Placental Dysfunction and Congenital Heart Disease: Investigating the Placenta-Heart Axis.

OBJECTIVE: Concurrent development of the placenta and heart during early gestation suggests a shared biological basis for the co-occurrence of abnormal placentation and congenital heart disease (CHD). This study investigated the association between placental vascular pathology and CHD type. METHODS: A retrospective study at two institutions included CHD (n = 521) and unaffected (n = 122) infants. CHDs were categorized into three groups (left and right ventricular outflow tract obstruction and mixed lesions) based on the anticipated effect of placental venous return streaming toward the fetal …

Heart transplantation in congenital heart disease: long-term outcomes and complications.

PURPOSE OF REVIEW: Heart transplantation referrals in congenital heart disease patients are increasing as this population grows in number, complexity, and disease severity. RECENT FINDINGS: We examine short-term and long-term outcomes data with special attention to single-ventricle patients and multiorgan transplantation, including recent advances in combined heart and liver transplantation. SUMMARY: Given the limited supply of available organs for transplantation, it is necessary to examine heart transplantation outcomes in congenital heart disease patients to better optimize outcomes and e…

Transcatheter management of left-sided valvular heart disease following heart transplantation.

OBJECTIVES: Aortic stenosis (AS) and mitral regurgitation (MR) are rare in heart transplant (HTx) recipients, but their incidence increases with extended post-transplant survival. This study assesses the safety, efficacy and outcomes of transcatheter interventions in this high-risk population. METHODS: An institutional series of HTx patients undergoing a transcatheter aortic valve implant (TAVI) or mitral transcatheter edge-to-edge repair (M-TEER) from March 2015 through April 2024 was analysed retrospectively. Both elective cases and patients in cardiac decompensation/cardiogenic shock were …

A Case of Transient Complete Heart Block During Left Heart Catheterization.

Iatrogenic complete heart blocks are rare but a reported complication of left heart catheterizations in patients with pre-existing right bundle branch blocks. We present the case of an 84-year-old male with a preexisting right bundle branch block who underwent a left heart catheterization for valve replacement evaluation. While attempting to engage the right coronary artery, the catheter instead crossed the aortic valve, causing the patient to become bradycardic to the 20s and hypotensive. The patient had a temporary transvenous pacer inserted and tolerated the rest of the procedure well. The …

GENERATOR HEART FAILURE DataMart: An integrated framework for heart failure research.

BACKGROUND: Heart failure (HF) is a multifaceted clinical syndrome characterized by different etiologies, risk factors, comorbidities, and a heterogeneous clinical course. The current model, based on data from clinical trials, is limited by the biases related to a highly-selected sample in a protected environment, constraining the applicability of evidence in the real-world scenario. If properly leveraged, the enormous amount of data from real-world may have a groundbreaking impact on clinical care pathways. We present, here, the development of an HF DataMart framework for the management of cl…

Heart transplantation after total artificial heart bridging-Outcomes over 15 years.

BACKGROUND: Data are limited on outcomes after heart transplantation in patients bridged-to-transplantation (BTT) with a total artificial heart (TAH-t). METHODS: The UNOS database was used to identify 392 adult patients undergoing heart transplantation after TAH-t BTT between 2005 and 2020. They were compared with 11 014 durable left ventricular assist device (LVAD) BTT patients and 22 348 de novo heart transplants (without any durable VAD or TAH-t BTT) during the same period. RESULTS: TAH-t BTT patients had increased dialysis dependence compared to LVAD BTT and de novo transplants (24.7% vs…

Automatic pediatric congenital heart disease classification based on heart sound signal.

Congenital heart diseases (CHD) are the most common birth defects, and the early diagnosis of CHD is crucial for CHD therapy. However, there are relatively few studies on intelligent auscultation for pediatric CHD, due to the fact that effective cooperation of the patient is required for the acquisition of useable heart sounds by electronic stethoscopes, yet the quality of heart sounds in pediatric is poor compared to adults due to the factors such as crying and breath sounds. This paper presents a novel pediatric CHD intelligent auscultation method based on electronic stethoscope. Firstly, a …

Assessing predicted heart mass size matching in obese heart transplant recipients.

