis and recent preventive and therapeutic strategies. The aim of this study is to assess in-hospital mortality risk factors in geriatric patients. This is a retrospective multicenter study. We included all patients hospitalized in geriatric wards with a positive nasopharyngeal rt-PCR for RSV during the winter seasons between November 2018 and April 2023 from three Parisian hospitals. We used a chi-square test for the ordinary variables and a Wilcoxon te

Does a bicycle accident as the cause of proximal femur fracture indicate that geriatric co-management is superfluous? A retrospective cohort study.

BACKGROUND: Deployment of geriatric care would be more sustainable if we could limit geriatric co-management to older hip fracture patients who benefit most from it. We assumed that riding a bicycle is a proxy of good health and hypothesized that older patients with a hip fracture due to a bicycle accident have a more favorable prognosis than patients whose hip fracture was caused by another type of accident. METHODS: Retrospective cohort study of hip fracture patients ≥ 70 years admitted to hospital. Nursing home residents were excluded. Primary outcome was length of hospital stay (LOS). Sec

[Antihypertensive drug treatment and renal function in geriatric emergency patients].

OBJECTIVE: The aim of this study was to assess drug prescriptions and renal function in a cohort of geriatric emergency patients with a focus on antihypertensive drug treatment. PATIENTS AND METHODS: The one-year observational study was conducted among patients from nursing and retirement homes or outpatient care who were treated by the emergency medical service. Overall, 109 patients (85 women, mean age 85+/-8 years) were studied. Glomerular filtration rate was estimated (eGFR) on the basis of creatinine and cystatin C serum concentrations. RESULTS: The most common emergency admission diagn

Management of falls in older persons: a prescription for prevention.

Although falls are a common cause of injury in older persons, they are not just a normal part of the aging process. The American Geriatrics Society and British Geriatrics Society recommend that all adults older than 65 years be screened annually for a history of falls or balance impairment. An individualized risk assessment should be performed, with corresponding multifactorial intervention, for those who report a single fall and have unsteadiness; who report two or more falls; who report difficulties with gait or balance; or who seek medical attention because of a fall. The following componen

VA Choose Home Program: An Intensive Geriatric Home-Based Model of Care.

BACKGROUND: The population of Veterans aged 85+ eligible for VA-financed long-term care will increase 500% over the next decade. Our team recognized a gap in care for older Veterans at risk of institutionalization between our acute and longitudinal home and community-based programs. This paper describes Choose Home, an innovative interprofessional home-based model of care developed to prevent avoidable acute care and long-term institutionalization. METHODS: Multicomplex Veterans aged ≥ 65 at imminent risk of nursing home placement were eligible for program enrollment. The interdisciplinary te

Accuracy and Reliability of Intraoral 3D Scans for Diagnostic Evaluations in Nursing Home Residents.

OBJECTIVES: To evaluate the accuracy and reliability of intraoral 3D scans for assessing the oral health of older patients living in nursing homes. MATERIALS AND METHODS: One examiner recorded missing teeth, restorations, caries lesions and oral hygiene (Geriatric-Debris-Index Simplified [GDI-S] ≥ 1.9) in nursing home residents using visual-tactile diagnostics (reference test) and afterwards obtained intraoral scans with an intraoral scanner (TRIOS 4, 3Shape). Two other independent investigators and the clinical examiner assessed only the scans to delineate the same diagnostic outcomes. For t

Should frail older adults be in long-term care facilities?

AIM: Home-based nursing care is relatively easy to access in Norway compared to the rest of Europe, and the threshold for applying for assistance is relatively low. The aim of the present study was to analyze factors that enable frail older adults to live in their own homes, with a low level of care burden stress. METHODOLOGY: In 2015 and 2016, eight municipalities from different parts of Norway participated in a cross-sectional study. The quantitative part of the project consisted of assessing care of 71 older adults, aged ≥80 years, using a geriatric comprehensive assessment. The qualitativ

Association between longitudinal clinical outcomes in patients with hip fracture and their pre-fracture place of residence.

BACKGROUND: Clinical outcomes of patients with proximal femoral fracture within 1 year after hospitalization are presented. In particular, associations between the patients' clinical status and their pre-fracture residence were evaluated (community-dwelling vs nursing home). METHODS: Patients aged ≥60 years with proximal femoral fractures were included in a prospective, single-centre observational study and followed for 12 months. Patients' clinical status at baseline was compared to their health status at follow-up 12 months later. Several standardized questionnaires were used to evaluate th

Risk factors associated with dehydration in older people living in nursing homes: Scoping review.

