Natural history of trisomy 13.

The poor prognosis of patients with trisomy 13 has long been accepted and has been ascribed to brain and heart malformations. It has been suggested, however, that the long term survival is better than was previously thought and that cardiac surgery may be justified. This population based study reviews the incidence, antenatal diagnosis, spectrum of survival from congenital heart disease, and mode of death for patients with trisomy 13 in the Northern Health Region from 1985 to 1992. There was an observed prevalence at birth of 0.049/1000 live births and an expected prevalence, allowing for ante

[Natural history of duodenal ulcer].

The authors have studied the natural history of duodenal ulcer from published data, in an attempt to determine precisely the evolutive potential of this disease both spontaneously and during its treatment. They deal successively with the risk of complication and the short- or long-term influence of medical and surgical treatment on the occurrence of exacerbations and complications. The role played by Helicobacter pylori in healing of the ulcer and in the long-term course of the ulcerous disease, as well as the problems concerning its duration and mortality, are discussed. The data clearly show…

Cancer control by family history.

Family history can be used to identify persons at high cancer risk and to target appropriate preventive and therapeutic measures. First-degree female relatives of women with breast cancer (n = 600) are at increased risk for breast cancer (RR: 1.7.95% CI: 1.4-1.9). First-degree relatives of colorectal cancer patients (n = 184) are at increased risk for colorectal cancer (RR: 1.8.95% CI: 1.4-2.3). Families with hereditary cancer syndromes are identified. The family history approach can be used in every part of the world, and it should become a part of future cancer control activities.

Early history of renal rickets.

The first reports of renal osteodystrophy came from Europe in the late 19th century. A case report by Davies Colley in 1883 probably represented the first ever description of renal rickets. Although these reports were mostly descriptive with little or no discussion of the pathophysiological relationship, some of the recommendations for treatment were amazingly insightful and would still be valid by today's standards. The significance of dietary protein/phosphate restriction with vitamin D supplementation was already emphasized in these early days. Studies into the pathophysiology of renal rick

History of hypertrophic osteoarthropathy (HOA).

Hypertrophic osteoarthropathy (HOA) was first described in 1868 as "hyperostosis of the entire skeleton". It has also been mistaken for acromegaly. In 1887-1888 the first description was given of a dermatological disorder "cutis verticis girata" which coexisted with the peculiar bone anomalies of HOA. In 1890 HOA was named "osteoarthropatie hypertrophiante pneumique". Later a distinction was made between the rare idiopathic (or primary) form, also called "pachydermoperiostosis" and the more common secondary form due to concomitant disorders involving the lungs and pleura. The primary form usua

History of postviral fatigue syndrome.

In writing a history of any illness there is always a dilemma whether to attempt the story of the condition 'itself', the medical attempts to define its nature, or to glimpse it via our changing reactions. The easiest is a straightforward account of the attempts of scientists to solve a problem--the classic medical detective story. However, this is often more fiction than fact. Medicine rarely moves smoothly from ignorance to knowledge, but often in a more circular fashion. A historical approach is thus not solely a record of who did what, but also contributes to our understanding of the probl…

Looking at medical history: vaccination.

Controversy with regard to medical procedures is not a new phenomenon. Two hundred years ago, vaccination against smallpox was both derided and acclaimed. The controversy engendered then can be "pictured" through the work of such graphic artists as James Gillray and Isaac Cruikshank.

Patellar subluxation. A recent history.

For 15 years (1955 to 1970), I was plowing "new ground" pretty much by myself. When you plow behind an old mule and hit a stump hole full of cottonmouth moccasins, you have to get on the move in a hurry and figure out some solution to the predicament or you won't get your corn planted. So, I was not treating the overweight, knock-kneed, loose-jointed, teenaged girl, but the young boy or girl who thought that he or she was an athlete and had dreams of being a star. If rehabilitation did not work, then I thought a lateral release, an Elmslie, and nothing short of a full reconstruction would solv

Sternal fractures--the natural history.

A five-year retrospective study was undertaken to analyze the natural history of sternal fractures and to scrutinize the current method of management. Sixty-six patients with documented sternal fractures were evaluated. The most common mechanism of injury was motor vehicle accidents (59%). Most of these victims did not wear seat belts. Thirty-five patients (53%) had associated injuries, of which rib fractures were the most common. Thirty-one patients (47%) had sternal fractures with no associated injury. The incidence of myocardial contusion in our study was 18%. Of the patients without associ…

Natural history of fetal ventriculomegaly.

The natural history of in utero ventriculomegaly was defined by a retrospective review of the outcome of 47 fetuses evaluated during a 5-year period by the Fetal Treatment Program at the University of California. In 20 fetuses, a diagnosis of ventriculomegaly associated with other severe abnormalities was made early in pregnancy. Termination of pregnancy was elected in 19 of 20 cases, and no fetus survived. In five fetuses, the diagnosis was made late in pregnancy and was associated with severe abnormalities. Fetuses were handled in a routine obstetric fashion and none survived. Of the other 2…

Listeriosis--history and actual developments.

Although apparently observed before, the history of listeriosis dates back approximately 60 years. First known as a cause of epidemics and sporadic cases in some 50 species of animals, the disease appears now with increased frequency among human populations at risk. The causative agent Listeria monocytogenes is primarily a psychrophilic soil-borne bacterium with a wide pathogenic potential thus provoking primarily septicemia, meningitis and intrauterine infections. Recent observations indicate certain types of food being the principle vehicle for transmission of human listeriosis. This would p…

Natural history of atrial fibrillation.

The clinical significance of atrial fibrillation was analyzed in cases with chronic or acute heart disease and its significance at the acute and chronic stages of the disease was investigated in various disease groups. The types of disease, number of patients, and incidence of atrial fibrillation were: atrial septal defect (92, 14.1%), mitral valve disease (128, 79.7%), nonrheumatic valvular disease (32, 56.2%), aortic regurgitation (71, 2.8%), aortic stenosis (10, 10.0%), hypertrophic cardiomyopathy (181, 11.6%), dilated cardiomyopathy (111, 37.8%), acute myocardial infarction (823, 9.0%), he

History of antipyretic analgesic therapy.

The use of naturally occurring plant materials for the relief of pain dates back to 3,000 B.C., although rapid advances in antipyretic analgesic therapy have been made more recently. Salicylic acid was synthesized in 1860, and the pyrazolone group, first represented by antipyrine, in 1883. Phenacetin was developed in 1886. Acetaminophen has been in use since the 1890s.

Natural history of breast pain.

258 patients with breast pain were restudied 2 to 7 years after initial assessment in a special mastalgia clinic. Pain persisted at follow-up in 65% of patients. Mastalgia was cyclical in two-thirds. Mean duration of pain in patients experiencing complete relief before follow-up examination was 6.8 years, while duration of pain persisting at follow-up ranged from 2 to 30 years. In patients who had relief or substantial improvement in pain, the improvement was spontaneous in 22% and resulted from a hormonally related event--menopause, pregnancy, or use of oral contraceptives--in the remainder.
