A 52 year-old man, treated by neuroleptic drugs, presented with a cow horn injury in the right Scarpa's triangle. The surgical management found no injury of femoral vessels. Surgical hemostasis of superficial vessels was performed. An ileal strangulation appeared three days later. At laparotomy, we found an internal parietal wound. A 30-year-old woman complained of increasing dysmenorrhea and progressive right lower-quadrant pain after cesarean section. A pelvic mass was observed and subsequently a congenital müllerian anomaly was diagnosed upon serial examination. BACKGROUND: A unicornuate uterus with a rudimentary horn is a müllerian anomaly associated with endometriosis and pregnancy complications, including miscarriage, ectopic pregnancy, uterine rupture, preterm labor and malpresentation. OBJECTIVE AND IMPORTANCE: We report the use of a transcortical transventricular approach to a P2 aneurysm, which was located near the choroidal fissure, protruded into the temporal horn, and was considered to be too difficult to approach by the conventional subtemporal route.
CLINICAL PRESENTATION: An 81-year-old woman suddenly developed severe headache with vomiting and subsequently lost consciousness. Computed tomographic scans revealed a diffuse intraventricular hemorrhage and subarachnoid hemorrhage. Cerebral angiography disclosed a saccular aneurysm in the right P2 segment.