BACKGROUND: Dehydration in the older people is a prevalent problem that is often associated with physiological changes, physical limitations and environmental conditions. OBJECTIVES: The scoping review was carried out to identify risk factors associated with dehydration in older people living in nursing homes. DESIGN: The revised scoping methodology framework of Arksey and O'Malley (2005) was applied. Study selection was carried out in accordance with Davis et al. (2009) and focused on the inclusion criteria (people over 65 years old and living in nursing homes). Risk factors were classified

Pattern of prescriptions issued by nursing home-based physicians versus office-based physicians for frail elderly patients in German nursing homes.

PURPOSE: The purpose of this study was to describe the physician prescription pattern for frail elderly patients in German nursing homes and to identify differences, if any, between that of physicians based in the nursing homes (A) and those with office-based practices (B). METHODS: Retrospective, longitudinal study of medication prescriptions on the basis of the database of a health insurance (Betriebskrankenkasse) in Berlin, Germany. We assessed the medication prescriptions for all 996 unselected insured individuals aged > or = 60 years who were insured throughout 1999 and lived in nursing

Hospitalization Experiences Among Nursing Home Residents With Dementia.

OBJECTIVES: Hospital admissions among nursing home residents with Alzheimer's disease and related dementias (ADRD) are burdensome, expensive, and provide limited clinical benefit. Compared with other patients, those with ADRD are more likely to experience adverse events when hospitalized. Our objective was to comprehensively describe the in-hospital experience of nursing home patients with ADRD. DESIGN: We analyzed 1 randomly selected hospitalization per patient from the electronic health record. SETTING AND PARTICIPANTS: Adult patients with an ADRD diagnosis from a single health system in O

Impact of a Geriatric Co-management Service on Surgical Cancer Care for Older Adults.

In 2011, Brown University Health's Division of Geriatrics started its inaugural geriatric co-management service to improve older adult (OA) care for patients with hip fractures. Its success in care and cost reduction prompted the health system to expand geriatric co-management. Meanwhile, oncology surgeons treat increasingly older and frailer patients due to improved therapies and survival. The literature established what we saw here: increased mortality and complications in frail OA and failure of strategies like Enhanced Recovery After Surgery and Prehabilitation. In 2021, the division launc

A randomized trial of in-home visits for disability prevention in community-dwelling older people at low and high risk for nursing home admission.

BACKGROUND: In-home preventive visits with multidimensional geriatric assessments can delay the onset of disabilities in older people. METHODS: This was a stratified randomized trial. There were 791 participants, community-dwelling people in Bern, Switzerland, older than 75 years. The participants' risk status was based on 6 baseline predictors of functional deterioration. The intervention consisted of annual multidimensional assessments and quarterly follow-up in-home visits by 3 public health nurses (nurses A, B, and C), who, in collaboration with geriatricians, evaluated problems, gave rec

[Urinary incontinence of the older adult].

The frequency and severity of urinary incontinence (UI) increase with age. Its prevalence varies according to living environment: it is less common among older people living at home than in hospitals or nursing homes. UI is a geriatric syndrome with multiple causes (comorbidities, polypharmacy, etc.) and harmful consequences: reduced quality of life for people and their careers, depression, falls, institutionalization. Diagnosis of UI is based on history-taking and a basic medical check-up, with the aim of ruling out red flags and the causes of transient UI, pointing to an etiology and determi

[Can falls in nursing homes be avoided?].

Fall-related injuries in nursing homes for the aged are frequent and cost-intensive. Their iatrogenic causes are well known and avoided in modern geriatric institutions. We examined all falls in an institution during the period of 19 months. Patients who carry out daily tasks on a semi-independent level were most at risk. The main risk-factor was dementia. Medication did not prove a major risk-factor. At 37% p. a. the risk of falling was lower than in previous studies. This was due to a restriction on benzodiazepines with short term effects, minimising of neuroleptics, diuretics and other type

Implementation of frailty screening for older people living with HIV in Brighton, UK.

OBJECTIVES: To evaluate the implementation of frailty screening in people living with HIV (PLWH) in a large urban cohort of patients in Brighton, UK. METHODS: Focus group discussions with HIV professionals and PLWH interviews helped inform the design and implementation of the frailty screening pathway in the clinic. Data were collected from PLWH aged over 60 years attending their HIV annual health check from July 2021 to January 2023 (n = 590), who were screened for frailty by nurses using the FRAIL scale. We assessed the proportions of PLWH who screened as frail, prefrail or robust and compa
