Effect of Intracuff Pressure Adjustment on Postoperative Sore Throat, Hoarseness, and Oral Comfort in Robotic Gynecological Surgery: A Quasi-experimental Study.

PURPOSE: Hoarseness and postoperative sore throat (POST) are common complications of endotracheal intubation during robotic-assisted gynecological surgery. Despite the increasing prevalence of robotic procedures, studies focusing on airway symptom management in this context are limited. This study aimed to evaluate the effects of intraoperative endotracheal tube (ETT) cuff pressure adjustment on hoarseness, POST, and oral comfort. Additionally, the study sought to inform nursing interventions to enhance postoperative care. DESIGN: This study employed an assessor-blinded, quasi-experimental de

Balloon vaginoplasty as a minimally invasive method in the management of vaginal aplasia.

OBJECTIVE: The aim of this article is to describe the management of vaginal aplasia and to introduce minimally invasive surgical procedures for neovagina formation. METHODOLOGY: Literature review obtained from studies and papers dealing with the management of congenital vaginal aplasia. CONCLUSION: Vaginal aplasia is a rare congenital anomaly, often in coincidence with congenital defects of the uropoietic system. Management nowadays favors non-surgical or minimally invasive surgical methods for neovagina formation. Saman et al introduced a new method of neovagina formation, namely balloon va

Laparoscopic splenectomy for secondary cytoreduction of ovarian cancer in a woman with localized splenic recurrence.

OBJECTIVE/BACKGROUND: Laparoscopic splenectomy is a potential surgical option for the treatment of isolated gynecologic cancer recurrence to the spleen [1-7]. The purpose of this video is to demonstrate a step-by-step approach for laparoscopic splenectomy in the setting of recurrent, oligometastatic ovarian cancer. METHODS: We present the case of a 47-year-old female with recurrent, platinum-sensitive high-grade serous ovarian cancer. A computer tomographic scan demonstrated an isolated 1.5 × 1.0 cm recurrence in the splenic hilum. A laparoscopic secondary cytoreduction with splenectomy was p

Updates and Controversies of Robotic-Assisted Surgery in Gynecologic Surgery.

minimally invasive surgery (MIS) is the standard approach to performance of several gynecologic procedures, including hysterectomy, gynecologic cancer staging procedures, myomectomy, pelvic organ prolapse repair, and select adnexal procedures. Robotic-assisted surgery, a computer-based MIS approach, has been adopted widely in the United States and several other countries. Robotics may offer technological and ergonomic benefits that overcome limitations associated with conventional laparoscopy; however, it is not clear that reported claims of superiority translate into improved gynecologic pati

Into the void: a review of postoperative urinary retention after minimally invasive gynecologic surgery.

PURPOSE OF REVIEW: Postoperative urinary retention (POUR) is a common occurrence after minimally invasive gynecologic surgery (MIGS). This review discusses the definition, incidence, evaluation, diagnosis, and treatment of POUR after MIGS. RECENT FINDINGS: Reported incidence of POUR after MIGS ranges from 0.5 to 21%. Active void trials or awaiting spontaneous void after surgery are both viable options for diagnosing POUR, but active void trials appear to be more accurate, quicker, and preferred by patients. The use of enhanced recovery after surgery pathways and minimizing postoperative opioi

A collaborative surgical approach to upper and lower abdominal cytoreductive surgery in ovarian cancer.

BACKGROUND AND OBJECTIVES: Cytoreductive surgery with complete macroscopic resection in patients with ovarian cancer is associated with improved survival. Institutional reports of combined upper and lower abdominal cytoreductive surgery for more advanced disease have described multidisciplinary approaches. We sought to investigate outcomes in patients undergoing cytoreductive surgery in patients with upper and lower abdominal disease at our institution. METHODS: Patients who underwent cytoreductive surgery for ovarian malignancies from 2008 to 2015 were retrospectively identified from an inst

[Endoscopic management of postoperative ureteral wound. Retrospective unicentric study from October 2003 to June 2014].

BACKGROUND: Ureteral wounds are rare with an incidence of 0.5 to 1% of pelvic surgeries. Their supports and their prognosis remain dependant of the period of support and the level of ureteral lesion. The importance of early treatment reduces morbidity and improves patient prognosis. METHODS: A retrospective study from October 2003 to June 2014 was performed in a university hospital using a systematic chart review of patients' urology, digestive surgery, vascular surgery and gynecology. RESULTS: Forty-six wounds were found in 43 patients. The majority of the ureteral wound was found at the pe

Minimally invasive approach to pelvic organ prolapse: a review.

Uterovaginal prolapse, vaginal vault prolapse, and rectal prolapse are pelvic floor support problems that have been traditionally addressed through abdominal, vaginal or perineal approaches. Over the past decade, minimally invasive approaches to these procedures have been reported and refined. We discuss both laparoscopic and robotic techniques for repair of uterovaginal prolapse, vaginal vault prolapse, and rectal prolapse. In addition, we summarize currently available success and complication data associated with each technique.

