Propofol in the management of refractory status epilepticus: a case report.

Status epilepticus is one of the most frequent neurological emergencies in the intensive care unit. Standard treatment includes intravenous barbiturates, benzodiazepines and phenytoin. However, drug coma is sometimes necessary to control refractory status epilepticus. We report such a case, successfully treated by intravenous propofol coma to EEG burst suppression.

The clinical outcome of patients with severe head injuries, treated with highdose dexamethasone, hyperventilation and barbiturates.

The outcome in 45 consecutive patients following severe head injuries is presented. All patients were evaluated according to the Glasgow Coma Scale and the Glasgow Outcome Scale. All patients received a combined treatment consisting of dexamethasone, barbiturate and hyperventilation. A primary operative intervention was performed in 14 patients with intracranial haematomas. There was a good recovery in 53%, 20% were moderately disabled, 11% were severely disabled. 16% died or remained in a vegetative state. The patients were aged between 5 and 83 years, 49% were under 20 years, and the relatio…

Neurological and mental outcome after severe head injury in childhood: a long-term follow-up of 318 children.

PURPOSE: Identification of prognostic risk factors in paediatric head injury. METHOD: Long-term follow-up of 318 children with severe head injuries with a mean follow-up period of 8 years and 9 months. The neurological and mental outcome was classified by the Glasgow Outcome Scale and the Frankfurt Mental Outcome Scale. RESULTS: Prognostic risk factors were a primary post-traumatic vigilance disturbance longer than 24 hours, less than seven points on the Glasgow Coma Scale, an increased intracranial pressure with cerebral perfusion pressure below 50 mmHg, age at accident younger than 2 years…

The Effect of Antiseizure Medication Administration on Mortality and Early Posttraumatic Seizures in Critically Ill Older Adults with Traumatic Brain Injury.

BACKGROUND: Traumatic brain injury (TBI) is a major cause of morbidity and mortality in the United States. Older adults represent an understudied and growing TBI population. Current Brain Trauma Foundation guidelines support prophylactic antiseizure medication (ASM) administration to reduce the risk of early posttraumatic seizures (within 7 days of injury) in patients with severe TBI. Whether ASM decreases mortality or early seizure risk in this population remains unclear. This study addresses the knowledge gap regarding the impact of ASM administration on the risk of seizure or mortality afte

Frequency of physical and occupational therapy in the ICU for patients with cognitive motor dissociation: a retrospective cohort study.

BACKGROUND: Supportive care is the primary focus of ICU management for unconscious patients early after severe acute brain injury. While benefits of early physical and occupational therapy are generally recognized, it remains uncertain how early therapy can begin and what patients most benefit form intensive, early therapy. Brain activation to motor commands detected by EEG also known as cognitive motor dissociation (CMD) is a predictor of recovery seen in up to 25% of patients. The purpose of this study was to determine if CMD patients that undergo early, intense physical and occupational the

COMPUTED TOMOGRAPHY SCORING SCALES AS EARLY OUTCOME PREDICTORS IN PATIENTS WITH TRAUMATIC BRAIN INJURY: WHICH ONE TO USE?

Computed tomography (CT) is an essential tool in diagnosing and treating traumatic brain injury (TBI). Marshall CT classification, Rotterdam and Helsinki CT scores were consecutively developed as prediction outcome scales by computing TBI CT abnormalities. None of them classifies the pathological CT findings in the same manner. We aimed to determine which one is most accurate and has the best grading discriminatory power in determining early outcome. All TBI patients treated at a single center in a one-year period having undergone a CT scan on admission were retrospectively included. After cal

Tranexamic Acid in Pediatric Traumatic Brain Injury: A Multicenter Retrospective Observational Study.

STUDY OBJECTIVE: Tranexamic acid (TXA) can be used after trauma to prevent bleeding. Our goal was to examine the influence of TXA on morbidity and mortality for children with severe traumatic brain injury (TBI). METHODS: We identified children aged <18 years with a severe TBI (Glasgow Coma Scale score less than 8) presenting to 1 of the 291 hospitals contributing to the Japanese Trauma Data Bank between 2019 and 2023. The primary outcome was inhospital death, and the secondary outcome was poor neurologic outcome defined with Glasgow Outcome Scale (GOS) score of 1 to 3 at hospital discharge. O…

Development of chronic hydrocephalus and early cranial CT findings in spontaneous intracerebral/intraventricular hemorrhage.

INTRODUCTION: Since intracerebral hemorrhage (ICH) is frequently associated with intraventricular bleeding (IVH), we sought to detect cranial computed tomography features that would indicate early on in the patient's history the development of chronic hydrocephalus with a permanent need for cerebrospinal fluid (CSF) diversion. METHODS AND MATERIALS: A total of 25 consecutive cases presenting with ICH/IVH was studied retrospectively. Outcome was assessed using the Glasgow Outcome Scale at an average of 8 months subsequently. Diagnosis of ICH/IVH and (acute) hydrocephalus was made by early cran

The natural history of post-traumatic neurohypophysial dysfunction.

BACKGROUND AND OBJECTIVES: Posterior pituitary function remains poorly investigated after traumatic brain injury (TBI). DESIGN AND METHODS: 50 consecutive patients with severe or moderate TBI (initial Glasgow Coma Scale (GCS) score 3/15-13/15) were prospectively studied on three occasions: at the acute phase and at 6 months and at 12 months following TBI. The Effect of Environmental Hypothermia on Survival in Isolated Blunt Traumatic Brain Injury.

BACKGROUND: Environmental hypothermia increases mortality in patients with major trauma; however, the impact of exposure hypothermia on outcomes in isolated traumatic brain injury (TBI) is underexplored in literature. The aim of this study is to determine the relationship between environmental hypothermia and survival in patients with isolated blunt TBI. METHODS: We analyzed data from the Trauma Quality Improvement Program database. We included patients who were aged ≥15 years, had an abbreviated injury scale ≥1 for the head/neck body region, an arrival Glasgow Coma Scale of <14, an abbreviat…

[Prognostic factors of post-traumatic vegetative states: 522 cases].

Long-term prognosis of post-traumatic vegetative state (VS) remains poorly defined. Three kinds of data have been collected for 522 patients in VS 1 month after head injury: pre-traumatic data; early data, collected during the first two weeks after trauma; late data, collected one month or more after trauma. Statistical relationships have been searched for, between these data and outcome one year after trauma, according to the Glasgow Outcome Scale. Some late data appear as essential prognostic factors of VS: threat blink; complications; ventricular dilatation; motor score (Glasgow Coma Score)

The effect of high dose barbiturate decompression after severe head injury. A controlled clinical trial.

Treatment resistant intracranial hypertension after severe head injury has a very high mortality with conventional therapy such as hyperventilation and mannitol infusions. In this report, we describe the use of large doses of thiopental as a means of treating such swelling. From a consecutive series of 107 severe head injuries with a Glasgow Coma Score (GCS) of 6 or below, we selected all patients below 40 years age with a progressive increase in intracranial pressure (ICP) to 40 mm Hg. The first 16 patients (mean age 20 years, mean GCS 4.3) were treated with deep barbiturate coma and hypother