BACKGROUND: Predicted heart mass (PHM) is currently the most reliable metric for donor-recipient size matching in heart transplantation. Undersizing PHM donor-recipient match more than 20% independently predicts reduced survival. However, it is unclear if this is the case in obese recipients, in whom size matching can be challenging. We examined the use of PHM undersized hearts in obese recipients and assessed its impact on survival. METHODS: The United Network for Organ Sharing database was queried for adult patients undergoing heart transplantation from 1995 to 2020. Obese recipients (BMI ≥…

Gated recurrent unit-based heart sound analysis for heart failure screening.

BACKGROUND: Heart failure (HF) is a type of cardiovascular disease caused by abnormal cardiac structure and function. Early screening of HF has important implication for treatment in a timely manner. Heart sound (HS) conveys relevant information related to HF; this study is therefore based on the analysis of HS signals. The objective is to develop an efficient tool to identify subjects of normal, HF with preserved ejection fraction and HF with reduced ejection fraction automatically. METHODS: We proposed a novel HF screening framework based on gated recurrent unit (GRU) model in this study. T…

Outcome following heart transplant assessment in adults with congenital heart disease.

OBJECTIVES: Adults with congenital heart disease (ACHD) are a growing group with end-stage heart failure. We aim to describe the outcomes of ACHD patients undergoing assessment for orthotopic heart transplant (OHT). METHODS: Case notes of consecutive ACHD patients (>16 years) assessed for OHT between 2000 and 2016 at our centre were reviewed. Decision and outcome were reported as of 2017. Data were analysed in three groups: systemic left ventricle (LV), systemic right ventricle (RV) and single ventricle (SV). RESULTS: 196 patients were assessed (31.8 years, 27% LV, 29% RV, 44% SV). 89 (45%) …

Heart Failure Interventions Targeting Impaired Left Ventricles in Structural Heart Disease.

PURPOSE OF REVIEW: Interventional techniques have been developed for a wide spectrum of mechanisms of heart failure (HF), especially in valvular heart disease and cardiomyopathies (ischaemic cardiomyopathy and hypertrophic cardiomyopathy). In this article, we review recent reports on catheter interventions to treat patients with HF. RECENT FINDINGS: Direct modification using the Parachute device and the REVIVENT-TC device for patients with impaired left ventricle with large infarct scars improves geometry and haemodynamic efficiency, resulting in a reduction of HF symptoms. Interatrial shunt …

Acute Heart Failure Registry: Risk Assessment Model in Decompensated Heart Failure.

BACKGROUND: Heart failure (HF) is a highly prevalent syndrome. Although the long-term prognostic factors have been identified in chronic HF, this information is scarcer with respect to patients with acute HF. despite available data in the literature on long-term prognostic factors in chronic HF, data on acute HF patients are more scarce. OBJECTIVES: To develop a predictor of unfavorable prognostic events in patients hospitalized for acute HF syndromes, and to characterize a group at higher risk regarding their clinical characteristics, treatment and outcomes. METHODS: cohort study of 600 pat…

Heart rate turbulence predicts ICD-resistant mortality in ischaemic heart disease.

AIMS: In high-risk patients, implantable cardioverter-defibrillators (ICDs) can convert the mode of death from arrhythmic to pump failure death. Therefore, we introduced the concept of 'ICD-resistant mortality' (IRM), defined as death (a) without previous appropriate ICD intervention (AI), (b) within 1 month after the first AI, or (c) within 1 year after the initial ICD implantation. Implantable cardioverter-defibrillator implantation in patients with a high risk of IRM should be avoided. METHODS AND RESULTS: Implantable cardioverter-defibrillator patients with ischaemic heart disease were in…

Heart transplantation for end-stage heart failure due to cardiac sarcoidosis.

BACKGROUND: Cardiac sarcoidosis with end-stage heart failure has a poor prognosis without transplantation. The rates of sarcoid recurrence and rejection are not well established after heart transplantation. METHODS: A total of 19 heart transplant recipients with sarcoid of the explanted heart were compared with a contemporaneous control group of 1,050 heart transplant recipients without cardiac sarcoidosis. Assessed outcomes included 1st-year freedom from any treated rejection, 5-year actuarial survival, 5-year freedom from cardiac allograft vasculopathy (CAV), 5-year freedom from nonfatal ma…