Failed omental flap vesicovaginal fistula repair subsequently repaired laparoscopically without an omental flap.

BACKGROUND: Conventional wisdom suggests that vascular interposition flaps are helpful in the treatment of all patients with vesicovaginal fistulas. Complicated fistulas are often treated with interposition omental flaps; however, failed omental flap repairs have not been described without the use of another interposition flap. CASE: A 52-year-old woman with a history of cystotomy repair at the time of hysterectomy and a subsequent abdominal omental flap vesicovaginal fistula repair failure underwent a successful repair using a laparoscopic-layered closure of her fistula without omental flap.

Prophylactic Burch colposuspension at the time of abdominal sacrocolpopexy: a survey of current practice patterns.

INTRODUCTION: The purpose of this study was to determine the utilization of the prophylactic Burch procedure with abdominal sacrocolpopexy since the publication of the Colpopexy and Urinary Reduction Efforts (CARE) trial. METHODS: Using an Internet survey, 1,134 members of the American Urogynecological Society (AUGS) were contacted in May 2008 and questioned regarding their practice patterns to prevent de novo stress incontinence after sacrocolpopexy. RESULTS: Two hundred sixty-six responses were obtained for a 23% response rate. Of the 235 respondents actively performing sacrocolpopexies, 1

Use of a portable bladder scanner to reduce the incidence of bladder catheterisation prior to laparoscopy.

The aim of this study was to evaluate the usefulness of estimating bladder volume with a dedicated portable ultrasound device immediately prior to gynaecological laparoscopy. Catheterisation was performed if the estimated volume was greater than 100 ml. Forty consecutive women were studied prospectively. Twenty-six women did not require catheterisation. The procedure was quick and effective in safely reducing the frequency of pre-operative catheterisation.

Compartment syndrome in the well leg resulting from fracture-table positioning.

A variety of patient positioning options exist on modern fracture tables for use during intramedullary nailing procedures. With the advent of interlocking and reconstruction nailing, some of these procedures can be complicated and prolonged. The use of the hemilithotomy position for prolonged intramedullary nailing procedures can result in the development of a compartment syndrome in the uninjured leg, no matter what type of suspension device is used for that leg. The occurrence of this iatrogenic injury in the patient's well leg can be devastating in light of the contralateral fracture. Two p

Bilateral Sacrospinous Vaginal Vault Fixation With Synthetic Mesh Arms: A Novel Technique.

Sacrospinous ligament suspension is used for suspension of apical prolapse; however, it has a high rate of recurrence compared with sacrocolpopexy, and a high rate of pain compared with uterosacral suspension. We developed a novel surgical procedure of bilateral sacrospinous vaginal vault fixation with synthetic mesh arms. We previously demonstrated its safety, and in Supplementary video 1, we describe a step-by-step surgical approach that could be replicated. This technique restores support, creating an anatomically correct midline configuration of the vaginal axis with minimal tension. A ran

Trans-vaginal repair of recurrent rectovaginal fistula with interposition of BIO-A Tissue Reinforcement.

Rectovaginal fistulas (RVFs) represent the majority of all symptomatic leakages after anterior and low anterior resection in women. Conservative management is useful in paucisymptomatic patients with small fistulas but is usually unsuccessful in all other cases. The surgical strategies are various and heavily dependent on the type and extent of anatomic involvement. We present a case of a 51-year-old female with a multi-recurrent rectovaginal fistula that occurred since a laparoscopic sigmoidectomy was performed for a complicated diverticular disease in May 2015. An attempt to close the fistul

Vaginal repair of nonradiogenic urogenital fistulas.

INTRODUCTION AND HYPOTHESIS: In developed countries urogenital fistulas are rare and usually a complication of surgery or radiation therapy. Surgical repair can be accomplished transvaginally or by laparotomy, laparoscopy, robotic-assisted laparoscopy, or transurethral endoscopy. Closure can be achieved with or without tissue interposition. The vaginal approach is the least invasive and a variety of techniques with or without tissue interpositions and flaps have been described. This study reviews surgical approaches and techniques for the repair of nonradiogenic urogenital fistulas. METHODS:

Management of arterial and venous hemorrhage during sacrospinous ligament fixation: cases and review of the literature.

INTRODUCTION AND HYPOTHESIS: Sacrospinous ligament fixation (SSLF) for pelvic organ prolapse repair can incur significant intraoperative hemorrhage. Management of vascular injury is challenging because of limited visualization of the surrounding pararectal space and is not well described in the literature. METHODS: We evaluate cases of intraoperative venous and arterial hemorrhage during SSLF. Based on a review of the literature, we present a systematic approach to the treatment of venous and arterial hemorrhage associated with SSLF. RESULTS: Vascular injury to the hypogastric and pudendal v
