Open reduction of displaced pediatric supracondylar humeral fractures through the anterior cubital approach.

The authors performed a retrospective review of the anterior cubital approach, an alternative exposure for displaced pediatric supracondylar fractures. Anatomic structures that hinder reduction, such as the brachialis muscle or joint capsule, and neurovascular tissues can be identified easily with this approach. Sixty-one children with displaced supracondylar humeral fractures were treated surgically with Kirschner wires using the anterior cubital approach between January 1996 and August 2002. The cohort comprised 35 boys and 26 girls with a mean age of 8.7 years (range 3-13 years). Mean follo…

[Descending intramedullary nailing for the treatment of displaced supracondylar humeral fractures in children].

BACKGROUND: Supracondylar humeral fractures are the most common lesions in childhood. Severely displaced fractures are commonly treated by crossed K-wire osteosynthesis. Such stabilized fractures require a cast and often involve postoperative complications such as iatrogenic lesions of the n. ulnaris and secondary displacements, sometimes leading to a consecutive cubitus varus. This study analyzed possible advantages of elastic stabile intramedullary nailing (ESIN) with postoperative release of motion. PATIENTS AND METHOD: All children aged 1-14 years suffering from a supracondylar humeral fra…

[Fractures of the medial humeral epicondyle in child: preliminary study about 18 cases].

The aim of this work is to study the epidemiological, radiological, clinical views of fractures of the medial humeral epicondyle in child and mainly to appreciate the results of surgical treatment. For this, we looked at again in a retrospective study eighteen (18) files of children with fracture of the medial humeral epicondyle. The average age of children was eleven (11) and the male sex most represented (16 boys versus 2 girls). Medial humeral epicondyle's fracture often occurs by accident in playing (15 cases). Accurate diagnostic of the fracture of the medial humeral epicondyle was rarely

[Unreamed locking intramedullary nailing of humeral fractures--basic evaluation of a patient group].

PURPOSE OF THE STUDY: The aim of the work is to provide an overview of the existing experience in Synthes unreamed humeral nail (UHN). MATERIAL: Between August 1996 and February 2000 we treated by means of UHN a group of 37 patients (12 men and 25 women), average age 55 years (range, 18-87 years) with a fracture of humeral shaft. According to AO classification in 10 patients it was a case of a fracture of proximal humerus involving the shaft (11B-3) and in 27 cases a fracture of the shaft (12 A, B, C). The whole group was operated on by 11 surgeons. METHOD: In 26 cases we used the antegrade

Treatment of humeral shaft fractures by closed fixation using multiple intramedullary Kirschner wires.

BACKGROUND: The purpose of this study is to present the results of closed fixation of humeral shaft fractures with multiple intramedullary Kirschner wires. METHODS: Twenty-nine humeral shaft fractures in patients 14 to 60 years old were treated by closed fixation with multiple intramedullary Kirschner wires. Each patient was placed supine on the table and was administered general anesthesia. The fractured arm was held vertically up by a transolecranon traction, while the C-arm was kept parallel to the ground and opposite to the affected arm. A cortical window was created approximately 1.5 cm

[Intramedullary nailing of humeral shaft fractures--initial experiences with an unreamed humerus nail]].

Interlocking intramedullary nailing has become the preferred method of internal fixation for diaphyseal fractures of the lower extremity. In shaft fractures of the humerus however open reduction and internal fixation with plates and screws is still considered the gold standard for operative treatment. The unreamed humerus nail (UHN) is a new implant for interlocking intramedullary nailing, which allows for stable fixation of humerus shaft fractures and immediate functional after treatment. The UHN could thus become an equal alternative to plate osteosynthesis. From this point of view we analyz

Fractures below the end of locking humeral nails: a report of three cases.

We report three cases of fracture of the humerus that occurred at the tip of interlocking humeral nails inserted previously for humeral shaft fractures. The fractures occurred at 8, 10, and 26 weeks postoperatively, and all occurred through the distal interlocking screws after the patient sustained a rotational force to the arm. All three patients had further surgery, two subsequently healed their fractures, and one is still in active treatment. Orthopaedists should be aware of this complication when choosing interlocking humeral nails for the treatment of humeral shaft fractures.

Displaced proximal humeral fractures. Seven years' experience with a modified Zickel supracondylar device.

A modified Zickel supracondylar rod was used in a prospective study for the fixation of 31 fractures of the proximal humerus in a patient population an average of 55 years of age. The follow-up period averaged 18 months and the overall Neer score at the time of final follow-up examination was 83. Six two-part fractures, ten three-part fractures and fracture dislocations, seven four-part fractures and dislocations, four pathologic fractures, and four humeral shaft fractures were treated with the device. The Neer score did not vary significantly among the groups, although the group with four-par

Fracture of the Humeral Shaft Associated to Elbow Dislocation and Fracture of the Distal-third of the Forearm: Case Report.

Humeral shaft fractures combined with elbow dislocation and fracture of the distal third of the bones of the forearm are uncommon. No description of this simultaneous association has been found in the same patient. Some studies report the association of these two lesions; however, no reports on the three ipsilateral lesions have been found at the PubMed, Lilacs and Bireme databases. The present report describes a case that occurred in a 13-year-old boy who suffered a fall from a height of approximately three meters and was admitted to a trauma hospital. Radiographs showed an ipsilateral humera

Epidemiology and patho-anatomical pattern of 2,011 humeral fractures: data from the Swedish Fracture Register.

BACKGROUND: Humeral fractures are common, but the association between the patho-anatomical fracture pattern and patient characteristics has been inadequately studied and epidemiological knowledge is scarce. Following the introduction of the Swedish Fracture Register (SFR), risk factors for various fractures can be studied, as well as the outcome of different treatments. The objective of this study was to analyse adult humeral fractures in Gothenburg from a descriptive epidemiological perspective. METHODS: All humeral fractures registered in the SFR at Sahlgrenska University Hospital in 2011-2…

Proximal third humeral shaft fractures -- a fracture entity not fully characterized by conventional AO classification.

INTRODUCTION: The retrospective study was made to evaluate the fracture patterns at the proximal humeral shaft for which the long version of a standard proximal humeral nail (PHNLV) has been used. The indication has been decided by the individual surgeons. PATIENTS AND METHODS: Over a five year period 72 consecutive PHNLV cases of an acute fracture were identified and were included in the study. Mean patient age was 68.9 years. Gender ratio was m/f=22/50. 86.1% of the patients fractured their humerus by a fall, the rest by a high velocity accident. We analysed patient comorbidity, ASA score,

Timing Matters: Challenges in Delayed Surgical Management of Pediatric Humeral Medial Epicondyle Fractures.

BACKGROUND: Humeral medial epicondyle fractures comprise 12% of pediatric elbow injuries. Delay in surgical intervention may be more involved than acute surgery due to soft tissue shortening, fragment-induced joint surface damage, elbow stiffness, and ulnar nerve displacement/scarring. There are limited prior case reports on delayed surgical management. The purpose of this study is to provide a descriptive analysis of a series of medial epicondyle fracture patients treated with delayed surgical management. METHODS: Delayed surgical management was defined as any operative repair of the medial
