BACKGROUND: Imaging and postmortem studies indicate that abnormalities in monoaminergic neurotransmission contribute to cognitive impairment in Parkinson's disease (PD). However, it remains uncertain if cerebrospinal fluid (CSF) monoamine metabolites can serve as biomarkers of cognitive decline in early PD.
OBJECTIVE: We therefore examined associations of CSF 3,4-dihydroxyphenylacetic acid (DOPAC), homovanillic acid (HVA), 3,4-dihydroxyphenylglycol (DOPEG), and 5-hydroxyindoleacetic acid (5-HIAA) with baseline Montreal Cognitive Assessment (MoCA) scores and with longitudinal rates of change.

This is a report of the concentrations of DA, DOPAC, HVA, 5-HT, and 5-HIAA in the brain of spontaneously diabetic male Wistar rats. These rats showed a marked increase in blood and urine glucose, polydipsia, polyuria, and weight loss that had an onset 11-23 days earlier. Controls were litter mates with no hyperglycemia, glucosuria or weight reduction. HVA and 5-HIAA in CSF were determined in five patients with non-fluctuating progressive dystonia before and after treatment with L-Dopa. The response to L-Dopa was unequivocally favourable in one patient. The HVA-level in her CSF was significantly reduced before treatment, whereas in the other four non-responding patients the pre-treatment HVA was normal. It is postulated, that the biochemical delineation of clinical responsiveness to a neurotransmitter may provide useful clues towards the classification of extrapyramidal disease in childhood.
