PURPOSE: Matching patients with effective targeted therapies requires timely molecular results, however the challenge today how to embed it into routine clinical workflows in ways that meaningfully affect patient-level treatment decisions.
METHODS: Building on prior work, the authors systematically reviewed laboratory operations, interdepartmental coordination, and administrative processes from two distinct healthcare settings: a United States based academic tertiary care center and a Canadian community hospital. We identified shared themes, context-specific challenges, and operational elemen

BACKGROUND: Child maltreatment, a major public health concern, has long-term neurobiological, psychological, and social consequences. Recognizing, documenting, and reporting suspected cases should be systematic across hospital and community services, but these processes are hindered by inconsistent surveillance systems, uneven professional training, and the lack of standardized data collection tools. Timely follow-up, updated clinical criteria, and structured multidisciplinary training can improve case assessment, traceability, and professionals' ability to report suspected maltreatment. The S

This qualitative study explored the hospitalization journey of people with dementia in Taiwan from their family caregivers' perspectives. Semi-structured interviews with nine caregivers revealed three key themes: care model tensions, psychological/moral burdens, and environmental conflicts. Hospitalization exposes systemic gaps between acute-care efficiency and dementia-friendly needs. Medical social workers play a vital role as humanizing advocates through early psychosocial support, caregiver advocacy, and integrated community transitions.

OBJECTIVE: To identify strategies with tactics that enable point-of-care (POC) testing (medical testing at or near the site of care) to effectively improve outcomes in emergencies, disasters, and public health crises, especially where community infrastructure is compromised.
DESIGN: Logic model-critical path-feedback identified needs for improving practices. Reverse stress analysis showed POC should be integrated, responders should be properly trained, and devices should be staged in small-world networks (SWNs). First responder POC resources were summarized, test clusters were strategized, as

BACKGROUND: Colorectal cancer (CRC) is the second most frequent tumor in developed countries. Since survival from CRC depends mostly on disease stage at the time of diagnosis, individuals with symptoms or signs suspicious of CRC should be examined without delay. Many factors, however, intervene between symptom onset and diagnosis. This study was designed to: 1) Describe the diagnostic process of CRC from the onset of first symptoms to diagnosis and treatment. 2) Establish the time interval from initial symptoms to diagnosis and treatment, globally and considering patient's and doctors' delay,

A survey in New York State revealed 210 medically stable technology-dependent children retained inappropriately in acute-care hospitals. They remained in hospitals primarily because of a shortage of skilled home nursing services in the communities where they lived, family problems that prevented their care at home, and a shortage of beds in appropriate skilled nursing facilities. Most of the children were younger than one year of age, most had disorders of the central nervous system or respiratory system, and most needed respiratory support. The average child needed more than 13 hours of skill

BACKGROUND: Racial disparities disproportionately affect the morbidity and mortality of Black patients with community-acquired pneumonia (CAP). Some scoring systems used in the ICU (eg, Sequential Organ Failure Assessment, APACHE IVa) overpredict mortality in Black patients. However, to our knowledge, the performance of pneumonia severity scoring systems in White and Black adult patients has never been comprehensively evaluated.
RESEARCH QUESTION: Does Black race differentially impact the ability of pneumonia severity scoring systems to predict in-hospital mortality, direct ICU admission, or

BACKGROUND: According to the International Liaison Committee on Resuscitation (ILCOR), the trigger words used by callers that are associated with cardiac arrest constitute a scientific knowledge gap. This study was designed to find hypothetical trigger words in emergency calls in order to improve the specificity of out-of-hospital cardiac arrest recognition.
METHODS: In this descriptive pilot study conducted in a Finnish hospital district, linguistic contents of 80 emergency calls of dispatcher-suspected or EMS-encountered out-of-hospital cardiac arrests between January 1, 2017 and May 31, 20

BACKGROUND: The prevalence of infections caused by extended-spectrum beta-lactamase producing Escherichia coli (ESBL-E. coli) is increasing worldwide, but the setting in which this increase is occurring is not well defined. We compared trends and risk factors for ESBL-E. coli bacteriuria in community vs healthcare settings.
METHODS: We collected electronic health record data on all patients with E. coli isolated from urine cultures in a safety-net public healthcare system from January 2014 to March 2020. All analyses were stratified by healthcare-onset/associated (bacteriuria diagnosed > 48 h

BACKGROUND AND OBJECTIVES: As many rural hospitals continue to close their labor and delivery units and the gap between the demand for and supply of physicians practicing obstetrics continues to grow, this study seeks to understand the barriers and facilitators to delivering obstetric care in rural Wisconsin communities.
METHODS: We conducted interviews with rural-based family medicine and OB-GYN physicians to glean their perspectives on challenges and opportunities that they face in the provision of high-quality obstetrical care and used thematic analysis techniques to generate results.

INTRODUCTION: We present a robust and up-to-date synthesis of evidence on the effectiveness of interventions to prevent and treat newborn infections in low- and middle-income countries (LMICs). Newborn infection prevention interventions included strategies to reduce antimicrobial resistance (AMR), prevention of healthcare-associated infections (HAIs), clean birth kits (CBKs), chlorhexidine cleansing, topical emollients, and probiotic and synbiotic supplementation. Interventions to treat suspected neonatal infections included prophylactic systemic antifungal agents and community-based antibioti

AIM: Children born to mothers with chronic Hepatitis B virus (HBV) infection are at substantial risk of developing chronic HBV-infection without appropriate perinatal post-exposure treatment. This study aimed to explore midwives' and public health nurses' (PHNs) experiences with HBV-post-exposure treatment for infants and identify factors influencing the timeliness of follow-up care.
METHODS: Data were collected through individual and focus group interviews with midwives and PHNs in hospitals and catchment-area municipalities and analysed with thematic analysis.
RESULTS: The study revealed s

BACKGROUND AND METHODS: To characterize acute bacterial meningitis in adults, we reviewed the charts of all persons 16 years of age or older in whom acute bacterial meningitis was diagnosed at Massachusetts General Hospital from 1962 through 1988. We included patients who were admitted after initial treatment at other hospitals.
RESULTS: During the 27-year period, 445 adults were treated for 493 episodes of acute bacterial meningitis, of which 197 (40 percent) were nosocomial. Gram-negative bacilli (other than Haemophilus influenzae) caused 33 percent of the nosocomial episodes but only 3 per

Biliary atresia (BA) is a rare neonatal disease characterized by progressive obstruction of the extrahepatic biliary system, with an etiology that remains poorly understood. This case describes the presentation of BA in a female neonate and underscores the consequences of delayed communication in a rural healthcare setting. The patient was born at a rural hospital, where laboratory tests were obtained but not processed and communicated to her pediatrician in a timely manner. As a result, her first notable clinical finding occurred at the two-week wellness visit, when mild hepatomegaly was susp

BACKGROUND: Transforming care so that people with learning disabilities and/or autistic people can receive support at home rather than in hospital settings is a key priority, but progress has been slow. Despite significant national debate, little previous research has engaged directly with people in hospital, their families or front-line staff to understand the issues from their perspectives.
OBJECTIVES: This research seeks to better understand the experiences of people with learning disabilities and/or autistic people in long-stay hospital settings, their families and front-line staff - usin
